Mission and Program Overview

Mission

The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

Major Activities

Activity 2
Community Wellness and Prevention:There were 1,988 health screenings conducted by HCC at community events, clinics, worksites, grocery stores, libraries, food pantries, fairs/festivals, and on the Mobile Health Unit. At those screening events and others, an additional 7,154 total health education encounters were conducted by staff for a total of 9,142 encounters. Health screenings included 1,193 blood pressure tests, 253 cholesterol screenings, 210 bone density screenings, 4 clinical breast exams, 37 prostate exams/urine analysis, and 291 flu shots.Over 5,000 cancer-related educational contacts were made. Education included prevention and early detection for breast, cervical, colon, lung, prostate, oral, and skin cancer. Free clinical breast exams and skin analysis were provided at 30 events in the community and on the Mobile Health Unit. A total of 278 men and women received free care as a result of these efforts. March was Colorectal Cancer Awareness Month and over 200 colorectal cancer awareness pins and educational materials were given to primary care practices, worksites, and the community.

Women's Health:A total of 3,836 women, who ranged in age between 24 and 91, received breast health education. Of those, 625 women were referred for breast cancer screenings/mammograms and support through Susan G. Komen funding, and an additional 1,014 women were referred for breast cancer screenings/mammograms as a result of Avon Foundation Outreach funding. A total of 218 women received free bone density screenings and osteoporosis education. Women also took part in blood pressure and cholesterol screenings; lung, skin, colorectal and oral cancer prevention education; and received flu shots.

IRS990/ActivityOther/Description0Other Programs:Worksite Wellness - In 2011 HCC presented awards to 22 local businesses serving over 1,500 employees through the Work Healthy! program which recognizes those businesses that promote healthy worksite policies and practices including tobacco-free worksite policies, healthy food choices, and the promotion of physical activity. Substance Abuse Prevention - Over 200 parents and youth were educated on substance abuse prevention, physical activity and nutrition at schools, with Cub Scouts groups, and local parks and recreation centers. 80 6th grade kids and their teachers at Cascade Brook School were educated on substance abuse prevention. 22 people were educated on the dangers of high risk drinking during the United Way's May Synergy event. HCC held 2 Responsible Beverage Server Trainings (RBST). 23 servers from 6 establishments passed the post-test and received a certificate from the Maine Department of Public Safety that is valid for five years. Lead Outreach - To help educate community members and prevent lead poisoning among children, 28 lead buckets were distributed to families with children under the age of 6 who are living, or spending more than 10 hours per week, in a pre-1978 home, or those who use lead through work or hobbies.

IRS990/Description0Physical Activity/Nutrition/Tobacco (PANT):The Nutrition and Physical Activity Self-Assessment for Childcare (NAP SACC) program, an obesity prevention intervention that improves the nutrition and physical activity environment, policies, and practices of child care facilities was instituted at the Wilton Head Start and the Farmington Head Start. 15 Head Start program staff and 71 children were involved in the initiative.The Mapping Our Footsteps physical activity initiative resulted in the involvement of 144 youth and 54 adults from Farmington, Wilton, Rangeley, Phillips, Livermore, Livermore Falls, Jay, Kingfield, and Carrabassett Valley taking part in hiking and walking events that connected them to the environment and exposed them to the rich cultural history of the region. There were 192 tobacco-specific encounters made with community members, and 64 worksite encounters. Smokeless School Days were held 5 times educating 9 high school students who were caught using tobacco products on school property.
