Mission
The CareSource Foundation is organized and operated exclusively to further the charitable purposes of its supported organization, CareSource, by improving overall community health and addressing health-related issues in the counties in which CareSource operates. The CareSource Foundation improves the lives of the underserved by providing grants to non-profit organizations that can significantly impact individuals and families through a broad array of health and human services and programs. This charitable assistance targets the neediest populations in Ohio, and provides innovative solutions to critical health care needs and creates meaningful collaboration. The CareSource Foundation's mission is to advance the health and well-being of underserved people in the communities CareSource serves by providing innovative solutions and funding to address health care needs.

| IRS990/Description | 0 | The CareSource Foundation provides opportunities to advance innovative solutions and address healthcare needs of the underserved through grants, outreach, medical expertise, strategic community partnerships and volunteerism. The Foundation funds responsive grants for targeted areas of need, which are given to individual non-profit organizations or a collaboration of non-profits through a single year grant. Funding is in the following priority areas: (1) Childrens health (Ohio & Michigan), including critical health trends, children in poverty, and access to care, (2) crisis support for women and children (Ohio), including domestic violence, child abuse, and homelessness, and (3) special needs populations (Ohio), including the elderly, and those with disabilities. The CareSource Foundation also funds signature grants with numerous regional, state and/or national partners all focused on creating sustainable, high-impact change in one of two key areas: childhood obesity and programs to support basic health needs for the uninsured. The CareSource Foundation funded the following signature grants in 2011. (1) A $200,000 grant was awarded to Enroll America, a newly formed non-partisan, 501(c)(3) organization dedicated to securing health coverage enrollment for tens of millions of uninsured people. The organization was launched by a collaboration of key, diverse national groups representing stakeholders in America's health care system. Groups participating in the new collaboration range from companies and associations representing the hospital, insurance and pharmaceutical industries to organizations representing physicians, community health centers, free clinics, health care consumers and people with major illnesses and disabilities. Funding for this signature grant will occur over a two-year period beginning in 2011 and ending in 2012. (2) A $250,000 signature grant was awarded to Five Rivers Metroparks RiverScape River Run Fund c/o The Dayton Foundation. The RiverScape River Run will create additional sites for outdoor, recreational activities including kayaking, hiking, biking and cross-training. Most activities will be available at low or no cost which will provide access to both traditional and non-traditional physical fitness and wellness opportunities for all sectors of the community. (3) The CareSource Foundation also awarded a $150,000 signature grant to support the Montgomery County Ohio College Promise program, whose mission is to provide life-changing, educational opportunities for Montgomery County students. College Promise's goal is to provide children in poverty with the best opportunity for both high school and college graduation while also having an impact on their quality of life and their community. The grant will enable 10 College Promise Scholars to attend college at little or no cost to them or their families. The program provides each College Promise student with a supportive mentor to improve or uphold school attendance, grades and behavior. Students who fulfill their pledge will earn a full scholarship opportunity to attend Sinclair Community College or Wright State University. Funding for this signature grant will occur over a two-year period beginning in 2011 and ending in 2012. Funding for a signature grant of $65,000 awarded in 2010 to Think Tank is now in year two. Think Tank created a series of poverty simulations, a powerful, experience-based opportunity for community leaders, legislators and influencers from all sectors across Ohio to better understand the plight of the underserved, and influence policy that will ultimately benefit that population. Funding in the amount of $32,500 occurred in 2011 for the second year of the grant contract. Montgomery County Care, a signature grant awarded in 2010, is also in year two of the grant program.
