IRS990/Desc0HWOMental Health Awareness Training (HWO)The Mental Health Awareness Training (MHAT) Program made significant strides this year in advancing mental health education and suicide prevention across the Inland Empire. The team trained 204 participants through various initiatives, partnering with different organizations. AmeriCorps (HWO)Reach Out's AmeriCorps Community Health Worker (CHW) program continued to grow and welcomed new members and celebrated 15 graduates. Members completed over 1,200 hours of training and service, supporting community events such as IEHP's Grand Slam in Rancho Cucamonga and assisting with outreach events at the Jurupa Valley Resource Center. Several CHWs also began internships at UC RiversideCommunity Health Workers Pacific Empire College (HWO)Pacific Empire College achieved a major milestone this year with the launch and completion of its first Community Health Worker (CHW) cohorts, making an important step in strengthening California's community health workforce. The accredited institution, founded to address the region's growing need for trained health professionals, has already graduated 57 CHWs. The college also launched a summer CHW program in collaboration with Cajon High School's pre-apprenticeship program, welcoming 17 recent graduates who followed the behavioral health pathway. Work-based Learning (HWO)The Work-based Learning program continued to empower youth across the Inland Empire by connecting high school students to real-world healthcare experiences. This year, 1,396 students participated through internships, job shadowing, career presentations, and site visits, completing 85 internships and 3,128 hours of hands-on learning. Health Professions Conference (HWO)More than 600 high school students from across the Inland Empire came together for the annual Health Professions Conference. The conference offered students the chance to explore diverse healthcare careers and connect with industry professionals. Designed to inspire the next generation of health leaders, the conference featured keynote presentations, interactive workshops, and 24 breakout sessions covering fields such as forensic nursing, emergency medicine, kinesiology, and public health.

IRS990/ProgSrvcAccomActy2Grp/Desc0YFWMamas y Bebes (YFW)The Mamas y Bebes program supports mothers in strengthening emotional wellbeing and building healthy connections with their babies. This year, 133 women successfully graduated from the program, gaining valuable tools to manage stress, reduce depressive symptoms, and promote positive parent-child bonding. Nurturing Families (YFW)The Nurturing Families program continued to strengthen families by providing free, evidence-based support for parents and caregivers focused on building positive relationships and effective parenting skills. This year, 11 families proudly graduated after completing all core sessions. Healthy Families of America (HFA) (YFW)The Healthy Families America (HFA) program launched this year, marking an exciting step forward in supporting parents and caregivers during pregnancy and early childhood. This free, voluntary home-visiting program provides personalized one-on-one guidance from trained staff who are fully equipped to help families build strong, nurturing relationships with their children. Familias Unidas (YFW)The Familias Unidas program help strengthen family relationships and communication between parents and teens through culturally responsive classes. This year, Reach Out launched cohorts in San Bernardino and Riverside County, where families came together weekly to learn effective ways to build trust, improve communication, and foster mutual understanding. A total of 41 families successfully graduated from the program. Diaper Bank (YFW)The team strengthened its partnership with Community Action Partnership of San Bernardino to sustain and expand the Diaper Bank initiative, ensuring local families have access to the resources they need. The team distributed a total of 7,660 diapers throughout the year to continue supporting children. Chaffey Student Assistance Program (YFW)The Student Assistance Program continued to strengthen youth mental health and resilience across local schools by providing supportive spaces and resources. This year, student wellness centers received 9,474 visits, with 2,548 unduplicated students seeking support. In addition, the team hosted its 3rd Annual Future Forward Resource Fair, which welcomed more than 900 community members and helped families prepare for the school year. The event distributed over 700 backpacks, provided 100 free haircuts, and offered essential resources. Peace Over Violence (YFW)The Peace Over Violence program, implemented in partnership with the Jurupa Unified School District, empowered students in grades 3-12 with the knowledge and tools to build healthier relationships and resolve conflicts peacefully. Through a variety of workshops, 371 students learned about managing emotions, identifying healthy versus unhealthy friendships and relationships, and using communication as an alternative to violence.

This year, the center served 1,551 community members through express services, workshops, and events expanding access to essential resources such as food assistance, legal aid, and workforce development. JVRC strengthened partnerships by welcoming a Workforce Career Coach on-site weekly and establishing a monthly Family Law Clinic and monthly legal consultations. Helping Justice-involved Reenter Employment (HIRE) (CHEW)The HIRE program provides justice-involved youth with a second chance to pursue their education and career goals through personalized support and workforce development opportunities. This year, 10 participants enrolled in the program, receiving individualized case management. Health Equity (CHEW)The Health Equity team engaged in over 135,000 interactions and distributed more than 195,000 pieces of PPE while raising awareness about key issues such as mental health, heat safety, water conservation, and Individual Taxpayer Identification Number (ITIN) resources. CalFresh (CHEW)The CalFresh Healthy Living program continued to promote nutrition, wellness, and healthy habits. The team supported different elementary schools so that they can maintain their community gardens. They also offered engaging nutrition classes for both adults and youth, providing practical tips for making balanced food choices. Healthy Jurupa Valley (CHEW)Healthy Jurupa Valley (HJV) had an impactful year. The annual Chair's retreat brought together Action Team leaders to reflect, plan, and strengthen partnerships for the year. The Women's Empowerment Conference gathered 63 women for a day of connection, reflection, and inspiration. Community members made two Book Wrapping parties successful, resulting in over 800 books donated to Little Free Libraries and local families. The team maintained more than 20 Little Free Libraries and hosted a Future Water. Jurupa Valley Leadership Academy (CHEW)The Jurupa Valley Leadership Academy launched its sixth cohort this year guiding participants through a ten-week journey of growth, learning, and community engagement. The program concluded with a graduation ceremony celebrating 19 new community leaders. Jurupa Youth United (CHEW)Jurupa Youth United (JYU) had an impactful year filled with learning, service, and empowerment. Youth participated in hands-on prevention and wellness activities including an Opioid Overdose Response Training, Real Colors personality workshop, and financial literacy sessions. They also heard from inspiring guest speakers who shared insights on health careers and civic engagement. JYU members gave back to the community by assembling 100 hygiene kits for families in need and visiting Queen of Hearts Equine Therapy to learn more about the healing power of equine therapy. This year also marked the first-ever Youth Conference to which 34 youth attended.

Latino Health Collaborative (Other)The Latino Health Collaborative continued advancing its mission to improve health access, equity, and quality of life for Latino communities across the Inland Empire. With a total of 108 attendees throughout the year, LHC elevated community voices and built a strong foundation for continued action. Environmental Prevention (Other)Through the Alcohol and Other Drugs (AOD) presentations and opioid awareness trainings, Reach Out expanded its prevention and outreach efforts across the Inland Empire communities. Reach Out distributed 600 fentanyl test strips, 192 medication lock boxes, 259 Deterra bags, and 710 Narcan kits. Community Coalitions (Other)The Fontana Community Coalition (FCC), Partners for Innovative Communities (PIC), and PIC Perris and the Morongo Basin Community Coalition (MBCC) are dedicated grassroots partnerships committed to creating safer and healthier communities across the region. Collectively, the coalitions had a total of 63 attendees. Medication Assisted Treatment Program (Other)Mat is an evidence-based approach that combines FDA-approved medication with counseling and behavioral therapies to help individuals overcome opioid use disorder. Throughout the year, community health workers engaged in 752 community interactions, offering resources, and compassionate guidance to those seeking recovery (22 participants were enrolled) Youth Court (Other)Youth Court is a collaborative effort between Fontana School Police, Fontana Unified School District, and Reach Out. It serves as a voluntary diversion initiative that connects students with wraparound services designed to keep them out of the traditional juvenile justice system. This year, the program engaged 76 students across 14 different schools. Fontana Community Youth Group (Other)The Fontana Community Youth Group was a driving force for youth empowerment and positive change, reaching more than 71 young people across the community. Through 18 engaging sessions, participants developed leadership, communication, and refusal skills while learning about substance use prevention and personal resilience. National Innovative Communities Conference (Other)Reach Out's 15th Annual National Innovative Communities Conference (NICC), themed "Generations", brought together nearly 600 cross-sector leaders and community members. The two-day event united changemakers across the Inland Empire to honor legacy, inspire action, and strengthen a shared vision for a healthier, more equitable Inland Empire.
