Activity 2

Basic needs and support families and individualsthis program is designed to provide food services, information and referral and case management support to address both short and long-term needs of low-income and working poor families and individuals in st. Paul, little canada, roseville, lauderdale and falcon heights. Keystone community services provides food services to area residents through three food shelves. This program uses both formal support services and informal support. It increases the families' ability to cope during stressful times, reduces violence in the home, reduces crises for families, and best meets the needs of the family. It also provides advocacy, parenting skills training, supports for basic needs, and supports to improve family stability and community connections. Low income, at risk families receive services: case management services, resources for basic needs, and support to improve parenting skills. Services are culturally relevant and responsive to the individual family needs.2010 accomplishments:*19,073 individuals visited our food shelves and received 1,809,121 pounds of food.*971 families and individuals received case management services.

Activity 3

Case management programhelps people in challenging circumstances to maintain their stability and independence. Keystone community social workers help clients find and connect to resources and services and solve problems that threaten self-sufficiency. The program assists seniors and adults with disabilities through several home- and community-based medical assistance waivers.2010 accomplishments:* 440 seniors and adults with disabilities received case management services.* 102 families were assisted in accessing community services.
