Mission

The mission of healthy community coalition (hcc) is to measurably improve the health and well-being of all people in franklin county and neighboring towns, using a coordinated approach of education, health promotion and outreach. Hcc offers health screenings, health education, and health promotion to support healthy lifestyles that prevent disease and improve quality of life for all community members.

The Organization reviewed the Form 990 with the officers of the board of directors prior to filing the tax return in a form of a presentation and a Q&A session to address any questions or concerns.

On an annual basis board members and officers are required to submit a conflict of interest statement which is then reviewed by the compliance committee. The compliance committee is responsible for regularly and consistently monitoring and enforcing compliance with the conflict of interest policy.

IRS990/Desc0Physical Activity/Nutrition/Tobacco (PANT):Over 300 cholesterol and blood pressure screenings were conducted as part of the ScoreHealth Screenings. The screenings included education on nutrition, physical activity, and tobacco prevention. Over 300 smoke-free housing informational packets were mailed to landlords and multiple property owners to educate them on smoke-free laws and creating healthy homes. A Rural Active Living Assessment was completed for the towns of Industry, Livermore, Livermore Falls, Rangeley, Eustis, and Wilton. Staff has had conversations with school nurses and superintendents in RSU #9, SAD #58, and SAD #73 about enhancing their substance abuse, tobacco and wellness policies. Let's Go! 1. Registered 20 child centric sites for Let's Go! including 8 child care sites, 4 schools, 2 out of school programs and 6 health care sites.2. Promoted Let's Go! to over 20 health care providers at Franklin Community Health Network Primary Care Service Team. 3. Promoted Let's Go! to nearly 40 child care providers at Western Maine Community Action annual meeting.4. Promoted Let's Go! To approximately 100 teachers at the Maine School Site Health Promotion Wellness conference.5. Assisted in the development of a day-long training for child care providers to promote healthy eating and active living.6. Registered sites were evaluated to assess their environment, actions and policies related to Let's Go! Strategies.Supplemental Nutrition Assistance Program Education:SNAP Nutrition Educators taught 199 classes with 1,813 participants at HeadStart programs, schools, senior housing, food pantries, WIC offices and a homeless shelter. Classes were focused on nutrition and physical activity to include the additional topics: MyPlate guidelines, label reading, meal planning, healthy snack preparation, cooking, grocery shopping on a budget and growing vegetables in a container garden.Community Transformation Grant Programming:During the last 12 months of this program more than 170 youth participated in low or no cost physical activity programming in Carrabassett Valley, Rangeley, Strong, Kingfield, Wilton, and Farmington. Afterschool programming continued to thrive as well as the expansion of existing programs. There were 4 community events, ranging from free ski days to a free movie day as a reward for continued program attendance that about 350 youth attended. An additional 8 individuals were trained to conduct special physical activity programming with youth utilizing established, evidenced based programming. The most rewarding of all is that two communities have planned to continue programming beyond the grant period.

IRS990/ProgSrvcAccomActy2Grp/Desc0Community Wellness and Prevention:A total of approximately 8,000 individuals benefitted from HCC outreach activities throughout the year with over 3,000 served through Mobile Health Unit Outreach. Staff provided education on a variety of wellness topics including healthy eating, exercise, disease prevention, chronic disease management, lead prevention, well-water safety and the Affordable Healthcare Act. Wellness events occurred throughout the community with and without the Mobile Health Unit reaching community members throughout the county. The Mobile Health Unit staffed with RNs, Nurse Practitioner, Health Educators, and Medical Providers traveled to the towns of Rangeley, Eustis, Kingfield, Phillips, Farmington, Strong, Livermore Falls, and Wilton providing education, free medical screenings and 1:1 and small group instruction. A total of 190 individuals received a body composition analysis. Over 200 flu vaccines were provided on the Mobile Health Unit and at other public educational events. Cancer Prevention:A total of 3,500 individuals received cancer prevention education. Over 300 reported they would make a positive lifestyle change to improve their health and over 250 individuals were assisted scheduling preventative care appointments. A total of 35 medical providers and staff were educated about current screening guidelines for colorectal cancer. A total of 24 Home Visits were conducted in the towns of Livermore Falls, New Sharon, Farmington, Farmington Falls, Phillips, and StrongWorksite Wellness:HCC worked with Leap Inc, Re-Energy, The Maine State Teachers' Association, Work First, Franklin Chrysler, The Pierce House, Franklin Savings Bank, and Franklin Community Health Network to provide worksite wellness initiatives such as health screenings and education reaching over 500 employees. Additionally, HCC conducted quarterly worksite wellness roundtable for employers to attend to share their successes and challenges. Eleven employers participated. Lead Outreach - To assist in educating community members and prevent lead poisoning among children, 350 lead packets and informational tip sheets were distributed to families with children under the age of 6 who are living, or spending more than 10 hours per week, in a pre-1978 home, or those who use lead through work or hobbies.

IRS990/ProgSrvcAccomActy3Grp/Desc0Community Building:HCC hosted bimonthly meetings of the Franklin Resource Collaborative, a group of over 50 community nonprofit organizations serving Greater Franklin Community that meet regularly to discuss and identify community needs and solutions.With funding from the Maine Health Access Foundation, HCC partnered with several other community organizations to conduct a community health needs assessment. Partnering organizations included food pantries, community service organizations and individuals in the community experiencing food insecurity. Results will be utilized for planning in the upcoming year to address the highest priority identified.HCC mobilized volunteers, Master Gardeners, local farmers, and foundation funding to provide education, instruction, and free supplies to community members for individual home container gardens with the goal of increasing access to fresh vegetables and teaching a lifelong skill. In addition, HCC donated approximately 2,500 pounds of produce from the Hope Harvest Garden to local food pantries.Substance Abuse Prevention:1. Six tobacco retailers were recruited to participate in the NO BUTS! Program. 2. A total of 37 were trained in the use of the Prescription Monitoring Program (PMP)3. One Fake ID Training was held at UMF for 12 Greater Franklin County Police Officers. 4. Strategies implemented to increase perception among youth that they would be caught if using alcohol included Project Sticker Shock, a Media Campaign at the local Narrow Gauge Movie Theater, a HealthBeat Radio Show focused on substance abuse, and Card ME Trainings.A total of 21 employees at local businesses received Responsible Beverage Server Trainings through 2 educational events. Over 700 parents and youth were educated on substance abuse prevention including the risks of underage drinking, monitoring your teen, and underage drinking laws.CCHC - HCC has conducted extensive outreach to begin to determine the greatest health issue in the community. Partnering with 5 community organizations, Western Maine Community Action, Western Maine Transportation, United Way, Seniors Plus, United Methodist Economic Ministry over 400 surveys have been conducted to solicit feedback from community members.
