Trustees of Dartmouth CollegeHanover, NH501(c)(3)The NH Comprehensive Cancer Collaboration entered into an agreement with the Trustees of Dartmouth to award $7,361 for a NH Cancer Plan implementation initiative, Reduce Barriers to HPV Vaccines in a High-Need Community . This addresses an objective in the NH Cancer Plan to increase the percentage of NH youth who complete the recommended HPV vaccination series. Overall, HPV is thought to cause about 26,800 new cancer cases in the U.S. each year; that is about 5% of total cancer cases-3% of cases in women and 2% of cases in men. Based on what is known about the barriers to improving HPV vaccine initiation and completion rates among both boys and girls, the HPV Stakeholders network will develop interventions that will address two audiences: healthcare providers and parents of adolescents. The intent is to target 2 high-need communities and conduct CDC-based CME trainings for providers with evaluations, and offer resources that will promote the vaccine series with parents of adolescents.$7,361

The Form 990 and all schedules were made available to the board prior to filing the form. The Secretary/Treasurer and Executive Director reviewed the Form 990 and all schedules with accounting staff and return preparer to ensure accuracy prior to filing.

Each member of the Board is required to sign a conflict of interest policy at each yearly meeting. The Board members are asked at each Board meeting if any conflicts have arisen since the prior meeting. The conflict of interest policy is reviewed by the Executive Secretary.

The Executive Committee of the Organization contracts with a third party organization to produce a comparable state hospital association CEO compensation study, which is used for comparative purposes. IRS990/Desc0The Foundation for Healthy Communities currently manages/directs 16 established state and community level projects that exemplify the potential for enhanced outcomes by building relationships and partnerships. Our staff represents a broad level of expertise and experience in community and individual engagement and planning, project management, data systems, clinical-community partnerships, and collective impact. The FHC has collaborated with a broad range of organizations such as NH Department of Health and Human Services, NH Hospice and Palliative Care Organization, NH Council of Churches, and NH Department of Transportation. One of the Foundation's strengths is to engage and lead diverse partners in new forms of collaboration. The FHC serves as the catalyst and backbone for its various initiatives, and assists those partners in collaborations that elevate their collective impact. In addition, the Foundation has the benefit of facilitating program integration for the benefit of the communities served by housing its diverse group of programs and staff in one location, streamlining processes, and opening pathways for collaborations. Descriptions of a few of our current programs are below.NH Partnership for Patients Since starting work in 2011 with the Partnership for Patients, a national initiative aimed at improving patient safety and quality of care, New Hampshire hospitals have achieved significant results in patient safety and quality improvement.Patient & Family Engagement The Foundation continues its work with hospitals to help create a culture of patient and family engagement and establish patient and family advisory councils (PFACs) to enhance the delivery of patient and family-centered care throughout the continuum.NH Health Care Quality Assurance Commission Established in 2005, this Commission was reauthorized in 2016 through 2021, to review and analyze quality of care and patient safety issues in hospitals and ambulatory surgery centers.Rural Quality Improvement NetworkThe Foundation provides a broad array of resources to enhance the sharing of best practices for regulatory compliance, quality improvement and patient experience among New Hampshire's Critical Access Hospitals (CAHs).NH Comprehensive Cancer Collaboration (NHCCC)This partnership of individuals and organizations works together to eliminate cancer, the leading cause of death in the state, by implementing the NH Cancer Control Plan.Healthy Eating Active Living (HEAL NH) This program has many components including Healthy Food Access, Active Living, Active Transportation, and Active Recreation which help improve access to healthy foods and opportunities for physical activity, focusing on communities and populations with the greatest health disparities.NH Health & Equity PartnershipThe Partnership seeks to strengthen health services and educate the public on health equity issues to help build equitable, healthier communities.Community Falls Prevention ProgramThis program has increased access to evidence-based fall prevention programs for older adults and adults with disabilities to reduce falls risks and falls in three regions of NH with high risk of injuries due to falls. Community Health Benefits & Needs AssessmentsThe FHC worked closely with nonprofit hospitals & health systems to advance their knowledge and ability to effectively complete community health needs assessments, report on community benefits and communicate this information to the public. The FHC convened 11 hospitals to jointly conduct a random telephone survey of 2,691 individuals in order to identify community health needs and benefits.Substance Use Disorder TreatmentThe FHC began laying the groundwork to improve access to treatment for people with Substance Use Disorders (SUD) through a new project designed to increase the locations where people seeking Medication Assisted Treatment (MAT) for opioid use can receive care, as well as working with hospital emergency departments to improve conn
