Activity 2

Outpatient/ambulatory medical care - outpatient medical care includes the provision of professional diagnostic and therapeutic services rendered by a physician or other health care professionals certify to prescribe antiretroviral (arv) therapy. Aids arms provides hiv medical care at two medical clinics. The peabody health center (phc) was opened in 2001 with fewer 200 patients and is located in south dallas. The trinity health and wellness center (thwc) located in oak cliff was opened in 2011. In 2012, both clinics serviced 1,492 patients with 6,081 medical visits.

Activity 3

Comprehensive case management - the case management program provides a range of client-centered services that include the provision of advice and assistance in obtaining medical, social, community, legal, financial, and other needed services. Case management activities includes screening for eligibility, continued assessment for client needs, development and implementation of care plans based on clients' identified needs, referrals to needed services, and crisis interventions as needed. The program focus remains proactive to avoid crisis and to improve health outcomes and life stability. In 2012, aids arms served 2,289 hiv+ individuals, nearly 10,100 impacted family members with 18,208 case management units. A case management unit is defined as a 15 minute encounter with a client.
