“Part I, Line 1/Part III, Line 1 Briefly describe the organizations mission or most significant activities: Morris Hospital & Healthcare centers exist to provide healthcare in a general short term hospital and delivering primary healthcare services in the outpatient setting. Part III, Line 4 Describe the organizations program service accomplishments: The mission of Morris Hospital & Healthcare Centers is to improve the health of area residents. This is the focal point that steers strategy and decision making and gives meaning and focus to staff as they go about their daily work. As long as our actions and activities are focused on improving the health of area residents, we know that we are doing whats right for our community. We carry out our mission every day by serving the hundreds of patients who walk through our doors for inpatient care, outpatient testing, emergency care, surgery and rehabilitation services. There are so many other ways we carry out our mission daily beyond traditional hospital services, including our free patient transportation service, free exercise classes for seniors, financial assistance program, education and training for pre-hospital providers, student mentoring experiences, student career exploration, free health screenings and education programs. Theres also the behind-the-scenes support that we provide for community social service agencies, including preparing daily meals for clients of Mobile Meals, providing clean linens and meals for the local PADS, meals for St. Vincent Table, and monetary support for the Will-Grundy Medical Clinic and We Care of Grundy County. Delivering quality patient care remains the highest priority at Morris Hospital & Healthcare Centers, as event by a number of significant accomplishments in 2016 including making the full conversion to electronic medication reconciliation and electronic discharge instructions. Additionally, a year-long comprehensive unit based safety program concluded on 2 South that resulted in increased reporting of errors and a decline in C. diff infections. 400 employees took the annual Patient Safety Culture Survey, which provides excellent feedback on employees perception of safety throughout the organization. Morris Hospitals commitment to quality and service is further evident in the creation of a Patient and Family Advisory Council that facilitate open relationships between patients, family members and the healthcare team while identifying opportunities for improvement. Morris Hospital also achieved a milestone for soaring above the 90th percentile for the first time for outpatient services combined on the patient satisfaction surveys. As further evidence of Morris Hospitals commitment to safety, it became one of 72 hospitals in the nation to receive its 10th consecutive A grade for patient safety from Leapfrog Group. Meanwhile, the Morris Hospital Cardiac Rehabilitation program was re-certified by the American Association of Cardiovascular and Pulmonary Rehabilitation. Indisputably, the greatest quality achievement in 2016 was the launch of a daily safety huddle in April 2016. Every day at 10 a.m., the entire management team gathers along with representatives from infection prevention and security to report on any potential safety events that have occurred in the past 24 hours, as well as any potential safety issues anticipated in the coming 24 hours. Morris Hospital continued to expand services in 2016. Two additional physician offices were added in Ottawa to accommodate Morris Hospitals expanding services in this community which now include ob/gyn, cardiologist, pediatrics, and family medicine. Dr. Mike Cichon, a general practitioner in Dwight, completed an affiliation agreement that merged his practice with Morris Hospital & Healthcare Centers, and an addition was completed on the Morris OB/GYN Specialists office resulting in four additional exam rooms to accommodate the growing practice.
