During fiscal 3/31/2018,compensation to Dr. Bailey, Dr. Schlosser, and Dr. Andersen fell below the reportable Schedule L threshold, and did not compromise voting independence for these particular services.LVCHO had a service contract with Lompoc Valley Medical Center (LVMC) for the provision of healthcare services, which are now provided by a Primary Care Clinic owned by LVMC. LVCHO Board members James Raggio and Ray Down serve LVMC in the capacity of CEO and Board member, respectively. Since LVMC owns the Primary Care Clinic, this material business transaction is reportable on the schedule L. The transaction amount listed represents the amount that LVCHO paid to the Primary Care Clinic in the aggregate for all contract medical services received during the year.This reportable business transaction impairs the voting independence of Mr. Raggio and Mr. Down as Board Members.Lompoc Valley Medical Center (LVMC) - is a tax exempt governmental entity. LVCHO and LVMC do not meet the requirements to be reported as related organizations, as they do not share a majority of the same board members, nor does one exercise control over the other.”

| IRS990/ProgSrvcAccomActy2Grp/Desc | 0 | Community Health Improvement Program efforts including: Building off the previous three years' projects, the organization received a grant to implement a program for caregivers in the Lompoc Valley with the aim at strengthening the safety net for the aging population. Building off the first year of the project, the organization also recieved a second year of funding via a grant to promote the use of Electornic Benefits Transfer at local Farmer's Markets to increase access to healthy foods to low-income community members. The LVCHO managed a community campaign to raise funds and gather donations and contributions to benefit the victims of the fire and debris flow in southern Santa Barbara County in December of 2017 and January of 2018. The LVCHO also inititated a capital campaign to rennovate the track and field at Huyck Stadium in Lompoc. Once complete, the school district-owned stadium will be open for community use. The LVCHO and its community coalition continue to implement a strategic plan to further embed HEAL into the local culture. Provided funding for low income and uninsured individuals to receive needed prescription medications, coordinated a summer food program providing free lunch to over 300 children each day; presented healthy living workshops to multiple community organizations upon request; provided a weekly education article on healthy lifestyles in the local newspaper; reviewed all elements of the Lompoc General Plan to assure they reflect current knowledge about health and made recommendations for change when current policy does not further community health; staffed Healthy Lompoc Task Force to assure broad community participation in community health decision-making; recommended to Lompoc City Council that the city become a HEAL (Healthy Eating and Active Living) community; accepted responsibility for developing HEAL implementation plan; Implemented a Safe Routes to School Program in partnership with the Lompoc School District, Lompoc Police and Lompoc Engineering Department; held a city wide "walk and roll"day to promote walking and biking to school; held a bike to work day to encourage everyone to bike to work; Developed grade appropriate curricullum for K - 12 regarding bike safety; curricullum accepted by LUSD and incorporated into the classrooms at all LUSD K-8 schools; enhanced enforcement of traffic laws around school zones at beginning and end of school day; provided over 500 bike helmets to students who successfully completed a bike safety education program; held a city wide bike rodeo to further educate children and parents about bike safety; Conducted a community neededs assessments around caregiving and senior issues; coordinated collaborative partners to implement a collaborative program to strength the safety net for seniors; Supported the opening of a Caregiver Support Network; Managed campaign to raise awareness and self-idenfitication of caregivers; Facillitated the installation of creative crosswalks to promote art, economy and health; Facillitated the Healthy Lompoc Coalition and supported member agencies with thier health efforts; developed a HEAL outreach team to educate the community about the benefits of HEAL; Established a HEAL Advisory Cabinet to further embed HEAL into the culture of leadership in Lompoc; purchased and installed community bicycle repair station; promoted the use of electronic benefits transfer at local Farmers Markets; Facillitated discussion about opening a third Farmers Market for the Lompoc community; Contracted with the CA Office of Traffic Safety to promote pedestrian and bicycle safety; Planned and implemented Lompoc's first Open Streets event to promote active transporation; Raised funds for local disaster relief; Iniatiated a capital campaign and public awareness campaign around then rennovation of local track and field for athelete, student and community use. |
