Prior to filing, the form 990 is presented to the executive director, board treasurer and finance staff for review and approval. The form 990 is then electronically distributed to the board of directors in advance of filing. The board president signs the 990 (or 8879-eo) and the 990 is electronically filed.

Form 990, Page 6, Part VI, Line 12C

Each year, the board members are asked to review the organization's conflict of interest policy. Among other things, the policy makes clear that all decisions of the board, officers, and employees of the organization are made solely on the basis of a desire to promote the best interest of the organization and the public good. The conflict of interest statement requests board members to identify to the best of their knowledge affiliations with organizations that may be potentially related to the financials or other substantive operations of the organization. They are also asked to identify circumstances involving either themselves, or a member of their extended family, that may be construed as a conflict of interest. At the staff level, the organization's personnel also ensure that there are no conflicts of interest when considering the engagement of a new vendor. If a potential conflict is identified, appropriate steps are taken to both assess the nature of the potential conflict and, subsequently, to ensure that the possibility of an actual conflict is mitigated. Such mitigation is managed and the letter and spirit of the conflicts policy are upheld.

The mission of forbes house is empower individuals and families to end the cycle of domestic abuse. The vision of forbes house is to help victims thrive and to foster an end to domestic abuse through education and advocacy.

Form 990, Page 2, Part III, Line 4A

The organization operates forbes house, a temporary residential shelter for victims of domestic violence. Forbes house services and programs include: shelter - forbes house provides safe, supportive shelter 24/7 and can house 30 victims, including children. Helpline - available 24 hours, 7 days a week for crisis intervention and to provide information and referrals. Advocacy - our legal and family advocates assist abused adults and their children by acting as liaisons with the bureaucracies they encounter. Children's program - the children's counselor assesses the needs of the children, assists in their recovery and helps parents enhance their parenting skills. Community education - speakers address victims, schools, businesses, and community groups on domestic violence, emotional and date abuse. Aftercare - our goal is to help those who have left the shelter remain in healthy, nonviolent relationships. Counseling - peer counseling is available for victims as needed with shelter advocates. Long-term professional counseling services are facilitated by shelter advocates to partner victims with the local agency of their choice. 2022 accomplishments: shelter advocacy outreach support 439survivors provided with over 5,000 services 105 survivors sheltered 1,331 crisis calls 260 support group 335 court advocacy 2022's empowerment report is inspiring with much to share regarding efforts in the area of prevention education and expanding partnerships within local police departments and the public schools. We have implemented the use of a new evidence based curriculum and have received feedback about the benefits of our prevention efforts along with additional invitations to expand these efforts. Forbes house trainers are now part of the regularly scheduled crisis intervention team training for new and veteran police officers offered at crossroads health twice a year. Providing officers with education and options on dealing with domestic abuse. While we look ahead to also have a role in the training facility that is to coming to fruition in perry where officers from the region can train and hone their skills annually.

Forbes house is exempt from federal income taxes under section 501(c)(3) of the internal revenue code (the "code") as a charitable organization whereby only unrelated business income, as defined by section 509(a)(1) of the code, is subject to federal income tax. Forbes house currently has no unrelated business income. Accordingly, no provision for income taxes has been recorded.

FORBES HOUSE SERVICES AND PROGRAMS INCLUDE: SHELTER - FORBES HOUSE PROVIDES SAFE, SUPPORTIVE SHELTER 24/7 AND CAN HOUSE 30 VICTIMS, INCLUDING CHILDREN. HELPLINE - AVAILABLE 24 HOURS, 7 DAYS A WEEK FOR CRISIS INTERVENTION AND TO PROVIDE INFORMATION AND REFERRALS. ADVOCACY - OUR LEGAL AND FAMILY ADVOCATES ASSIST ABUSED ADULTS AND THEIR CHILDREN BY ACTING AS LIAISONS WITH THE BUREAUCRACIES THEY ENCOUNTER. CHILDREN'S PROGRAM - THE CHILDREN'S COUNSELOR ASSESSES THE NEEDS OF THE CHILDREN, ASSISTS IN THEIR RECOVERY AND HELPS PARENTS ENHANCE THEIR PARENTING SKILLS. COMMUNITY EDUCATION - SPEAKERS ADDRESS VICTIMS, SCHOOLS, BUSINESSES, AND COMMUNITY GROUPS ON DOMESTIC VIOLENCE, EMOTIONAL AND DATE ABUSE. AFTERCARE - OUR GOAL IS TO HELP THOSE WHO HAVE LEFT THE SHELTER REMAIN IN HEALTHY, NONVIOLENT RELATIONSHIPS. COUNSELING - PEER COUNSELING IS AVAILABLE FOR VICTIMS AS NEEDED WITH SHELTER ADVOCATES. LONG-TERM PROFESSIONAL COUNSELING SERVICES ARE FACILITATED BY SHELTER ADVOCATES TO PARTNER VICTIMS WITH THE LOCAL AGENCY OF THEIR CHOICE. 2022 ACCOMPLISHMENTS: SHELTER ADVOCACY OUTREACH SUPPORT 439SURVIVORS PROVIDED WITH OVER 5,000 SERVICES 105 SURVIVORS SHELTERED 1,331 CRISIS CALLS 260 SUPPORT GROUP 335 COURT ADVOCACY 2022'S EMPOWERMENT REPORT IS INSPIRING WITH MUCH TO SHARE REGARDING EFFORTS IN THE AREA OF PREVENTION EDUCATION AND EXPANDING PARTNERSHIPS WITHIN LOCAL POLICE DEPARTMENTS AND THE PUBLIC SCHOOLS. WE HAVE IMPLEMENTED THE USE OF A NEW EVIDENCE BASED CURRICULUM AND HAVE RECEIVED FEEDBACK ABOUT THE BENEFITS OF OUR PREVENTION EFFORTS ALONG WITH ADDITIONAL INVITATIONS TO EXPAND THESE EFFORTS. FORBES HOUSE TRAINERS ARE NOW PART OF THE REGULARLY SCHEDULED CRISIS INTERVENTION TEAM TRAINING FOR NEW AND VETERAN POLICE OFFICERS OFFERED AT CROSSROADS HEALTH TWICE A YEAR. PROVIDING OFFICERS WITH EDUCATION AND OPTIONS ON DEALING WITH DOMESTIC ABUSE. WHILE WE LOOK AHEAD TO ALSO HAVE A ROLE IN THE TRAINING FACILITY THAT IS TO COMING TO FRUITION IN PERRY WHERE OFFICERS FROM THE REGION CAN TRAIN AND HONE THEIR SKILLS ANNUALLY.

AMONG OTHER THINGS, THE POLICY MAKES CLEAR THAT ALL DECISIONS OF THE BOARD, OFFICERS, AND EMPLOYEES OF THE ORGANIZATION ARE MADE SOLELY ON THE BASIS OF A DESIRE TO PROMOTE THE BEST INTEREST OF THE ORGANIZATION AND THE PUBLIC GOOD. THE CONFLICT OF INTEREST STATEMENT REQUESTS BOARD MEMBERS TO IDENTIFY TO THE BEST OF THEIR KNOWLEDGE AFFILIATIONS WITH ORGANIZATIONS THAT MAY BE POTENTIALLY RELATED TO THE FINANCIALS OR OTHER SUBSTANTIVE OPERATIONS OF THE ORGANIZATION. THEY ARE ALSO ASKED TO IDENTIFY CIRCUMSTANCES INVOLVING EITHER THEMSELVES, OR A MEMBER OF THEIR EXTENDED FAMILY, THAT MAY BE CONSTRUED AS A CONFLICT OF INTEREST.
