The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

On an annual basis board members and officers are required to submit a conflict of interest statement which is then reviewed by the compliance committee. The compliance committee is responsible for regularly and consistently monitoring and enforcing compliance with the conflict of interest policy.

Compensation for Top Official The process followed for determining compensation of the CEO compensated by this or other related organization is a formal process performed annually by the Executive Committee of the Board of Directors. A related organization uses an outside firm, Sullivan Cotter, to perform an independent benchmark analysis for the CEO of Healthy Community Coalition, along with other executives in the MaineHealth Services system. The firm meets with the Executive Committee to review each executive's benchmark report. The Committee then deliberates on MaineHealth Services' written salary and incentive plan philosophy before making a final decision. All decisions and meetings are captured in minutes and there are appropriate approvals at all levels. Compensation for Officers All compensation is determined via compensation surveys and comparability data. Executive compensation is approved by the Organization's Human Resource department and the CEO of the parent company, MaineHealth Services.

FIN 48 Footnote The Internal Revenue Service has previously determined that the MaineHealth System and its subsidiaries (except Maine Medical Partners (MMP) (a subsidiary of MaineHealth) are organizations as described in Section 501(c)(3) of the Internal Revenue Code (IRC) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the IRC. MMP had significant net operating loss carryovers as of September 30, 2020 and 2019. A valuation allowance has been provided for the entire deferred tax benefit for the net operating losses, due to uncertainty of realization. MMP did not have material taxable income in 2020 and 2019. Accordingly, a provision for income taxes has not been made in accompanying consolidated financial statements. The MaineHealth System recognizes the effect of income tax positions only if those positions are more likely than not of being sustained. Recognized income tax positions are measured at the largest amount of benefit that is greater than fifty percent likely to be realized upon settlement. Changes in measurement are reflected in the period in which the change in judgment occurs. The MaineHealth System did not recognize the effect of any income tax positions in either 2020 or 2019.

IRS990/Desc0COMMUNITY WELLNESS: Community Wellness and PreventionHCC outreach provided through 40 Mobile Health Unit and other outreach events including over 1,140 clinical screenings and educational encounters. Staff provided education and health screenings including cholesterol, glucose and blood pressure checks, breast health and breast cancer prevention screening education, bone density screenings, clinical breast exams, COVID- 19, cancer prevention, interactive nutrition events, cardiovascular, tobacco, substance misuse and (continued at Schedule O)diabetes prevention education, medication safety, lead prevention, and well-water safety. Staff counseled individuals on a variety of wellness topics including healthy eating, exercise, disease prevention, and chronic disease management. Individuals were assisted with the Affordable Healthcare Act, and financial assistance and provider referrals were made. Healthcare providers volunteered their time and provided free screenings and acute care for uninsured and underinsured individuals. Wellness events occurred throughout the community with and without the Mobile Health Unit reaching community members throughout the county. The Mobile Health Unit staffed with RNs, Health Educators, Health & Wellness Coaches, Nutrition Specialists, Insurance Navigators and Medical Providers traveled to the towns of Farmington, Jay, Strong, Carrabassett Valley, Rangeley, Stratton/Eustis, Kingfield, Phillips, Livermore Falls, and Wilton providing education, free medical screenings and 1:1 and small group instruction. In addition, during the months of April - July while outreach services were limited due to COVID-19 restrictions, the Mobile Health Unit was utilized to accommodate operations at Franklin Memorial Hospital for patients who require regularly scheduled lab work. Approximately 561 patients utilized the MHU for lab draws and received educational handouts on a variety of topics including COVID-19. Worksite WellnessHCC worked with Carrabassett Valley Academy, Northern Lights, Re-Energy - Livermore, Pierce House, LEAP, Farmington Ford, Town of Wilton, Daggett's Garage, Maine Woods Mobile Vet, Vet at your door, Robins Flower Pot, Share and Care Food Closet to provide health screenings, flu shot vaccinations, CPR/First Aid training, health counseling regarding chronic disease and cancer prevention education, and tobacco/nicotine recovery support reaching approximately 300 employees. Rural Community Network DevelopmentWith Federal funding HCC created a rural communities stakeholders group called the RHAN (Rural Health Action Network) to assess the community for greatest needs and facilitate a sustainable solution. This multifaceted group, representing all geographic regions and academic and social services organizations of Franklin County, completed a detailed needs assessment and strategic plan. The project selected by this group outlined strategies of collaboration to increase access to medical and social services, in coordination with Primary Care. Additional funding has been applied for.Food InsecurityAs part of the Community Health Needs Assessment implementation plan, food insecurity has been a large focus within the healthcare community. Staff members have worked with MaineHealth, Good Shepherd Food Bank and Franklin Health Practices to expand food insecurity screening. Three practices started screening for food insecurity with youth under 21. The screening rates reached over 90% during this period. Over 300 emergency food bags were distributed during the year to families who screened positive or asked for access to food resources. Basic Needs Staff worked in partnership with the Salvation Army to assist local residents in crisis to meet basic needs. Over 115 families and individuals were assisted with meeting basic needs such as food, emergency lodging, shelter, clothing, personal care products, utilities, fuel, clothing, and connections to services.Access to CareStaff members, including one certifi

Franklin staff worked with 80 youth serving sites to promote healthy eating and active living. These sites include 31 child care facilities, 15 schools, 9 out-of-school programs, 17 school cafeterias, and 7 pediatrics practices throughout Franklin County. Let's Go! Franklin also worked with one adult healthcare practice using the Small Steps Program. Let's Go! child care sites had a reach of 585 children, schools had a reach of 4,788 students, and out-of-school programs (continued at Schedule O)had a reach of 529 children. The Let's Go! Franklin School nutrition workgroup met quarterly and included nutrition directors from all 5 school districts in the county. Workgroup members worked to promote healthy choices in their school cafeterias and support one another as they navigated the task of feeding remote students during COVID-19. Each Let's Go! Site worked with a coordinator to set goals for the year to foster healthy environments and habits for the youth that they serve. Let's Go! Franklin and the Let's Go! home office worked together to provide trainings and resources for champions in all settings.

IRS990ScheduleD/SupplementalInformationDetail/ExplanationTxt0FIN 48 Footnote The Internal Revenue Service has previously determined that the MaineHealth System and its subsidiaries (except Maine Medical Partners (MMP) (a subsidiary of MaineHealth) are organizations as described in Section 501(c)(3) of the Internal Revenue Code (IRC) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the IRC. MMP had significant net operating loss carryovers as of September 30, 2020 and 2019. A valuation allowance has been provided for the entire deferred tax benefit for the net operating losses, due to uncertainty of realization. MMP did not have material taxable income in 2020 and 2019. Accordingly, a provision for income taxes has not been made in accompanying consolidated financial statements. The MaineHealth System recognizes the effect of income tax positions only if those positions are more likely than not of being sustained. Recognized income tax positions are measured at the largest amount of benefit that is greater than fifty percent likely to be realized upon settlement. Changes in measurement are reflected in the period in which the change in judgment occurs. The MaineHealth System did not recognize the effect of any income tax positions in either 2020 or 2019.
