IRS990/Desc0Youth and Family WellnessNurturing Parenting ProgramThe Nurturing Parenting Program is a 16-week program funded by First 5 San Bernardino to develop nurturing skills and build family resilience and connections to community resources. In 2020-21, a total of 101 parents graduated from the program. Approximately 65% of enrolled parents reduced risks of child maltreatment and successfully completed the program, with an average parent entering the program with a moderate level of risk and exiting with a low level of risk. We continued providing classes and case management through video conference due to the pandemic. For parents who were not interested or able to participate through video conference, we worked with them one on one through phone calls. Facilitators dropped off diapers and class supplies on the doorsteps for parents who could not make it to the office. Parents were given several options and flexibility of class times and were able to complete the program and receive certificates. We also held a drive-through holiday celebration event to give out presents and necessities to families.Mamas Y BebesMamas y Bebes is a 2-month program funded through Riverside County for pregnant or postpartum mothers to learn new coping skills and develop a healthy relationship with the newborn. In 2020-21, a total of 61 participants at risk for depression successfully graduated from the program. The average depression assessment score fell well below the at-risk for clinical depression cutoff after program completion. A participant stated, "This program helped me that I need to take care of my wellbeing as I do for others. It helped me understand the importance of emotional health and its effect on my baby."Earned Income Tax Credit outreachReach Out began the Earned Income Tax Credit (EITC) outreach program to assist in expanding knowledge and access to government credits, in which we reached over 66,000 through outreach, social media, and printed materials with seven partners. The partnership also served 300 families by providing direct EITC education. EITC is a credit claimed on taxes to reduce tax and therefore increase the potential for higher refund. According to the IRS, California had the fourth-lowest EITC participation rate in the nation (73.8%), ranking behind only the District of Columbia, Alaska, and Oregon. We worked tirelessly to educate our communities on eligibility criteria and claiming of the credit with the goal of increasing those percentages.

IRS990/ProgSrvcAccomActy2Grp/Desc0Community Health and PolicyEnvironmental PreventionOne of the most significant achievements this year was the passage of the Smoke-Free Ordinance (applies to multi-unit housing, hotels, public area, outside plaza) in the city of Jurupa Valley, the first ordinance of its kind in the Inland Empire. The HJV team worked hard to support and facilitate this policy in partnership, which was a joint effort of HJV, the Riverside County Tobacco-Free Coalition, and the City of Jurupa Valley. Agencies worked closely with Jurupa Valley Councilman Chris Barajas. Workshops were held, and a "Coffee with Councilman Barajas" events were also held. The ordinance passed the City Council by unanimous vote in February 2021.In addition, Reach Out's Community Health and Policy Program (CHP) participated in the Drug Enforcement Administration's National Take Back Day (NTBD). NTBD ensures the proper disposal of unused prescription medications. Reach Out provided personnel and a booth at community events to collect unused prescription drugs. We also provided data on opioid abuse prevention, information on our various programs, youth engagement opportunities, and information of COVID-19 vaccinations. These events accounted for a total of 266 pounds of collected drugs which were turned over for safe disposal.Healthy Jurupa ValleyHealthy Jurupa Valley is a Healthy Cities initiative implemented in partnership with the city and has five resident-led action teams. Through one of the action teams, HJV implemented the Jurupa Valley Leadership Academy for the second year in a row. In July 2020, the 2nd cohort of leaders, consisting of 22 community members, celebrated their graduation from the academy with a drive-thru event. The leaders received special recognition from Reach Out, the City of Jurupa Valley, the Office of Assemblymember Sabrina Cervantes, and the Office of Representative Mark Takano. The leaders continued their involvement with Reach Out and the Jurupa Valley community through stipend work disseminating Opioid Use Disorder, Medication Assisted Treatment and Listos educational materials. They continue to demonstrate the skills they learned in the academy to become key connectors in their communities.LISTOSThrough 35 outreach events as part of Listos California, we provided information and resources to help individuals, families and communities prepare for disasters like wildfires, earthquakes and floods, as well as the COVID-19 pandemic. Our priority was to reach the Latinx community, people in poverty, older adults, and individuals with disability. In total, the outreach events reached 6,304 community members.Cal Fresh Healthy LivingWith continued funding from Cal Fresh in Riverside county, we focused on environmental and policy changes that address food insecurity. We worked with four food pantries in the cities of Jurupa Valley, Moreno Valley and Corona to assess potential areas of intervention to increase access to healthy food, such as developing a food insecurity screening protocol for healthcare facilities. Because of the continued pandemic, an area of important focus for Reach Out was connecting with food pantries and providing resources so they could become drive-thru facilities. Reach Out also provided needed PPE to their staff, volunteers, and clients.Youth GrantmakersYouth Grantmakers (YGM) develops knowledge and awareness about the importance of philanthropy in our communities. Participants are high school students expressing an interest in leadership. In 2021, three organizations were awarded grants for activities and causes ranging from focusing on at risk youth, scholarships for DACA students, and using a creative writing to help those recovering from mental health issues. Total grants awarded by the Youth Grantmakers amounted to $5,600.00. IHPC provided 83 professional career speakers, 22 health/college campus site visits, technical assistance to nine school-based health clubs, 20 virtual internships, and 315 virtual job shadows. All of the work was possible through continued partnership building through the Inland Health Professions Coalition and the four regional Nexus meetings held monthly with professionals in the healthcare, education, government, and other sectors. Our partnerships include the CSUSB Counseling Center, Riverside University, the University of Redlands, and others.Community Health Worker trainingIn 2020-21, Reach Out presented two Community Health Worker (CHW) programs in unison with Victor Valley College and Local Apprenticeships Uniting a Network of Colleges & High Schools (LAUNCH) Apprenticeship Network. The CHW Training program attracted 109 applicants, including healthcare employees seeking to upgrade their skills, volunteers, and community members. Participants received 80 hours of class training and 40 hours of field experience in community health. There were 46 applicants for the CHW Apprenticeship Program. These were individuals already employed as healthcare workers on either a full or part-time basis. The six-month program, offered at no charge, was approved by the Department of Apprenticeship Standards. The curriculums of both programs covered common diseases in the community, including diabetes, high blood pressure, heart disease and Covid. Participants learned about the latest resources, how to advocate for the patients, communicating, and home-health assistance. The CHW Apprenticeship Program is only the second apprenticeship program recognized and accredited by the Division of Apprenticeship Services (DAS).Youth Advocates for Health AccessOne of the ways in which Reach Out answered the challenge of Covid-19 in 2021 was with our Youth Advocates for Health (YAHA) program. The YAHA program provides learning opportunities for young adults from 16 to 19 years of age who are entertaining a career in in one of the various healthcare fields. Participants were exposed to a variety of skills, including communication, advocacy and professionalism. As part of this year's experience, 17 students spent their summer deployed into the two-county communities community with more than a dozen health equity partners of Reach Out. Their mission was to address vaccine hesitancy and acceptance in various populations. In total, the participants contributed over 900 hours of community service.MINDMIND is a year-long program to increase mental health and career awareness among high school students who are in a health career pathway. In 2020-21, MIND was implemented at Corona High School and provided career speakers; college campus visits to California Baptist University, California State University Fullerton, University of California Riverside, and California State University San Bernardino; and four trainings (QPR, Psychosis Awareness, Healthy Relationships, Vaping and Tobacco Awareness through partnerships with University of Redlands and RUHS). Youth Drop-In Center (THE BLOCK)The purpose of the BLOCK is to provide a safe space for youth who are at risk of homelessness, where they could have access to resources, including food, clothing, and information on shelters. Although the continued COVID-19 restrictions prevented full re-opening of the BLOCK, it had over 170 visits in a short period of time. Staff soon discovered that many of the youth were ambitious and eager to become self-employed as adults. Reach Out expanded the BLOCK, adding the Back to Business program. The partnership promoted the Masks are Medicine contests on Univision and Telemundo television stations, and through press releases and outreach to our partners and Riverside Unified School District. The social media campaigns began August 28th with all the agencies starting to push out messaging on the PSA's and Health Ambassador contests. Each agency sent out messaging a minimum of twice per week regarding COVID safety protocols through November 30, 2020. In addition, the partnership distributed personal protective equipment to community members, including over 891,000 masks. Riverside County Vaccine Equity projectReach Out is coordinating the work of eight community-based organizations to reduce disparities in COVID-19 vaccinations and increase access to resources. Vaccine Equity work focuses on locations that are identified under the Healthy Places Index, and also four primary populations: Native Hawaiian/Pacific Islander, Latinx, African American, and LGBTQ. In 2021, the partnership helped over 10,000 residents make vaccine appointments, contributed 4,500 staff hours at vaccine clinics, and distributed over 500,000 masks to community members.
