Mission

Community housing network's (chn) mission is to strengthen communities by connecting people to housing resources. This is accomplished by providing affordable housing, administering supportive housing, being a leading provider of homeless prevention and rapid re-housing resources, and offering other housing programs including housing counseling, foreclosure prevention counseling and financial counseling. Through these programs chn serves people who are homeless or at risk of homelessness, people with disabilities, people with low incomes, and other vulnerable people in southeast michigan.

A. Each director, officer, employee and volunteer is provided with and asked to review a copy of this policy and to acknowledge in writing that he or she has done so. B. Additional Explanations

Form 990, Part III, Line 2

The 48-unit grafton townhomes development in eastpointe, with two- and three-bedroom offerings, was completed and fully occupied. Twelve units are set aside for residents in need of supportive services, and five units are fully accessible. A $6.4 million tax credit reservation from the michigan state housing development authority was secured to construct 24 additional long-term, lease-to-purchase homes in chn's unity park rentals development in pontiac. The additional 24 homes, unity park rentals, phase ii and phase iv, will bring the total number of homes chn has constructed in this development to 68. Awarded funding for three additional rapid rehousing grants from the u.s. Department of housing and urban development to provide short-term assistance and one additional permanent supportive housing program.

Community housing network has an audit/finance committee that assumes responsibility for oversight of the audit; the executive committee prepares a recommendation of an independent accounting firm to the board for their approval.

Financial Statement Notes

PART X, LINE 2:

Chn and independent choice have received notification that they qualify as tax-exempt organizations under section 501(c)(3) of the u.s. Internal revenue code and corresponding provisions of state law and, accordingly, are not subject to federal or state income taxes on related income. Unrelated business income may be subject to taxation. Midwest is a single member limited liability corporation, with chn as the sole member. Midwest is treated as a disregarded entity for tax purposes, with all activity included in the sole member's tax filings. Ohp, ohp ii, ohp iii, ohp iv, ohp v, ohp vi and midg are for-profit corporations that act as general partners in the lihtc partnerships. Palmer, unity park, unity park iii, unity park iv and grafton are included in the income tax returns of the members and allocated to members in accordance with their respective partnership agreements. As a result, palmer, unity park, unity park iii, unity park ii and grafton did not incur any federal income tax obligations and the consolidated financial statements as of september 30, 2016 do not include a provision for federal income taxes. Unity park iv and jefferson remain in the project development stage at september 30, 2016. The organization follows guidelines included in standards relating to the accounting for uncertainty in income taxes. The tax effects from an uncertain tax position can be recognized in the consolidated financial statements, only if the position is more likely than not to be sustained on audit, based on the technical merits of the position. The organization recognizes the financial statement benefit of a tax position only after determining that the relevant tax authority would be more likely than not to sustain the position following an audit. For tax positions meeting the more likely than not threshold, the amount recognized in the consolidated financial statements is the largest benefit that has a greater than 50 percent likelihood of being realized, upon ultimate settlement with the relevant tax authority. Based on its evaluation, the organization has concluded that there are no significant uncertain tax positions requiring recognition in its consolidated financial statements. The organization is no longer subject to u.s. Federal income tax examinations by the internal revenue service for the years before september 30, 2013. The organization is not currently under examination by any taxing jurisdiction.

IRS990/ProgSrvcAccomActy3Grp/Desc0COMMUNITY HOUSING NETWORK IS THE LEAD HOMELESS MANAGEMENT INFORMATION SYSTEM (HMIS) AGENCY IN OAKLAND COUNTY AND IS RESPONSIBLE FOR THE COUNTY WIDE IMPLEMENTATION. AS LEAD AGENCY, THE HMIS ADMINISTRATORS MANAGE AND ASSIST USE OF THE SYSTEM FOR APPROXIMATELY 8 PARTNER-AGENCIES AND MORE THAN 100 END-USERS. PROJECTS AT CHN UTILIZE HMIS TO INPUT CLIENT AND PROJECT DATA THAT IS REPORTED TO FUNDERS AND TO THE COMMUNITY AT LARGE. THE SYSTEM ADMINISTRATORS WORK CLOSELY WITH PARTNER AGENCIES AND THE MICHIGAN COALITION AGAINST HOMELESSNESS TO ENSURE COMPLIANCE AND DATA QUALITY STANDARDS ARE MET. HMIS HAS BEEN USED AS A TOOL TO MEASURE OUTCOMES AND EFFECTIVENESS AND HAS SUPPORTED THE COMMUNITY IN BEING LEADERS OF INNOVATION.
IRS990/ProgSrvcAccomActyOtherGrp/Desc0COMMUNITY PROGRAMS:COMMUNITY HOUSING NETWORK'S HOUSING RESOURCE CENTER CALL CENTER RESPONDED TO 24,517 CALLS WITH A WIDE RANGE OF HOUSING ISSUES AND QUESTIONS. AS PART OF THE SCREENING PROCESS AND FOLLOW-UP LINKAGE TO PROGRAMS, 541 INDIVIDUALS/HOUSEHOLDS MET WITH HRC HOUSING SPECIALISTS FOR FACE-TO-FACE MEETINGS TO PURSUE HOUSING RESOURCES. THE PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS (PATH) TEAM REACHED OUT TO 556 HOMELESS INDIVIDUALS, OF WHICH 310 WERE ENROLLED IN THE PROGRAM AFTER MEETING ELIGIBILITY CRITERIA AND PROVIDING CONSENT. THE EMERGENCY SOLUTIONS GRANT PROGRAM ASSISTED INDIVIDUALS IN OAKLAND COUNTY WITH RAPID REHOUSING AND 37 INDIVIDUALS WITH PREVENTION ASSISTANCE. HUD RAPID REHOUSING ASSISTED AN ADDITIONAL 69 INDIVIDUALS WITH TEMPORARY RENTAL ASSISTANCE. THE OAKLAND COUNTY RAPID REHOUSING GRANT FROM THE U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT SERVED 16 FAMILIES WITH SHORT-TERM ASSISTANCE. EIGHT IN-OFFICE PRELIMINARY AND VOUCHER BRIEFINGS WERE FACILITATED WITH ASSIGNED MSHDA HOUSING AGENTS AS PART OF A NEW SYSTEM TO EXPEDITE THE VOUCHER PROCESSING FOR THOSE PULLED FOR MSHDA HOMELESS PREFERENCE VOUCHERS. THE HOUSING COUNSELING PROGRAM PROVIDED HOMEBUYER EDUCATION TO 805 HOUSEHOLDS ASSISTING THEM IN THE PURCHASE OF A NEW HOME. CHN'S FORECLOSURE PREVENTION SERVICES ASSISTED 153 HOUSEHOLDS EXPERIENCING MORTGAGE OR PROPERTY TAX DELINQUENCY. COMMUNITY HOUSING NETWORK WAS CONTACTED BY 159 PEOPLE EXPERIENCING FORECLOSURE IN OAKLAND COUNTY OR MACOMB COUNTY AND WAS ABLE TO SECURE MORTGAGE AND PROPERTY TAX ASSISTANCE OF NEARLY $130,000, WHICH REPRESENTS THE PRESERVATION OF $5 MILLION OF REAL ESTATE VALUE. DEVELOPMENT:A TOTAL OF 692 INDIVIDUALS AND FAMILIES WERE SUPPORTED THROUGH THE 2015 HOLIDAY SEASON ANNUAL ADOPT A FAMILY PROGRAM. VOLUNTEER OPPORTUNITIES TOTALED 8,659 HOURS OF SERVICE, MUCH OF THAT WAS CONTRIBUTED BY FIVE AMERICORPS MEMBERS.HOUSING DEVELOPMENT:CONSTRUCTION WAS COMPLETED ON THE 48-UNIT MULTIFAMILY GRAFTON TOWNHOMES DEVELOPMENT IN EASTPOINTE. ALL OF THE UNITS, TWO AND THREE BEDROOMS, WERE FULLY LEASED BY THE SUMMER OF 2016. TWELVE UNITS ARE SET ASIDE FOR RESIDENTS IN NEED OF SUPPORTIVE SERVICES, AND FIVE UNITS ARE FULLY ACCESSIBLE.GROUND WAS BROKEN IN THE SPRING OF 2016 FOR UNITY PARK RENTALS, PHASE III, 12 LONG-TERM, LEASE-TO-PURCHASE TWO-, THREE-, AND FOUR-BEDROOM HOMES SCATTERED THROUGHOUT THE PONTIAC DEVELOPMENT. SIX OF THE HOMES WILL BE FOR SUPPORTIVE HOUSING WITH PROJECT-BASED RENTAL ASSISTANCE. OCCUPANCY IS EXPECTED IN SPRING 2017.UNITY PARK RENTALS, PHASE II, RECEIVED A $3.2 MILLION HOUSING TAX CREDIT THROUGH MICHIGAN STATE HOUSING DEVELOPMENT AUTHORITY TO CONSTRUCT 12 LONG-TERM, LEASE-TO-PURCHASE TWO-, THREE-, AND FOUR-BEDROOM HOMES SCATTERED THROUGHOUT THE PONTIAC DEVELOPMENT. SIX OF THE HOMES WILL BE FOR SUPPORTIVE HOUSING WITH PROJECT-BASED RENTAL ASSISTANCE. CONSTRUCTION STARTED ON THE HOMES IN THE FALL OF 2016 AND OCCUPANCY IS EXPECTED IN AUGUST 2017.UNITY PARK RENTALS, PHASE IV, RECEIVED A $3.2 MILLION HOUSING TAX CREDIT THROUGH MICHIGAN STATE HOUSING DEVELOPMENT AUTHORITY TO CONSTRUCT 12 LONG-TERM, LEASE-TO-PURCHASE TWO-, THREE-, AND FOUR-BEDROOM HOMES SCATTERED THROUGHOUT THE PONTIAC DEVELOPMENT. SIX OF THE HOMES WILL BE FOR SUPPORTIVE HOUSING WITH PROJECT-BASED RENTAL ASSISTANCE. GROUNDBREAKING IS EXPECTED IN 2017 AND OCCUPANCY IS EXPECTED IN MARCH 2018.JEFFERSON OAKS, RECEIVED A $15.8 MILLION HOUSING TAX CREDIT THROUGH MICHIGAN STATE HOUSING DEVELOPMENT AUTHORITY TO DEVELOP 60 NEW UNITS IN AN ADAPTIVE REUSE OF THE VACANT THOMAS JEFFERSON SCHOOL AND SITE IN OAK PARK. THE SCHOOL WILL OFFER 20 ONE- AND TWO-BEDROOM UNITS IN THE ADAPTIVE REUSE OF THE SCHOOL AND 40 THREE AND FOUR-BEDROOM TOWNHOMES ON THE PROPERTY. DEVELOPMENT IS EXPECTED TO START SPRING 2017 WITH FULL OCCUPANCY EXPECTED IN FALL 2019.AS A CHDO, CHN SOLD TWO HOMES IN THE UNITY PARK, PONTIAC, NEIGHBORHOOD THAT WERE PURCHASED AND REHABILITATED.
