PART X, LINE 2:

Asc 740 footnote - generally accepted accounting principles in the united states of america prescribe the threshold a tax position is required to meet before being recognized in the financial statements. The tax-exempt status of an entity is considered a tax position. A liability for uncertain tax positions ("utps") is recognized and recorded as a component of current income tax expense for differences between financial and income tax reporting positions which do not meet this threshold. Any interest and penalties related to utps are recorded as a component of income tax expense. The organization has not taken any tax positions, including any tax positions that would jeopardize its tax exempt status, which would have a material effect, individually or in the aggregate, on its consolidated financial statements, and thus has not recorded a liability at december 31, 2015 or 2014. Fsg files tax returns in federal, state, local, and foreign jurisdictions. Fsg, inc. Is no longer subject to examinations by tax authorities for years prior to 2011. Currently, there are no tax audits in process.

IN 2015, FSG ASSISTED THE STATEN ISLAND FOUNDATION IN DEVELOPING INITIATIVES TO ADDRESS SUBSTANCE ABUSE, WORKED FOR BLUE CROSS BLUE SHIELD OF MASSACHUSETTS FOUNDATION TO DEVELOP A COLLECTIVE IMPACT EFFORT ON THE INTERSECTION OF HOUSING AND HEALTH, AND PARTNERED WITH COMMUNITY HEALTH CENTER OF RICHMOND TO HELP BUILD THE STATEN ISLAND PERINATAL NETWORK A COALITION FOCUSED ON IMPROVING MATERNAL AND INFANT HEALTH ON STATEN ISLAND, USING THE COLLECTIVE IMPACT MODEL AND FUNDED BY A FEDERAL HEALTHY START GRANT. FSG ALSO WORKED HELPED A NUMBER OF HEALTH FOUNDATIONS IMPROVE THEIR APPROACH TO LEARNING AND EVALUATION. IN 2015, FSG PARTNERED WITH THE KAISER PERMANENTE TO PROVIDE SUPPORT AND TECHNICAL ASSISTANCE IN THE DEVELOPMENT OF A STRATEGIC LEARNING AND EVALUATION SYSTEM, CONTINUED OUR WORKED WITH THE KRESGE FOUNDATION TO DEVELOP A STRATEGIC LEARNING AND EVALUATION SYSTEM FOR SIX PROGRAM AREAS INCLUDING HEALTH, AND WORKED WITH THE FORD FOUNDATION TO DEVELOP EVALUATION FRAMEWORKS WITHIN THEIR GENDER, SEXUALITY AND REPRODUCTIVE RIGHTS PORTFOLIO. FSG ALSO WORKED WITH PRIVATE CORPORATIONS WHO ARE ADVANCING COMMUNITY HEALTH IN THE US.
