Form 990, Part VI, Line 11B: Form 990 Review Process

The form 990 is initially provided to the treasurer to review. The treasurer then communicates any questions he/she may have regarding the form 990 to the tax return preparer. Once all of the treasurer's questions have been answered/resolved to his/her satisfaction, the form 990 is made available to the governing body to review. Once reviewed, the form 990 is then signed by an officer of the organization.

The conflict of interest policy is presented to each employee as part of the new hire process. Also on an annual basis as new board members are voted in they are presented a copy of the conflict of interest policy.

VENUES OF PERFORMANCE INCLUDE BAKER MEMORIAL UNITED METHODIST CHURCH (ST. CHARLES), ST. MICHAEL CHURCH (WHEATON), FOURTH PRESBYTERIAN CHURCH (CHICAGO), ST. VINCENT DEPAUL (CHICAGO), HOLY FAMILY CHURCH (CHICAGO), ST. MARY CATHOLIC CHURCH (ELGIN), AND GRACE LUTHERAN CHURCH (RIVER FOREST).THE CHOIR HAS PRODUCED TWO CDS ON THE PROTEUS ENTERTAINMENT LABEL AND TWO CDS ON THE MSR LABEL.THE EDUCATIONAL OUTREACH PROGRAM "SING" IS AN ANNUAL WORKSHOP FOR AREA STUDENTS THAT EXPLORES THE ART OF ENSEMBLE SINGING.TWO SCHOLARSHIPS WERE GRANTED TO GRADUATING HIGH SCHOOL STUDENTS IN THE FOX VALLEY AND WHEATON AREAS.THE CHOIR OCCASIONALLY EMBARKS ON INTERNATIONAL TOURS, MOST RECENTLY THEY PERFORMED AT SIX CATHEDRALS OF ENGLAND IN 2017.
