Mission and Program Overview

Mission

The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

Major Activities

Activity 2
Physical Activity/Nutrition/Tobacco (PANT):Over 400 cholesterol and blood pressure screenings were conducted as part of the ScoreHealth Screenings. The screenings included education on nutrition, physical activity, and tobacco prevention. Over 75 smoke-free housing informational packets were mailed to landlords to educate them on smoke-free laws and creating healthy homes. A Rural Active Living Assessment was completed for the town of Eustis. Staff has had conversations with school nurses and superintendents in RSU #9 and RSU #58 about enhancing their substance abuse, tobacco and wellness policies. Environmental Funders Network: Staff educated and assisted 66 community members to increase activity through using natural trails in the community.

Women's Health:A total of 4,000 women, who ranged in age between 12 and 95, received breast health education. Women also took part in blood pressure and cholesterol screenings; lung, skin, colorectal and oral cancer prevention education; and received flu shots. Women were assisted with enrolling in the Maine Breast and Cervical Health Program and scheduling physicals and mammograms. Over 30 events were held throughout the year promoting women's wellness and cancer prevention.

IRS990/ActivityOther/Description0Other (Substance Abuse, Community Building via CCHC) Substance Abuse Prevention - Over 1,500 parents and youth were educated on substance abuse prevention including the risks of underage drinking, monitoring your teen, and underage drinking laws. A total of 77 employees at local businesses received Responsible Beverage Trainings through 5 educational events held throughout the community. CCHC - HCC conducted a local foods survey reaching over 200 people in the community. Through a series of events over 300 individuals received instruction about gardening and received a free container garden with a variety of planted vegetables. Container gardening sessions were also taken on the road to Stratton low income housing and Kingfield, Phillips and Strong Senior housing. This education was also provided at the East Wilton WIC office, DHHS, and at the Good Sheppard mobile food van in Farmington and Stratton, and Phillips food pantries. Additionally, over 400 lbs of fresh vegetables were grown at HCC's Hope Harvest Garden and donated to local food pantries in Franklin County. Over 50 community members received a canning starter kit and instruction on proper canning techniques. The Franklin Resource Collaborative (FRC) has convened several meetings with educational speakers on nutrition topics and as a result of these discussions the FRC promoted the Senior Food Mobile in Farmington, Summer Feeding Program sites at Kineowatha Park and RSU #9 Mallett School in Wilton and a meeting was held with the 10 food pantry coordinators from Greater Franklin County.

IRS990/Description0Community Wellness and Prevention:A total of over 8,000 individuals benefitted from HCC outreach activities throughout the year with over 2,500 served through Mobile Health Unit Outreach. Staff provided education on a variety of wellness topics including healthy eating, exercise, disease prevention, and chronic disease management. Wellness events occurred throughout the community with and without the Mobile Health Unit reaching community members throughout the county. The Mobile Health Unit staffed with RNs, a CNA, Health Educators, and Medical Providers traveled to the towns of Rangeley, Eustis, Kingfield, Phillips, Farmington, Strong, Livermore Falls, and Wilton providing education, free medical screenings and 1:1 and small group instruction. Over 3,500 cancer-related educational contacts were made. A total of 205 individuals received a body composition analysis. Over 200 flu vaccines were provided on the Mobile Health Unit and at other public educational events. Cancer Prevention:A total of 3,533 individuals received cancer prevention education. Of this total 375 reported they would make a positive lifestyle change to improve their health and 163 individuals were assisted scheduling preventative care appointments. A total of 15 medical providers and staff were educated about current screening guidelines for colorectal cancer. A total of 9 Home Visits were conducted in the towns of Livermore, Phillips, Rangeley and Carrabassett Valley.Worksite Wellness:HCC worked with Leap Inc., Re-Energy, The Maine State Teachers' Association, Work First, Franklin Chrysler, The Pierce House, Franklin Savings Bank, and Franklin Community Health Network to provide worksite wellness initiatives such as health screenings and education reaching over 500 employees. Additionally, HCC conducted quarterly worksite wellness roundtable for employers to attend to share their successes and challenges. Eleven employers participated. Lead Outreach - To help educate community members and prevent lead poisoning among children, 400 lead packets and informational tip sheets were distributed to families with children under the age of 6 who are living, or spending more than 10 hours per week, in a pre-1978 home, or those who use lead through work or hobbies. Supplemental Nutrition Assistance Program Education:Nutrition Education Sessions were conducted with Seniors at Food Pantries and 4 Cooking Matters Classes were conducted with Youth. A total of 8 Shopping Classes were conducted instructing community members on how to purchase the healthiest foods possible on a budget. Nutrition and Physical Activity Classes were conducted with multiple grades at multiple schools.Community Transformation Grant Programming:During the first 9 months of this program a total of 82 youth participated in low or no cost physical activity programming in Carrabassett Valley, Rangeley, Strong, Kingfield, Wilton, and Farmington. New afterschool programming was established as well as the expansion of several existing programs. A total of 24 individuals were trained to conduct special physical activity programming with youth utilizing established, evidenced based programming.
