Mission

The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

On an annual basis board members and officers are required to submit a conflict of interest statement which is then reviewed by the compliance committee. The compliance committee is responsible for regularly and consistently monitoring and enforcing compliance with the conflict of interest policy.

Compensation for Top Official The process followed for determining compensation of the CEO compensated by this or other related organizations is a formal process performed annually by the Executive Committee of the Board of Directors. A related organization, MaineHealth (EIN: 01-0431680), uses an outside firm, Sullivan Cotter, to perform an independent benchmark analysis for the CEO of Healthly Community Coalition, along with other executives in the MaineHealth system. The firm meets with the Executive Committee to review each executive's benchmark report. The Committee then deliberates on MaineHealth's written salary and incentive plan philosophy before making a final decision. All decisions and meetings are captured in minutes and there are appropriate approvals at all levels. Compensation for Officers All compensation is determined via compensation surveys and comparability data. Executive compensation is approved by the Organization's Human Resource department and the CEO of the parent company, Franklin Community Health Network.

IRS990/Desc0Physical Activity/Nutrition/Tobacco:259 one-on-one tobacco/nicotine recovery support encounters were provided via Mobile Health Unit events, worksites, the HCC office and telephone. To promote smoke-free environments for youth, HCC staff held four Sidekicks trainings and worked with 9 youth serving entities, resulting in 7 new smoke-free policies. HCC staff worked with area school districts to review and strengthen their existing tobacco policies, resulting in one new tobacco-free policy with the local school district. Working to support the community, staff provided assistance to 7 municipalities and passed one (Continued at Schedule O)recreational smoke-free policy as a result. Staff also worked with eight local property owners resulting in 2 new smokeless housing policies that included marijuana smoke as well as tobacco smoke. Nine buildings were covered under these new policies. In addition to working with housing property owners, staff also worked with three lodging sites, resulting in one new smoke-free lodging policy. The tobacco cessation referral program was introduced to one local social service agency, focusing on increasing referrals to the Maine Tobacco Helpline for low income families in Greater Franklin County, resulting in 4 referrals to the Helpline. Smoke-Free home pledges were distributed at outreach events, resulting in 162 pledges signed. Supplemental Nutrition Assistance Program EducationSNAP-Ed Nutrition Educators taught a total 86 series (classrooms) comprised of 294 sessions(lessons). Educators reached 1,485 individuals making a total of 2,817 contacts. Classes were held at Head Start centers, Adult Education, local schools, senior housing sites, health centers, recreation centers, homeless shelters, churches and at community festivals. Classes were focused on nutrition and physical activity to include the additional topics: MyPlate guidelines, label reading, meal planning, healthy snack preparation, choosing whole grains, cooking skills, grocery shopping on a budget, and growing vegetables in a container garden. Educators are offering new classes at Academy Hill and Spruce Mountain Elementary and Primary School for the first time. Let's Go! The coordinators worked with 69 child centric Let's Go! sites to promote healthy eating and physical activity including 24 child care facilities, 17 schools, 13 school cafeterias, 8 out-of-school programs and 7 health care sites. The coordinators also worked with 1 adult health care center to pilot the Small Steps program. For the 2017-2018 program year Let's Go! child care sites had a reach of 441 children, schools had a reach of 4,871 students, out of school programs had a reach of 517 children and child centered healthcare sites had a reach of over 5,500 patients. The Let's Go! school nutrition workgroup met regularly to address healthy school meals and involved school nutrition directors from 4 school districts. In the fall of 2018 all sites received Let's Go! for building environments in which unhealthy food choices are limited and opportunities for physical activity are plentiful. A professional development training was held for child care providers around physical activity benefits to children and their families.

Additionally, staff worked with Mt. Blue Middle School and Strong Area School to coordinate Red Ribbon Week substance use prevention activities for youth in grades 5 -8. HCC staff presented the results of the Maine Integrated Youth Health Survey (MIYHS) to area school boards, community groups and youth serving organizations in Greater Franklin County. The survey results include past 30-day use of alcohol, marijuana and prescription drugs by youth in 6-12 grade. HCC hosted two chem-free events for youth and families at Titcomb Mountain in Farmington, with 177 youth and adults participating. Additionally substance use prevention information was disseminated throughout the area on the Mobile Health Unit and at fairs and festivals in Greater Franklin County.

Staff provided education health screenings including cholesterol, glucose and blood pressure checks, breast health and breast cancer prevention screening education, clinical breast exams, colorectal cancer prevention and FIT screening test kits, interactive nutrition events, and educated and counseled individuals on a variety of wellness topics including healthy eating, exercise, disease prevention, chronic disease management, lead prevention, well-water safety and the Affordable Healthcare Act. Wellness events occurred throughout the community with and without the Mobile Health Unit reaching community (Continued at Schedule O)members throughout the county. The Mobile Health Unit staffed with RNs, Nurse Practitioner, Health Educators, Health & Wellness Coaches, Nutrition Specialist and Medical Providers traveled to the towns of Farmington, Jay, Rangeley, Stratton/Eustis, Kingfield, Phillips, New Sharon, Strong, Livermore Falls, East Dixfield, and Wilton providing education, free medical screenings and 1:1 and small group instruction. Cancer Prevention:Over 800 individuals received cancer prevention education and assistance scheduling preventative care appointments. The main focus of this prevention education was on breast, lung and colorectal cancer. Information about skin and prostate cancer was also provided. Worksite Wellness:HCC worked with Carrabassett Valley Academy, Franklin Chrysler, Irving Forest Products, Kingfield Wood Products, Northern Lights, Stratton Lumber, Re-Energy - Stratton, Re-Energy - Livermore, Pierce House, WorkFirst, and Rangeley Health and Wellness, to provide health screenings, flu shot vaccinations, CPR/First Aid training, and Maine Department of Transportation and health counseling regarding chronic disease and cancer prevention education, and tobacco/nicotine recovery support reaching approximately 400 employees.
