The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

MaineHealth's Audit & Compliance Services sends Conflict of Interest (COI) disclosure form requests to over 3,000 Care Team Members (including all Board Members, Officers, and other Senior Leaders) each year. Audit & Compliance Services collects the disclosure forms and reviews each submission for possible reported conflicts to determine if a management mitigation plan is required. The results of the annual COI process are reported to the MaineHealth Executive Compliance Committee, the Audit Committee, Local Health System Presidents, and the Board of Trustees Chairpersons.

Compensation process for Presidents (Line 15a) MaineHealth, a related organization uses an outside firm, Sullivan Cotter, to perform an independent benchmark analysis annually for the President of the Health Community Coalition. The firm meets with the Executive Committee of the Board of Directors of MaineHealth to review the executive compensation report. The Executive Committee then deliberates on MaineHealth's written salary and incentive plan philosophy before making a final decision. All meeting decisions are captured in minutes with appropriate approvals at all levels. Compensation process for officers (Line 15b) Compensation for officers of of the Healthy Community Coalition is part of the independent benchmark analysis performed annually by Sullivan Cotter. All compensation decisions are reviewed and approved by the MaineHealth Chief Executive Officer, Chief Operating Officer and Chief People Officer.

FIN 48 Footnote The Internal Revenue Service has previously determined that the MaineHealth System and its subsidiaries (except Maine Medical Partners (MMP) (a subsidiary of MaineHealth) are organizations as described in Section 501(c)(3) of the Internal Revenue Code (IRC) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the IRC. The MaineHealth System recognizes the effect of income tax positions only if those positions are more likely than not of being sustained. Recognized income tax positions are measured at the largest amount of benefit that is greater than fifty percent likely to be realized upon settlement. Changes in measurement are reflected in the period in which the change in judgment occurs. The MaineHealth System did not recognize the effect of any income tax positions in either 2024 or 2023.

IRS990/Desc0COMMUNITY WELLNESS AND PREVENTION: HCC continues to have a full schedule of outreach events on our mobile health unit where staff provides education and free screenings. We held 75 regularly scheduled and special outreach events with an engagement of over 1,684 community members providing health services and education. Staff setup tables at 16 locations in greater Franklin County including senior housing and food pantries. This past year we provided 228 blood pressure checks, thirty-three cholesterol test and 56 body composition analyses for the community and distributed 52 emergency food and hygiene bags through our mobile health unit.HCC Community Health Worker team has been providing resources each day to our community. The Community Health Worker team received 936 referrals for assistance, with 68% of these referrals (636) focusing on Social Drivers of Health (SDOH) needs, connecting vulnerable individuals to vital resources like housing, transportation and nutritious foods. The outreach and access team continues to work on their patient panels with 475 enrolled onto the Community Health Worker patient panels with 117 proactive outreach phone calls made to patients.HCC staff provided education and free screenings at the following community events and locations:- Franklin County Children's Task Force Festival- Local Food Pantries and Good Shepherd Food Distribution Site - Franklin Memorial Hospital - Hope Harvest Community Garden - Livermore Falls Town Office- French Falls - Home Visits- Local churches and church groups- Eustis Town Office- RSU #9 Schools- Spruce Mountain Schools- SAD #58 Schools- Rangeley IGA- Fotter's Market, Eustis- Edmund's Market, Phillips- Trenten's Market, Kingfield

IRS990/ProgSrvcAccomActy2Grp/Desc0PHYSICAL ACTIVITY/NUTRITION/SUBSTANCE USE PREVENTION (STOP)Funding for HCC's Project STOP grant in FY23 was used to continue to support the ongoing efforts of preventing the onset and reducing the progression of substance use in Greater Franklin County. Funding from the grant specifically impacted youth, young adults, and adults in the service area. These funds support staff time to train two staff members to facilitate the evidence-based program, Botvin Life Skills Training (LST) for both middle school and high school students. The LST program is comprehensive, dynamic and developmentally designed to promote mental health and positive youth development, while helping young people resist substance use and other high-risk behaviors. Several cases of LST students books, LST training sets for trainers, and class supplies to support the implementation of the LST program were purchased. With the purchased materials, HCC facilitated the LST program to all middle school students at Day Mtn. Regional Middle School in Strong, ME. STOP grant funds were used to support the transportation and hotel costs for 11 students from Rangley Lakes Regional School to attend the Annual Maine Youth Action Network's (MYAN) Annual Maine Youth Leadership Conference in November. The conference is designed for young people by young people to learn and talk about issues and topics meaningful to youth. Discussion topics included youth leadership, restorative practices in schools, mental health and wellness, school climate and issues that influence youth substance use rates. Successful efforts for medication safety including distribution of disposal bags, medication safety boxes and social media through Maine CDC's Eye open campaign reaching over 2,600 community members and one hundred seventy safe storage lock bags were disseminated to families, individuals in non-traditional roles and other community members. MaineHealth received the Maine CDC's Maine Prevention Network grant for the Western Public Health District, with HCC as the District Lead. This lead to many new programs for staff.

The MaineHealth System recognizes the effect of income tax positions only if those positions are more likely than not of being sustained. Recognized income tax positions are measured at the largest amount of benefit that is greater than fifty percent likely to be realized upon settlement. Changes in measurement are reflected in the period in which the change in judgment occurs. The MaineHealth System did not recognize the effect of any income tax positions in either 2024 or 2023.
