Form 990, Page 2, Part III, Line 4A

(continued from 990, part iii, line 4a) spruce mountain high school for students who have violated the school's smoking policy. Supplemental nutrition assistance program education: snap-ed nutrition educators taught 53 classes with 901 participants at head start programs, schools, senior housing, health centers, and the wic office. Classes were focused on nutrition and physical activity to include the additional topics: myplate guidelines, label reading, meal planning, healthy snack preparation, cooking, grocery shopping on a budget, and growing vegetables in a container garden. Let's go: the coordinator worked with 29 child centric let's go sites to promote healthy eating and physical activity including 9 child care facilities, 7 schools, 6 out of school programs and 7 health care sites. Let's go child care sites have a reach of 123 children, schools have a reach of 2,205 students, out of school programs have a reach of 1,135 children and healthcare sites have a reach of 3,675 patients. In the spring of 2015 nearly 3,000 in mini-grants was distributed to 17 let's go sites primarily for equipment that promoted physical activity and materials that supported healthy meals, snacks or celebrations or promoted drinking water. In the fall of 2015 seven child care sites, three healthcare sites, three out of school sites and one school received recognition from let's go for implementing all 5 evidence-based priority strategies. The let's go school nutrition workgroup met regularly to address healthy school meals and involves school nutrition directors from all 5 school districts with a total reach of 4,822 students. Carol white pep grant: staff from hcc and participating schools have implemented curriculum changes, staff trainings, and physical fitness testing with students throughout the three school districts that are participating in the grant. There has been a positive average increase of 13% for students who engage in 60 minutes or more of daily physical activity. A new school wellness team has been engaged to work on improving school wellness policies as per grant expectations. Physical education equipment (strength and cardio, and canoes, kayaks and paddleboards) has been provided for schools and community partners. Hcc sponsored a school food service workshop day at mt. Blue rsd. The workshop covered introducing new recipes and cooking techniques for staff in addition to how to advertise to youth. After school and summer recreation programs as well as adventure based cooperative learning programs took place throughout the schools and communities involved in this grant. Community events have been held to involve families in physical activity and nutrition education opportunities such as free ski nights, taste testing nights, sledding days, and bowling nights.

(continued from 990, part iii, line 4b) hcc staff has provided education and free screenings to over 600 individuals at the following events: -children's task force -farmington home and leisure show -farmington homeless shelter -four free ski nights through a mini-grant from the maine bureau of highway, hcc was able to reach teenage youth throughout franklin county to deliver important driving safety messages and offer safe driver teen/parent driving contracts. One text or call could wreck it all promotion was placed in high schools and ran at the narrow gauge cinema before movies. Hcc received a total of 74 signed safe driver teen/parent driving contracts and positive feedback from others who took a safe driving contract to review with their family. Despite the end of the grant, hcc is still receiving requests from the community for safe driving contracts. Substance abuse prevention hcc has been disseminating information about marijuana, prescription drugs, tobacco, and alcohol through community partners and is part of the state marijuana workgroup. The group continues to work on youth messages in order to decrease the amount of adolescent use. The franktips program has been very successful in busting a drug operation. Local law enforcement continued this program, beyond the funding period. 4 successful responsible beverage server trainings were held - one off and three on premise. Ads were submitted to narrow gauge cinema to help educate the community about alcoholscreening.org, prescription drug abuse, and marijuana. Each ad was played for an entire month. The substance abuse prevention coalition, comprised of local law enforcement, educators, parents, youth and community members met monthly to work on various youth substance abuse initiatives including party-buster patrols and parents who host the most campaigns.

(continued from 990, part iii, line 4c) unit reaching community members throughout the county. The mobile health unit staffed with rns, nurse practitioner, health educators, and medical providers traveled to the towns of rangeley, eustis, kingfield, phillips, farmington, strong, livermore falls, and wilton providing education, free medical screenings and 1:1 and small group instruction. A total of 122 individuals received a body composition analysis. Over 40 flu vaccines were provided. Cancer prevention: over 1,000 individuals received cancer prevention education and assistance scheduling preventative care appointments. The main focus of this prevention education was on colorectal, skin, breast, bladder and lung cancer. Worksite wellness: hcc worked with franklin savings bank, stratton lumber, re-energy, franklin chrysler, the pierce house, transcanada and northern lights to provide health screenings, vaccinations, cpr/first aid training, and education reaching over 200 employees. Work healthy annual award program- to help assist employers with their worksite wellness initiatives in conjunction with providing technical assistance on creating and revising wellness, tobacco and substance abuse policies. The 2015 work healthy worksite wellness awards were given to franklin savings bank, community concepts, western maine community action, university of maine at farmington, healthquest and leap.

Two staff members certified as navigators to assist community members with health insurance marketplace enrollment provided education to over 300 individuals and enrollment assistance to approximately 165 individuals throughout franklin county. Additional education about the health insurance marketplace was provided through posts on healthbeat radio show, public service announcements, healthbeat tv show on local cable access, facebook and the fchn and hcc websites.

LET'S GO CHILD CARE SITES HAVE A REACH OF 123 CHILDREN, SCHOOLS HAVE A REACH OF 2,205 STUDENTS, OUT OF SCHOOL PROGRAMS HAVE A REACH OF 1,135 CHILDREN AND HEALTHCARE SITES HAVE A REACH OF 3,675 PATIENTS. IN THE SPRING OF 2015 NEARLY 3,000 IN MINI-GRANTS WAS DISTRIBUTED TO 17 LET'S GO SITES PRIMARILY FOR EQUIPMENT THAT PROMOTED PHYSICAL ACTIVITY AND MATERIALS THAT SUPPORTED HEALTHY MEALS, SNACKS OR CELEBRATIONS OR PROMOTED DRINKING WATER. IN THE FALL OF 2015 SEVEN CHILD CARE SITES, THREE HEALTHCARE SITES, THREE OUT OF SCHOOL SITES AND ONE SCHOOL RECEIVED RECOGNITION FROM LET'S GO FOR IMPLEMENTING ALL 5 EVIDENCE-BASED PRIORITY STRATEGIES. THE LET'S GO SCHOOL NUTRITION WORKGROUP MET REGULARLY TO ADDRESS HEALTHY SCHOOL MEALS AND INVOLVES SCHOOL NUTRITION DIRECTORS FROM ALL 5 SCHOOL DISTRICTS WITH A TOTAL REACH OF 4,822 STUDENTS. CAROL WHITE PEP GRANT: STAFF FROM HCC AND PARTICIPATING SCHOOLS HAVE IMPLEMENTED CURRICULUM CHANGES, STAFF TRAININGS, AND PHYSICAL FITNESS TESTING WITH STUDENTS THROUGHOUT THE THREE SCHOOL DISTRICTS THAT ARE PARTICIPATING IN THE GRANT. THERE HAS BEEN A POSITIVE AVERAGE INCREASE OF 13% FOR STUDENTS WHO ENGAGE IN 60 MINUTES OR MORE OF DAILY PHYSICAL ACTIVITY. A NEW SCHOOL WELLNESS TEAM HAS BEEN ENGAGED TO WORK ON IMPROVING SCHOOL WELLNESS POLICIES AS PER GRANT EXPECTATIONS. PHYSICAL EDUCATION EQUIPMENT (STRENGTH AND CARDIO, AND CANOES, KAYAKS AND PADDLEBOARDS) HAS BEEN PROVIDED FOR SCHOOLS AND COMMUNITY PARTNERS. HCC SPONSORED A SCHOOL FOOD SERVICE WORKSHOP DAY AT MT. BLUE RSD. THE WORKSHOP COVERED INTRODUCING NEW RECIPES AND COOKING TECHNIQUES FOR STAFF IN ADDITION TO HOW TO ADVERTISE TO YOUTH. AFTER SCHOOL AND SUMMER RECREATION PROGRAMS AS WELL AS ADVENTURE BASED COOPERATIVE LEARNING PROGRAMS TOOK PLACE THROUGHOUT THE SCHOOLS AND COMMUNITIES INVOLVED IN THIS GRANT. COMMUNITY EVENTS HAVE BEEN HELD TO INVOLVE FAMILIES IN PHYSICAL ACTIVITY AND NUTRITION EDUCATION OPPORTUNITIES SUCH AS FREE SKI NIGHTS, TASTE TESTING NIGHTS, SLEDDING DAYS, AND BOWLING NIGHTS.

HCC COMPLETED A RURAL ACTIVE LIVING ASSESSMENT (RALA) FOR THE TOWN OF KINGFIELD AND IS PLANNING ON GOING OVER THE REPORT WITH THE TOWN OFFICE TO ADDRESS TOWN NEEDS. (CONTINUED ON SCHEDULE O) (CONTINUED FROM 990, PART III, LINE 4B) HCC STAFF HAS PROVIDED EDUCATION AND FREE SCREENINGS TO OVER 600 INDIVIDUALS AT THE FOLLOWING EVENTS: -CHILDREN'S TASK FORCE -FARMINGTON HOME AND LEISURE SHOW -FARMINGTON HOMELESS SHELTER -FOUR FREE SKI NIGHTS THROUGH A MINI-GRANT FROM THE MAINE BUREAU OF HIGHWAY, HCC WAS ABLE TO REACH TEENAGE YOUTH THROUGHOUT FRANKLIN COUNTY TO DELIVER IMPORTANT DRIVING SAFETY MESSAGES AND OFFER SAFE DRIVER TEEN/PARENT DRIVING CONTRACTS. ONE TEXT OR CALL COULD WRECK IT ALL PROMOTION WAS PLACED IN HIGH SCHOOLS AND RAN AT THE NARROW GAUGE CINEMA BEFORE MOVIES. HCC RECEIVED A TOTAL OF 74 SIGNED SAFE DRIVER TEEN/PARENT DRIVING CONTRACTS AND POSITIVE FEEDBACK FROM OTHERS WHO TOOK A SAFE DRIVING CONTRACT TO REVIEW WITH THEIR FAMILY. DESPITE THE END OF THE GRANT, HCC IS STILL RECEIVING REQUESTS FROM THE COMMUNITY FOR SAFE DRIVING CONTRACTS. SUBSTANCE ABUSE PREVENTION HCC HAS BEEN DISSEMINATING INFORMATION ABOUT MARIJUANA, PRESCRIPTION DRUGS, TOBACCO, AND ALCOHOL THROUGH COMMUNITY PARTNERS AND IS PART OF THE STATE MARIJUANA WORKGROUP. THE GROUP CONTINUES TO WORK ON YOUTH MESSAGES IN ORDER TO DECREASE THE AMOUNT OF ADOLESCENT USE. THE FRANKTIPS PROGRAM HAS BEEN VERY SUCCESSFUL IN BUSTING A DRUG OPERATION. LOCAL LAW ENFORCEMENT CONTINUED THIS PROGRAM, BEYOND THE FUNDING PERIOD. 4 SUCCESSFUL RESPONSIBLE BEVERAGE SERVER TRAININGS WERE HELD - ONE OFF AND THREE ON PREMISE. ADS WERE SUBMITTED TO NARROW GAUGE CINEMA TO HELP EDUCATE THE COMMUNITY ABOUT ALCOHOLSCREENING.ORG, PRESCRIPTION DRUG ABUSE, AND MARIJUANA. EACH AD WAS PLAYED FOR AN ENTIRE MONTH. THE SUBSTANCE ABUSE PREVENTION COALITION, COMPRISED OF LOCAL LAW ENFORCEMENT, EDUCATORS, PARENTS, YOUTH AND COMMUNITY MEMBERS MET MONTHLY TO WORK ON VARIOUS YOUTH SUBSTANCE ABUSE INITIATIVES INCLUDING PARTY-BUSTER PATROLS AND PARENTS WHO HOST THE MOST CAMPAIGNS.

STAFF PROVIDED EDUCATION HEALTH SCREENINGS INCLUDING CHOLESTEROL, GLUCOSE AND BLOOD PRESSURE CHECKS, AND EDUCATED AND COUNSELED INDIVIDUALS ON A VARIETY OF WELLNESS TOPICS INCLUDING HEALTHY EATING, EXERCISE, DISEASE PREVENTION, CHRONIC DISEASE MANAGEMENT, LEAD PREVENTION, WELL-WATER SAFETY AND THE AFFORDABLE HEALTHCARE ACT. WELLNESS EVENTS OCCURRED THROUGHOUT THE COMMUNITY WITH AND WITHOUT THE MOBILE HEALTH (CONTINUED ON SCHEDULE O) (CONTINUED FROM 990, PART III, LINE 4C) UNIT REACHING COMMUNITY MEMBERS THROUGHOUT THE COUNTY. THE MOBILE HEALTH UNIT STAFFED WITH RNS, NURSE PRACTITIONER, HEALTH EDUCATORS, AND MEDICAL PROVIDERS TRAVELED TO THE TOWNS OF RANGELEY, EUSTIS, KINGFIELD, PHILLIPS, FARMINGTON, STRONG, LIVERMORE FALLS, AND WILTON PROVIDING EDUCATION, FREE MEDICAL SCREENINGS AND 1:1 AND SMALL GROUP INSTRUCTION. A TOTAL OF 122 INDIVIDUALS RECEIVED A BODY COMPOSITION ANALYSIS. OVER 40 FLU VACCINES WERE PROVIDED. CANCER PREVENTION: OVER 1,000 INDIVIDUALS RECEIVED CANCER PREVENTION EDUCATION AND ASSISTANCE SCHEDULING PREVENTATIVE CARE APPOINTMENTS. THE MAIN FOCUS OF THIS PREVENTION EDUCATION WAS ON COLORECTAL, SKIN, BREAST, BLADDER AND LUNG CANCER. WORKSITE WELLNESS: HCC WORKED WITH FRANKLIN SAVINGS BANK, STRATTON LUMBER, RE-ENERGY, FRANKLIN CHRYSLER, THE PIERCE HOUSE, TRANSCANADA AND NORTHERN LIGHTS TO PROVIDE HEALTH SCREENINGS, VACCINATIONS, CPR/FIRST AID TRAINING, AND EDUCATION REACHING OVER 200 EMPLOYEES. WORK HEALTHY ANNUAL AWARD PROGRAM- TO HELP ASSIST EMPLOYERS WITH THEIR WORKSITE WELLNESS INITIATIVES IN CONJUNCTION WITH PROVIDING TECHNICAL ASSISTANCE ON CREATING AND REVISING WELLNESS, TOBACCO AND SUBSTANCE ABUSE POLICIES.
