Form 990, Part VI, Line 1A: Explanation of Delegated Broad Authority to Committee

The organization has a committee that assumes responsibility for oversight of the audited financial statements and selection of an independent accountant.

Form 990, Part VI, Line 2: Description of Business or Family Relationship of Officers, Directors, Et

Dr. Andersen, and Dr. Bailey are contracted with the LVCHO to provide physician services to inmates when such services are requested by the Bureau of Prisons, and provide regular quality/utilization review of all medical services provided by contractors of the LVCHO. Dr. Schlosser is contracted to provide quality/utilization review. During fiscal 3/31/2020,compensation to Dr. Bailey, Dr. Schlosser, and Dr. Andersen fell below the reportable Schedule L threshold, and did not compromise voting independence for these particular services.LVCHO had a service contract with Lompoc Valley Medical Center (LVMC) for the provision of healthcare services, which are now provided by a Primary Care Clinic owned by LVMC. LVCHO Board members Stephen Popkin and Ray Down serve LVMC in the capacity of CEO and Board member, respectively. Since LVMC owns the Primary Care Clinic, this material business transaction is reportable on the schedule L. The transaction amount listed represents the amount that LVCHO paid to the Primary Care Clinic in the aggregate for all contract medical services received during the year.This reportable business transaction impairs the voting independence of Mr. Popkin and Mr. Down as Board Members.Lompoc Valley Medical Center (LVMC) - is a tax exempt governmental entity. LVCHO and LVMC are reported as related organizations, as they have common agents.

LVCHO does not categorize any expense as "Fundraising" as it does not typically conduct significant traditional fundraising in the form of special events, etc. Grants that LVCHO receives are considered to be Program in nature, and are used for program activities which generate Program expenses.

Form 990, Part V lines 7A&7B

The organization was the recipient of contributions from donors. However, no contribution was made partly for goods and also for services. Donors made cash donations without the receipt of any goods or services, and the donor was properly notified of the deductible value of the donation they made. Thus questions 7a&b were left blank, since no goods and services in combination were received from donors.

The LVCHO managed a capital campaign to renovate the track and field at Huyck Stadium in Lompoc. Once complete, the school district-owned stadium will be open for community use. Pledges receivable for this purpose are recorded on this line.

IRS990/ProgSrvcAccomActy2Grp/Desc0Community Health Improvement Program efforts including: Building off the previous five years' projects, the organization received a grant to implement a program for caregivers in the Lompoc Valley with the aim at strengthening the safety net for the aging population. Building off the third year of the project, the organization also received a fourth year of funding via a grant to promote the use of Electronic Benefits Transfer at local Farmer's Markers to increase access to healthy foods to low-income community members. This also included support to open a new Farmer's Market in the Lompoc Valley and an additional contract with the Santa Barbara County Public Health Deparment to fund this work. The LVCHO managed a capital campaign to renovate the track and field at Huyck Stadium in Lompoc. Once complete, the school district-owned stadium will be open for community use. The LVCHO took over responsibility as the fiscal agent for the annual Senior Health Expo due to the closure of Sansum Clinic's Lompoc offices. The LVCHO also took on the role in partnership with the Santa Barbara County Trails Council to bring the Healthy People Healthy Trails program to Lompoc including guided hikes and prescriptions for physical activity. The LVCHO signed a contract with Santa Barbara County to implement a 2020 Census Complete Count effort to ensure Lompoc residents were counted. The LVCHO compelted a three-year strategic plan with some suport through Foundation funding. The LVCHo also recieved a COVID-19 response grant to help identify isloated seniors and connect them to needed resources during the pandemic. The LVCHO and its community coalitions continue to implement a strategic plan to further embed HEAL into the local culture. Provided funding for low income and uninsured individuals to receive needed prescription medications; presented healthy living workshops to multiple community organizations upon request; staffed Healthy Lompoc Task Force to assure broad community participation in community health decision-making; recommended to Lompoc City Council that the city become a HEAL (Healthy Eating and Active Living) community; accepted responsibility for developing HEAL implementation plan; Developed grade appropriate curricullum for K - 12 regarding bike safety; curricullum accepted by LUSD and incorporated into the classrooms at all LUSD K-8 schools; enhanced enforcement of traffic laws around school zones at beginning and end of school day; Conducted a community needs assessments around caregiving and senior issues; coordinated collaborative partners to implement a collaborative program to strength the safety net for seniors and caregivers; Supported the opening of a Family Caregiver Support Network; Managed campaign to raise awareness and self-idenfitication of caregivers; Facillitated the installation of creative crosswalks to promote art, economy and health; Facillitated the Healthy Lompoc Coalition and supported member agencies with thier health efforts; developed a HEAL outreach team to educate the community about the benefits of HEAL; Established a HEAL Advisory Cabinet to further imbed HEAL into the culture of leadership in Lompoc; purchased and installed community bicycle repair stations; Helped secure grant funds for refrigerated and filtered water bottle refill and water fountain stations; promoted the use of electronic benefits transfer at local Farmers Markets; Facillitated discussion about opening a third Farmers Market for the Lompoc community; Helped to open a new Farmer's Market in the Lompoc Valley; Increased food literacy and healthy food access through the new Farmer's Market in the Lompoc Valley; Contracted with the CA Office of Traffic Safety to promote pedestrian and bicycle safety; Planned and implemented Lompoc's first and second annual Open Streets event to promote active transporation; Raised funds for local disaster relief; Iniatiated and completed fundraising for a capital campaign and public awareness campai
