Mission and Program Overview

Mission

The mission is to help people prevent or better manage chronic disease through comprehensive nutrition care which combines home-delivered meals and nutrition education as a means to reinforce the connection between informed food choices and improved quality of life.

Major Activities

Activity 2
Good measure meals (gmm) is open hand's fee-for-service program, which offers nutritious, gourmet-quality meal plans to people who wish to manage chronic illnesses such as diabetes, hypertension or obesity, and people looking to improve or maintain a healthy lifestyle through better nutrition. This program extends open hand's mission of comprehensive nutrition care sm across the full socio-economic spectrum, making an even greater impact on our community. Approximately 760 clients received 516,690 meals in 2011 through this program. Additionally, 62 clients received either individual or group nutrition assessments/counseling on a fee for service basis.
Activity 3
Open hand's senior nutrition program provides for one or two freshly cooked health-promoting meals, combined with snacks or supplements, to meet the daily nutrition needs of homebound senior clients and seniors in congregate settings. In 2011, open hand prepared and delivered approximately 447,742 meals to approximately 1,500 senior clients. Additionally, open hand provided nutrition education and wellness coaching to approximately 4,416 seniors in group sessions conducted in senior centers and senior high-rise communities throughout metro-atlanta and provided home nutrition assessments to 358 senior clients, a quadruple increase over 2010. As part of comprehensive nutrition care sm, nutrition education and counseling is offered by our registered dietitians (rds) to open hand clients on a regular basis. Clients receive nutrition education with the meals themselves, and/or at home, neighborhood centers and/or in community settings. Follow-up reinforcement is provided by rds, and may include review sessions, group training, referral(s) to additional support services, and/or phone consultations. Menus are designed by open hand's registered dietitians.

THE SUCCESS OF PHASE I LED TO A GOAL OF DOUBLING THE NUMBER OF PARTICIPANTS IN PHASE II. 2. CARE TRANSITIONS PILOT PROGRAM- TO HELP CREATE A MODEL OF SERVICE THAT WILL REDUCE THE NUMBER OF RE-HOSPITALIZATIONS FOR SENIORS WHO ARE DISCHARGED FROM A HOSPITAL STAY, OPEN HAND SERVED ON A LOCAL COUNTY PLANNING COMMITTEE CHARGED WITH LAUNCHING A "CARE TRANSITIONS PILOT" PROGRAM. THE PILOT ENSURED THAT SENIORS HAVE VITAL SERVICES IN PLACE, SUCH AS MEDICALLY APPROPRIATE NUTRITION, AS SOON AS THEY ARE RELEASED FROM THE HOSPITAL, THUS REDUCING THE RISK FOR RE-HOSPITALIZATION. THE OUTCOMES DERIVED FROM THIS PILOT PROGRAM WILL BE USED TO DEMONSTRATE THE COST BENEFIT TO THE COMMUNITY OF THIS INITIATIVE IN HOPES OF INSPIRING ADDITIONAL FUNDING TO EXPAND THIS PROGRAM. 4. PARTNERING WITH PRIMARY HEALTHCARE PROVIDERS TO STRENGTHEN PATIENT CARE-TO CREATE GREATER PATIENT ACCESS TO OPEN HAND'S MNT SERVICES, LATE IN 2011 OPEN HAND INVESTED IN A KEY NEW STAFF POSITION: A COMMUNITY WELLNESS REPRESENTATIVE. OTHER QUALIFYING INDIVIDUALS MAY INCLUDE DEPENDENT CHILDREN OF ANY OPEN HAND CLIENT, REGARDLESS OF THEIR HEALTH STATUS.
