The STRAVA dataset, in conjunction with census demographic data, and data from the NH Department of Transportation and Regional Planning Commissions (RPC), has the potential to be transformed into a spatially explicit decision-support tool for state and local decision-makers. This project will provide an incredibly rich baseline dataset from which the impact of road improvements can be assessed and future HEAL, transportation, and redevelopment efforts can be prioritized, as well as help identify barriers to increased physical activity. Within GIS, model-based tools will be developed to summarize the 2014 STRAVA dataset, compare summary results to 2014 RPC manual counts, analyze spatial patterns in walking and biking and assess their relation$9,496
Manchester Community Health CenterManchester, NH501(c)(3)The NH Comprehensive Cancer Collaboration signed an LOA to award $8,000 to the Manchester Community Health Center to increase the percent of average risk patients who receive colorectal cancer screening using a take home FOBT (fecal occult blood test) or FIT (Fecal immunochemical test) method. This would include a health systems approach to train team members in offering these methods as testing options, purchase of the test cards and placement in each treatment room, provide employee recognition for those who accomplish the FIT/FOBT training checklist, and incorporate electronic health record reminders for providers and patient mail reminders. NH Medication Bridge builds upon and expands community efforts to improve access to free medications from pharmaceutical companies. Healthy Eating Active Living fosters implementation of effective interventions to promote healthy eating and active living which result in a positive and demonstrable impact on health and quality of life. Hospital Preparedness assists in emergency preparedness planning by providing the coordination and funding to upgrade the preparedness of acute care hospitals for responding to incidents such as bioterrorism, outbreaks of infectious disease, and natural or man made disasters within their communities. NH Health Access Network strengthens the NH health care safety net by improving financial access systems based on a uniform approach for low-income, uninsured and under-insured people statewide. The Community Health Improvement program supports new strategies to improve health. Rural Quality Improvement Network helps to improve the quality of care in small rural hospitals. The Health and Equity Partnership works to eliminate disparities based on race, ethnicity, language barriers, etc. to insure all people have access to high quality health services. NH Comprehensive Cancer Collaboration works to prevent and reduce the cancer burden for people in NH. The Foundation for Healthy Communities has entered into a partnership with the Health Research & Educational Trust (HRET) to adopt evidence-based practices that have the potential to reduce harm in ten areas. It will build upon New Hampshire's Initiative to Eliminate Harm by 2015. In addition, FHC and HRET will equip hospital teams with leadership skills, including the areas of change management, facilitation, quality improvement and teamwork and safety culture.
