Mission and Program Overview

Mission

Our mission is to strengthen communities by connecting people to housing resources. We accomplish this by providing affordable housing, administering supportive housing, being a leading provider of homeless prevention and rapid re-housing resources for people who are homeless or at risk of homelessness, people with disabilities and other vulnerable members of the southeastern michigan community, and offering other housing programs including housing counseling, foreclosure prevention counseling and financial counseling.the principal belief supporting everything chn does is that people and communities are more successful when everyone - including people struggling with homelessness, those with disabilities, and those with low incomes - are part of the fabric of stable communities. Our programs resolving housing crises for people in a variety of situations involve solutions as complex and individualized as those we serve.

Community housing network has a finance and audit committee that assumes responsibility for oversight of the audit; the executive committee has the authority to approve the audits and prepares a recommendation of an independent accounting firm to the board for their approval.

Form 990, Part VI, Section B, Line 11B

The form 990 is reviewed by cfo and the finance and audit committee. Upon completion of review, the form 990 is given to each director of the board prior to filing.

Form 990, Part VI, Section B, Line 12C

A. Each director, officer, and employee is provided with and asked to review a copy of this policy and to acknowledge in writing that they have done so. B. Annually each director and officer completes a disclosure form identifying any relationships, positions, or circumstances in which he or she is involved that they believe could contribute to a conflict of interest. Employees complete a form upon hire and are asked to update their forms annually. Such relationships, positions or circumstances might include service as a director of or consultant to another nonprofit organization, or ownership of a business that might provide goods or services to community housing network, inc. Any such information regarding the business interests of a director, officer, employee, or a family member thereof, shall be treated as confidential and shall generally be made available only to the chair, the president, human resources, and any committee appointed to address conflicts of interest, except to the extent additional disclosure is necessary in connection with the implementation of this policy. C. Any significant changes to the policy is reviewed by the governance committee of the board of directors. Any significant changes to the policy shall be communicated to staff/board.

Form 990, Part III, Line 2

As the covid-19 pandemic has passed, covid-related funding was spent down this past year. Demand for chn's housing and housing navigation services remain high, however, due to high housing/rent prices, inflation and a lack of adequate housing inventory. Chn administered its housing stability services (hss) program - designed to help individuals and families who have been financially affected by or during the pandemic - in oakland county. It served 417 people in the county, ensuring families and individuals remain stably housed through case management, housing navigation, eviction prevention/diversion, landlord mediation, and connection to other community resources like housing counseling. Chn was awarded funding to provide housing mobility services to housing choice voucher holders in oakland and macomb counties as part of a msdha pilot project. This program was implemented using a housing stability coordinator position with significant support from existing chn staff and systems. Chn leveraged its expertise and experience around landlord engagement, housing navigation, case management, fair housing, and renter education to support housing choice voucher (hcv) holders. Through the program, the hcv housing stability coordinator provided wrap-around services to help families move to higher-opportunity areas, addressing individual barriers and facilitating smooth transitions. Chn served 201 people through this robust model that empowers families to live in stable housing with more opportunity for upward socio-economic mobility.

EACH COMMUNITY WAS BUILT FROM THE GROUND UP WITH THE USE OF A VARIETY OF FUNDING SOURCES, BUT MAINLY TAX CREDITS, TRANSFORMING PREVIOUSLY BLIGHTED AREAS INTO ATTRACTIVE, AFFORDABLE COMMUNITIES. THERE WERE 758 PEOPLE LIVING IN THESE COMMUNITIES, 358 OF WHOM WERE CHILDREN. IN TOTAL, CHN HAS DEVELOPED, IS THE OWNER OF, AND/OR PROVIDES PROPERTY MANAGEMENT SERVICES FOR 734 LEASES WITHIN 553 PROPERTIES AT SCATTERED SITES IN OAKLAND, MACOMB, WAYNE, AND ST. CLAIR COUNTIES. THIS ALLOWS THE LOW-INCOME RESIDENTS AND RESIDENTS WITH DISABILITIES TO LIVE AS INDEPENDENTLY AS POSSIBLE IN AN AFFORDABLE HOME. OF THESE PROPERTIES, OUR CARING AND COMPASSIONATE PROPERTY MANAGEMENT TEAM AFFORDABLY SERVICES 107 OAKLAND COUNTY LICENSED GROUP HOMES THAT ALLOW OVER 600 PEOPLE TO LIVE AS INDEPENDENTLY AS THEY ARE ABLE.

SUPPORTIVE HOUSING PRIORITIZES THE HEALTH AND WELLBEING OF OUR COMMUNITY MEMBERS AND PROVIDES RESOURCES TO HELP THEM REMAIN HOUSED PERMANENTLY. COMMUNITY HOUSING NETWORK REMAINS THE LARGEST PERMANENT SUPPORTIVE HOUSING (PSH) PROVIDER IN OAKLAND AND MACOMB COUNTIES, AS WELL AS PROVIDING PSH IN WAYNE COUNTY. DURING THE PROGRAM YEAR, CHN SERVED 566 PEOPLE, INCLUDING 177 CHILDREN, IN LEASING ASSISTANCE PROGRAMS AND SHELTER PLUS CARE, HOUSED IN SCATTERED SITE UNITS ACROSS SOUTHEAST MICHIGAN. CHN SERVED 374 PEOPLE, INCLUDING 207 CHILDREN, IN SUPPORTIVE HOUSING UNITS LOCATED AT CHN'S MULTIFAMILY HOUSING COMMUNITIES.

TRAINED STAFF HELP PEOPLE NAVIGATE A SYSTEM THAT CAN SEEM OVERWHELMING, AND IN OUR 2023/24 FY WE HAVE SUCCESSFULLY SCREENED AND PROVIDED RESOURCES TO 12,990 INDIVIDUALS.STREET OUTREACH -THE PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS (PATH) OFFERS STREET OUTREACH, AND WITH IT, A PATH OUT OF HOMELESSNESS FOR THOSE WHO CHOOSE TO TAKE IT. WORKING IN COLLABORATION WITH COMMUNITY PARTNERS, OUR PATH TEAM FINDS PEOPLE WHO ARE LIVING ON THE STREETS (MANY WITH SERIOUS MENTAL ILLNESS) AND ACTIVELY ASSISTS THOSE WHO ARE ELIGIBLE FOR SUPPORTS AND SERVICES INCLUDING: A "HOUSING FIRST" APPROACH TO FIND THEM A HOME, SERVICES FOR THEIR PHYSICAL AND MENTAL HEALTH, AS WELL AS SUBSTANCE USE TREATMENT, SOCIAL SECURITY, HEALTH INSURANCE AND OTHER BENEFITS THEY MAY QUALIFY FOR, AND ASSISTANCE THAT WILL ADDRESS BOTH SHORT- AND/OR LONG-TERM HOUSING NEEDS.THE PATH OUTREACH TEAM MADE CONNECTIONS WITH 167 PEOPLE WHO WERE LIVING ON THE STREET HOMELESS. OF THOSE, 139 WERE PROVIDED WITH SERVICES THROUGH THE PATH PROGRAM, WITH THE ADDITIONAL 28 PEOPLE NOT ELIGIBLE FOR PATH SERVICES WERE CONNECTED TO OTHER RESOURCES AND PROGRAMS.HOMEBUYER & FINANCIAL EDUCATION -COMMUNITY HOUSING NETWORK WANTS FAMILIES TO ACHIEVE THEIR FINANCIAL GOALS AND DREAMS OF HOMEOWNERSHIP. OUR HOMEBUYER, "RENT RIGHT, AND FINANCIAL EDUCATION CLASSES ARE DESIGNED TO HELP INDIVIDUALS AND FAMILIES ACHIEVE THEIR GOALS OF HOUSING STABILITY AND ASSET BUILDING. OUR CERTIFIED HOUSING COUNSELORS WORKED WITH 192 HOUSEHOLDS TO EDUCATE THEM ON CREDIT, DEBT, AND VARIOUS TYPES OF LOANS, CREATING A BUDGET, AND THE RIGHTS AND RESPONSIBILITIES OF BEING A TENANT OR HOMEOWNER. ADDITIONALLY, 52 INDIVIDUALS SEEKING TO PURCHASE A HOME RECEIVED ONE ON ONE COUNSELING.VOLUNTEER ENGAGEMENT -COMMUNITY HOUSING NETWORK UTILIZES BOTH COMMUNITY VOLUNTEERS AND STUDENT INTERNS TO HELP ADVANCE OUR MISSION. DURING OUR 2023/24 FISCAL YEAR, 124 VOLUNTEERS AND INTERNS PROVIDED 2,321 HOURS OR ALMOST 300 FULL WORK DAYS OF COMMUNITY SERVICE AT ENGAGEMENT EVENTS FOR RESIDENTS AT OUR LIVING COMMUNITIES, FUNDRAISERS THAT HELP SUPPORT OUR WORK AND OTHER ACTIVITIES.
