The nobuts training program was promoted to 13 local tobacco retailers, encouraging employees to take the training in order to reduce the risk of underage tobacco use. A total of 130 informational packets were mailed to landlords and property management sites offering assistance writing policies for alcohol, tobacco, and other substances. Also in the packets were information on programs like party smarter, franktips, and the tobacco helpline. Smokeless school days classes were held at mt. Blue high school and spruce mountain high school for students who have violated the school's smoking policy. Supplemental nutrition assistance program education snap-ed nutrition educators taught 64 classes comprised of 337 sessions. Educators reached 838 individuals making a total of 3,736 contacts. Classes were held at head start programs, adult education, schools, senior housing, health centers, homeless shelters, and the wic office. Classes were focused on nutrition and physical activity to include the additional topics: myplate guidelines, label reading, meal planning, healthy snack preparation, choosing whole grains, cooking skills, grocery shopping on a budget, and growing vegetables in a container garden. Snap-ed nutrition educators implemented a policy and environmental change strategy at kingfield elementary school to provide ta and streamline the expansion and use of their school garden. Let's go the coordinators worked with 40 child centric let's go sites to promote healthy eating and physical activity including 9 child care facilities, 16 schools, 8 out of school programs and 7 health care sites. Let's go child care sites have a reach of 123 children, schools have a reach of 4,562 students, out of school programs have a reach of 1,385 children and healthcare sites have a reach of 7,131 patients. The let's go school nutrition workgroup met regularly to address healthy school meals and involves school nutrition directors from all 5 school districts with a total reach of 4,822 students. In the fall of 2016 eight child care sites, four healthcare practices, six out of school sites, 11 school cafeterias and one school received recognition from let's go for building environments in which unhealthy food choices are limited and opportunities for physical activity are plentiful. Trainings were held for health care professionals around motivational interviewing, for child care providers around creating healthy spaces for children and for out of school programs to train staff about let's go strategies. Carol white pep grant staff from hcc and participating schools have implemented curriculum changes, staff trainings, and physical fitness testing with students throughout the three school districts that are participating in the grant. There has been a positive average increase of approximately 30% for students who engage in 60 minutes or more of daily physical activity and the percentage of students who consume 2 fruits and 3 vegetables daily has doubled. School wellness teams have been engaged to work on improving school wellness policies as per grant expectations and meeting regularly. Physical education equipment has been provided for schools and community partners to offer variety and engage students in active participation of programs. Adventure programs have been offered, bowling, ice skating, snowshoeing, canoeing, bike camp, nutrition education programs, nutrition nights, free ski nights and ski lessons, and summer programs have all been implemented to offer students opportunity to practice what they have learned in school. Equipment has been purchased to also meet special needs of a variety of students as well. Staff have had numerous opportunities for professional development at state and national health and pe conferences in addition to professional development in specialized areas such as yoga and drums alive. After school and summer recreation programs as well as adventure based cooperative learning programs took place throughout

Management, lead prevention, well-water safety and the affordable healthcare act. Wellness events occurred throughout the community with and without the mobile health unit reaching community members throughout the county. The mobile health unit staffed with rns, nurse practitioner, health educators, and medical providers traveled to the towns of rangeley, eustis, kingfield, phillips, farmington, strong, livermore falls, and wilton providing education, free medical screenings and 1:1 and small group instruction. Cancer prevention over 1,000 individuals received cancer prevention education and assistance scheduling preventative care appointments. The main focus of this prevention education was on breast, lung and colorectal cancer. Information about skin and prostate cancer was also provided. Worksite wellness hcc worked with franklin savings bank, stratton lumber, re-energy - stratton, re-energy - livermore, pierce house, orchard park, kingfield lumber, rangeley fitness and northern lights to provide health screenings, flu shot vaccinations, cpr/first aid training, and health counseling regarding chronic disease and cancer prevention education, and tobacco/nicotine recovery support reaching over 200 employees. Work healthy worksite wellness program hmp staff worked with a total of 7 worksites on the work healthy worksite wellness program reaching a total of 358 employees.

Substance abuse prevention hcc disseminated information about marijuana, prescription drugs, tobacco, and alcohol through community partners. The group continues to work on youth messages in order to decrease the amount of adolescent substance misuse. Two successful responsible beverage server trainings were held in 2016. Executed multiple campaigns for prescription drug safety, marijuana risks and prevention, underage alcohol throughout the year in local papers, radio and social media. The substance abuse prevention coalition, comprised of local law enforcement, educators, parents, youth and community members met monthly to work on various youth substance abuse initiatives including party-buster patrols and parents who host the most campaigns.

Staff members, including one certified navigator, provided education about the affordable care act throughout franklin county and assisted community members with health insurance marketplace questions and enrollment. Approximately 170 individuals throughout franklin county received enrollment assistance. Education about the health insurance marketplace was provided through outreach and 1:1 education, posts on healthbeat radio show, public service announcements, facebook and the fchn and hcc websites. Three hcc staff members participated in real balance's health and wellness coach training. All three staff are now completing addition requirements to become national certified.

BLUE HIGH SCHOOL AND SPRUCE MOUNTAIN HIGH SCHOOL FOR STUDENTS WHO HAVE VIOLATED THE SCHOOL'S SMOKING POLICY. SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM EDUCATION SNAP-ED NUTRITION EDUCATORS TAUGHT 64 CLASSES COMPRISED OF 337 SESSIONS. EDUCATORS REACHED 838 INDIVIDUALS MAKING A TOTAL OF 3,736 CONTACTS. CLASSES WERE HELD AT HEAD START PROGRAMS, ADULT EDUCATION, SCHOOLS, SENIOR HOUSING, HEALTH CENTERS, HOMELESS SHELTERS, AND THE WIC OFFICE. CLASSES WERE FOCUSED ON NUTRITION AND PHYSICAL ACTIVITY TO INCLUDE THE ADDITIONAL TOPICS: MYPLATE GUIDELINES, LABEL READING, MEAL PLANNING, HEALTHY SNACK PREPARATION, CHOOSING WHOLE GRAINS, COOKING SKILLS, GROCERY SHOPPING ON A BUDGET, AND GROWING VEGETABLES IN A CONTAINER GARDEN. SNAP-ED NUTRITION EDUCATORS IMPLEMENTED A POLICY AND ENVIRONMENTAL CHANGE STRATEGY AT KINGFIELD ELEMENTARY SCHOOL TO PROVIDE TA AND STREAMLINE THE EXPANSION AND USE OF THEIR SCHOOL GARDEN. LET'S GO THE COORDINATORS WORKED WITH 40 CHILD CENTRIC LET'S GO SITES TO PROMOTE HEALTHY EATING AND PHYSICAL ACTIVITY INCLUDING 9 CHILD CARE FACILITIES, 16 SCHOOLS, 8 OUT OF SCHOOL PROGRAMS AND 7 HEALTH CARE SITES. LET'S GO CHILD CARE SITES HAVE A REACH OF 123 CHILDREN, SCHOOLS HAVE A REACH OF 4,562 STUDENTS, OUT OF SCHOOL PROGRAMS HAVE A REACH OF 1,385 CHILDREN AND HEALTHCARE SITES HAVE A REACH OF 7,131 PATIENTS. THE LET'S GO SCHOOL NUTRITION WORKGROUP MET REGULARLY TO ADDRESS HEALTHY SCHOOL MEALS AND INVOLVES SCHOOL NUTRITION DIRECTORS FROM ALL 5 SCHOOL DISTRICTS WITH A TOTAL REACH OF 4,822 STUDENTS. IN THE FALL OF 2016 EIGHT CHILD CARE SITES, FOUR HEALTHCARE PRACTICES, SIX OUT OF SCHOOL SITES, 11 SCHOOL CAFETERIAS AND ONE SCHOOL RECEIVED RECOGNITION FROM LET'S GO FOR BUILDING ENVIRONMENTS IN WHICH UNHEALTHY FOOD CHOICES ARE LIMITED AND OPPORTUNITIES FOR PHYSICAL ACTIVITY ARE PLENTIFUL. TRAININGS WERE HELD FOR HEALTH CARE PROFESSIONALS AROUND MOTIVATIONAL INTERVIEWING, FOR CHILD CARE PROVIDERS AROUND CREATING HEALTHY SPACES FOR CHILDREN AND FOR OUT OF SCHOOL PROGRAMS TO TRAIN STAFF ABOUT LET'S GO STRATEGIES. CAROL WHITE PEP GRANT STAFF FROM HCC AND PARTICIPATING SCHOOLS HAVE IMPLEMENTED CURRICULUM CHANGES, STAFF TRAININGS, AND PHYSICAL FITNESS TESTING WITH STUDENTS THROUGHOUT THE THREE SCHOOL DISTRICTS THAT ARE PARTICIPATING IN THE GRANT. THERE HAS BEEN A POSITIVE AVERAGE INCREASE OF APPROXIMATELY 30% FOR STUDENTS WHO ENGAGE IN 60 MINUTES OR MORE OF DAILY PHYSICAL ACTIVITY AND THE PERCENTAGE OF STUDENTS WHO CONSUME 2 FRUITS AND 3 VEGETABLES DAILY HAS DOUBLED. SCHOOL WELLNESS TEAMS HAVE BEEN ENGAGED TO WORK ON IMPROVING SCHOOL WELLNESS POLICIES AS PER GRANT EXPECTATIONS AND MEETING REGULARLY. PHYSICAL EDUCATION EQUIPMENT HAS BEEN PROVIDED FOR SCHOOLS AND COMMUNITY PARTNERS TO OFFER VARIETY AND ENGAGE STUDENTS IN ACTIVE PARTICIPATION OF PROGRAMS. ADVENTURE PROGRAMS HAVE BEEN OFFERED, BOWLING, ICE SKATING, SNOWSHOEING, CANOEING, BIKE CAMP, NUTRITION EDUCATION PROGRAMS, NUTRITION NIGHTS, FREE SKI NIGHTS AND SKI LESSONS, AND SUMMER PROGRAMS HAVE ALL BEEN IMPLEMENTED TO OFFER STUDENTS OPPORTUNITY TO PRACTICE WHAT THEY HAVE LEARNED IN SCHOOL. STAFF PROVIDED EDUCATION HEALTH SCREENINGS INCLUDING CHOLESTEROL, GLUCOSE AND BLOOD PRESSURE CHECKS, BREAST HEALTH AND BREAST CANCER PREVENTION SCREENING EDUCATION, CLINICAL BREAST EXAMS, COLORECTAL CANCER PREVENTION AND FIT SCREENING TEST KITS, AND EDUCATED AND COUNSELED INDIVIDUALS ON A VARIETY OF WELLNESS TOPICS INCLUDING HEALTHY EATING, EXERCISE, DISEASE PREVENTION, CHRONIC DISEASE (CONTINUED ON SCHEDULE O) MANAGEMENT, LEAD PREVENTION, WELL-WATER SAFETY AND THE AFFORDABLE HEALTHCARE ACT. WELLNESS EVENTS OCCURRED THROUGHOUT THE COMMUNITY WITH AND WITHOUT THE MOBILE HEALTH UNIT REACHING COMMUNITY MEMBERS THROUGHOUT THE COUNTY. THE MOBILE HEALTH UNIT STAFFED WITH RNS, NURSE PRACTITIONER, HEALTH EDUCATORS, AND MEDICAL PROVIDERS TRAVELED TO THE TOWNS OF RANGELEY, EUSTIS, KINGFIELD, PHILLIPS, FARMINGTON, STRONG, LIVERMORE FALLS, AND WILTON PROVIDING EDUCATION, FREE MEDICAL SCREENINGS AND 1:1 AND SMALL GROUP INSTRUCTION. CANCER PREVENTION OVER 1,000 INDIVIDUALS RECEIVED CANCER PREVENTION EDUCATION AND ASSISTANCE SCHEDULING PREVENTATIVE CARE APPOINTMENTS. THE MAIN FOCUS OF THIS PREVENTION EDUCATION WAS ON BREAST, LUNG AND COLORECTAL CANCER. INFORMATION ABOUT SKIN AND PROSTATE CANCER WAS ALSO PROVIDED. WORKSITE WELLNESS HCC WORKED WITH FRANKLIN SAVINGS BANK, STRATTON LUMBER, RE-ENERGY - STRATTON, RE-ENERGY - LIVERMORE, PIERCE HOUSE, ORCHARD PARK, KINGFIELD LUMBER, RANGELEY FITNESS AND NORTHERN LIGHTS TO PROVIDE HEALTH SCREENINGS, FLU SHOT VACCINATIONS, CPR/FIRST AID TRAINING, AND HEALTH COUNSELING REGARDING CHRONIC DISEASE AND CANCER PREVENTION EDUCATION, AND TOBACCO/NICOTINE RECOVERY SUPPORT REACHING OVER 200 EMPLOYEES. EXECUTED MULTIPLE CAMPAIGNS FOR PRESCRIPTION DRUG SAFETY, MARIJUANA RISKS AND PREVENTION, UNDERAGE ALCOHOL THROUGHOUT THE YEAR IN LOCAL PAPERS, RADIO AND SOCIAL MEDIA.
