Mission

Community housing network's (chn) mission is to strengthen communities by connecting people to housing resources. This is accomplished by providing affordable housing, administering supportive housing, being a leading provider of homeless prevention and rapid re-housing resources, and offering other housing programs including housing counseling, foreclosure prevention counseling and financial counseling. Through these programs chn serves people who are homeless or at risk of homelessness, people with disabilities, people with low incomes, and other vulnerable people in southeast michigan.

The form 990 is reviewed by management and the finance committee. Upon completion of review, the form 990 is given to each director of the board prior to filing.

Form 990, Part VI, Section B, Line 12C

A. Each director, officer, employee and volunteer is provided with and asked to review a copy of this policy and to acknowledge in writing that he or she has done so. B. Annually each director and officer completes a disclosure form identifying any relationships, positions, or circumstances in which he or she is involved that he or she believes could contribute to a conflict of interest. Employees and volunteers complete a form upon hire/assistance and are asked to update their forms annually. Such relationships, positions or circumstances might include service as a director of or consultant to another nonprofit organization, or ownership of a business that might provide goods or services to community housing network, inc. Any such information regarding the business interests of a director, officer, employee or volunteer, or a family member thereof, shall be treated as confidential and shall generally be made available only to the chair, the president, and any committee appointed to address conflicts of interest, except to the extent additional disclosure is necessary in connection with the implementation of this policy. C. This policy is reviewed annually by each member of the board of directors. Any changes to the policy shall be communicated to all staff and volunteers.

PERMANENT SUPPORTIVE HOUSING ALLOWS RESIDENTS THE ABILITY TO STAY IN THE PROGRAM PERMANENTLY, BUT MANY RESIDENTS MOVE ON TO OTHER HOUSING WHICH REQUIRES FEWER INTENSIVE SUPPORTS. 23 PEOPLE EXITED INTO HOUSING WITH A RENTAL SUBSIDY, 7 PEOPLE EXITED INTO HOUSING WITHOUT A SUBSIDY, AND 1 PERSON EXITING THE PROGRAM BY BECOMING A HOMEOWNER.

AS LEAD AGENCY, THE HMIS ADMINISTRATORS MANAGE AND ASSIST USE OF THE SYSTEM FOR 12 PARTNER-AGENCIES AND MORE THAN 100 END-USERS. PROJECTS AT CHN UTILIZE HMIS TO INPUT CLIENT AND PROJECT DATA THAT IS REPORTED TO FUNDERS AND TO THE COMMUNITY AT LARGE. THE SYSTEM ADMINISTRATORS WORK CLOSELY WITH PARTNER AGENCIES AND THE MICHIGAN COALITION AGAINST HOMELESSNESS TO ENSURE COMPLIANCE AND DATA QUALITY STANDARDS ARE MET. COMMUNITY PROGRAMS:COMMUNITY HOUSING NETWORK'S HOUSING RESOURCE CENTER CALL CENTER RESPONDED TO 20,609 CALLS WITH A WIDE RANGE OF HOUSING ISSUES AND QUESTIONS. AS PART OF THE SCREENING PROCESS AND FOLLOW-UP LINKAGE TO PROGRAMS, 672 INDIVIDUALS/HOUSEHOLDS MET WITH HRC HOUSING SPECIALISTS FOR FACE-TO-FACE MEETINGS TO PURSUE HOUSING RESOURCES. THE PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS (PATH) PROGRAM NOW SERVES THE MOST VULNERABLE AND HARD TO REACH PEOPLE WHO ARE LITERALLY ON THE STREET HOMELESS. THE PATH TEAM REACHED OUT TO 200 HOMELESS INDIVIDUALS, OF WHICH 158 WERE ENROLLED IN THE PROGRAM AFTER MEETING ELIGIBILITY CRITERIA AND PROVIDING CONSENT. THE EMERGENCY SOLUTIONS GRANT PROGRAM ASSISTED 175 INDIVIDUALS IN OAKLAND COUNTY WITH RAPID REHOUSING AND 114 INDIVIDUALS WITH PREVENTION ASSISTANCE. THE OAKLAND COUNTY RAPID REHOUSING GRANT FROM THE U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT SERVED 831 PEOPLE WITH SHORT-TERM ASSISTANCE, COMPRISED OF 460 ADULTS, 375 CHILDREN, AND 6 PEOPLE WHOSE AGE WAS NOT IDENTIFIED. HOSTED 18 VOUCHER BRIEFINGS, FACILITATED BY MSHDA HOUSING AGENTS, WERE HELD AT COMMUNITY HOUSING NETWORK TO ASSIST HOMELESS HOUSEHOLDS IN COMPLETING THEIR APPLICATION FOR THE HOMELESS PREFERENCE HOUSING CHOICE VOUCHER. THE HOUSING COUNSELING PROGRAM PROVIDED HOMEBUYER EDUCATION TO 1,252 HOUSEHOLDS ASSISTING THEM IN THE PURCHASE OF A NEW HOME. DEVELOPMENT:A TOTAL OF 447 INDIVIDUALS AND FAMILIES WERE SUPPORTED THROUGH THE 2018 HOLIDAY SEASON ANNUAL ADOPT A FAMILY PROGRAM. CHN'S PASSIONATE VOLUNTEERS PROVIDED 6,868 HOURS OF SERVICE TO THE ORGANIZATION.HOUSING DEVELOPMENT:IN TOTAL, CHN COMPLETED CONSTRUCTION OF THE FINAL 30 UNITS OF AFFORDABLE RENTAL HOUSING IN THE 60-UNIT JEFFERSON OAKS COMMUNITY, INCLUDING 11 FOR SUPPORTIVE HOUSING WITH PROJECT-BASED RENTAL ASSISTANCE. BY THE END OF DECEMBER 2018, THE 60 UNIT JEFFERSON OAKS DEVELOPMENT WAS FULLY LEASED. IN TOTAL, CHN DEVELOPED 31 UNITS INCLUDING: 30 IN JEFFERSON OAKS, OAK PARK; 1 HOME PROPERTY WAS REHABILITATED AND SOLD TO A HOMEBUYER AT OR BELOW 80% AMI IN OAKLAND COUNTY.
