“The compensation committee is responsible for overseeing the evaluation process for the chief executive officer. The following steps are utilized to carry out this review: a) the compensation committee reviews performance measures and targets on or before the start of the first meeting of the fiscal year. “Clinic services - a) health center services: the diagnostic health center division of jawonio, inc. Provides services to individuals with (1) developmental and/or physical disabilities,(2) behavioral health issues and (3) complex multi-system medical conditions. The medical and behavioral health center affords professional, highly individualized, coordinated care, by licensed physicians in the areas of primary care, podiatry, psychiatry, dental and gynecology. Therapeutic services that are available through the health center include psychology, social work, physical therapy, occupational therapy and speech, language and swallowing therapy. Jawonio also offers specialized services in the area of audiology (including a hearing aid dispensary) as well as dental services. All physicians and clinicians are licensed in the state of new york and have experience in providing care to individuals with disabilities and special needs. Many of our services are available for individuals from infancy through adulthood. Jawonio prides itself in being recognized as an exceptional provider of highly specialized, coordinated health services. In 2012, the director of the health center, carin shapiro, md, mph was recognized by the rock and county association for learning disabilities and received their humanitarian of the year award for her commitment to individuals with learning and other developmental disabilities and her philosophy of patient care. The jawonio health center organizes monthly educational breakfasts to community nurses and caregivers on topics relative to the care of individuals with developmental and physical disabilities and/or serious mental illness; these are attended by approximately 25 community nurses per session. The jawonio health center treated approximately 2500 individuals in 2012. Patient satisfaction is extremely high, at 100% satisfaction rate for all disciplines in 2012. B) behavioral health services: the pros (personalized recovery oriented services) program is designed to encourage participants to move forward with their recovery in a manner that will allow more independent functioning through the use of community based services. Members "graduate" and move on to the use of other, less intense, community resources, including our article 28 clinic. The goal of pros is to integrate treatment, support, and rehabilitation in a manner that facilitates an individual's recovery. Goals for individuals are to improve functioning, reduce inpatient utilization, reduce emergency services, increase employment, attain higher levels of education, and secure preferred housing. Individuals participate in a diversity of group/class activities designed to assist in managing their mental illness and restoring skills, coupled with a strong emphasis on relapse prevention and clinical treatment to stabilize, ameliorate and control symptoms. In 2012, there were 220 consumers in the program with 52% of them employed and attending part-time. The program has the largest employment component in pros within the state, and collaborates with jawonio's community employment services, to help people with seeking and maintaining jobs. The adult home case management program is an ancillary service to our recovery oriented programs serving 120 people and helping to coordinate services and supports for the residents with their recovery and life skill goals. This is accomplished by our supportive care manager working in tandem with a peer specialist who engages them with an increase of activities (15-20 per week) and community outings. We are able to help people move out of the adult home into living safely into the community. In 2012, 96% of the consumers were satisfied with their services. Care management/health home model: 97 rockland county case management slots were transferred to jawonio in 2012. The program grew with 80 of the 97 slots filled by the end of 2012. “Jawonio, Inc. adopted the provisions of ASC 740, "Income Taxes", on January 1, 2009. Under ASC 740, an organization must recognize the tax benefit associated with tax positions taken for tax return purposes when it is more likely than not that the position will not be sustained upon examination by a taxing authority. The implementation of ASC 740 had no impact on the organization's combining financial statements. The organization does not believe it has taken any material uncertain tax positions and, accordingly, it has not recorded any liability for unrecognized tax benefits. The organization has filed for and received income tax exemptions in the jurisdictions where it is required to do so. Additionally, The organization has filed Internal Revenue Service Form 990 information returns, as required, and all other applicable returns in jurisdictions where so required. For the year ended December 31, 2013, there were no interest or penalties recorded or included in the combining statement of activities. The organization is subject to routine audits by taxing authorities. As of December 31, 2013, the organization was not subject to any examination by a taxing authority. JAWONIO'S COMMUNITY LIVING SERVICES PROVIDES SERVICES IN DIVERSE SETTING THAT ARE DESIGNED TO MEET THE NEEDS OF THE RESIDENTS. INTERMEDIATE CARE FACILITIES (3) - THESE HOMES PROVIDE COMPREHENSIVE MEDICAL & HABILITATIVE SUPPORTS IN A TWENTY-FOUR HOUR SUPERVISED SETTING FOR 36 INDIVIDUALS. THE PROGRAM IS DESIGNED TO ASSIST PERSONS WITH SEVERE INTELLECTUAL DISABILITIES AND/OR SEVERE MEDICAL FRAILTIES TO BECOME AS INDEPENDENT AS POSSIBLE. COMPREHENSIVE ON-SITE MEDICAL SUPERVISION IS PROVIDED. INDIVIDUALIZED RESIDENTIAL ALTERNATIVES (10) - JAWONIO PROVIDE HOMES IN THE COMMUNITY SPECIFICALLY DESIGNED AND FURNISHED TO MEET THE NEEDS OF THE DEVELOPMENTALLY DISABLED ADULT. THESE PROGRAMS DIFFER FROM THE ICF RESIDENCES IN THAT THE INDIVIDUALS REQUIRE LESS MEDICAL OVERSIGHT AND CARE, AND FOR THE MOST PART ARE ABLE TO GO INTO THE COMMUNITY FOR DAY AND RECREATION PROGRAMS. TWENTY-FOUR HOUR SUPERVISION AND ASSISTANCE IS PROVIDED. THE CONDOMINIUM PROGRAM (6) - SERVICES DEVELOPED SPECIFIC TO INDIVIDUALS' NEEDS ARE PROVIDED IN ONE AND TWO BEDROOM CONDOMINIUMS IN THE COMMUNITY. LOW COST HOUSING IS PROVIDED THROUGH THE OFFICE OF HOUSING AND URBAN DEVELOPMENT FOR SIX CONDOMINIUMS. THE PAUL J. TENDLER INDEPENDENT LIVING CENTER (17) - THIS GARDEN APARTMENT COMPLEX PROVIDES SERVICE AND LOW COST HOUSING TO 17 PEOPLE. ALL BUT ONE APARTMENT ARE SINGLE OCCUPANCY, PROVIDING DIGNITY AND PRIVACY TO EACH INDIVIDUAL. IN 2012, INDIVIDUALS AND THEIR FAMILIES AND ADVOCATES REPORTED A 91% SATISFACTION RATE FOR ALL RESIDENTIAL SERVICES, NOTING THE ESPECIALLY HIGH LEVEL OF COMMUNICATION BETWEEN THE PROGRAMS AND THE CONSUMERS, FAMILIES AND ADVOCATES; THE OUTSTANDING CARE PROVIDED BY OUR DIRECT SUPPORT PROFESSIONALS AND THE HIGH QUALITY OF LIFE PROVIDED ON A DAY TO DAY BASIS NOT ONLY IN MEETING THE INDIVIDUALS' UNIQUE CARE NEEDS BUT ALSO THE CHOICES OF COMMUNITY AND RECREATIONAL OPPORTUNITIES MADE AVAILABLE TO THEM BASED ON EACH PERSON'S INDIVIDUAL PREFERENCES AND INTERESTS. MEDICAID SERVICE COORDINATION SERVICES ARE PROVIDED BY SERVICE COORDINATORS WHO OVERSEE ALL SERVICES PROVIDED FOR CHILDREN AND ADULTS WHO HAVE A DEVELOPMENTAL DISABILITY; HAVE OR ARE ELIGIBLE FOR MEDICAID; RESIDE EITHER INDEPENDENTLY OR WITH THEIR FAMILY; OR IN A FAMILY CARE OR GROUP HOME.
