The CareSource Foundation is organized and operated exclusively to further the charitable purposes of its supported organization, CareSource, by improving overall community health and addressing health-related issues in the counties in which CareSource operates. The CareSource Foundation improves the lives of the underserved by providing grants to non-profit organizations that can significantly impact individuals and families through a broad array of health and human services and programs. This charitable assistance targets the neediest populations in Ohio, and provides innovative solutions to critical health care needs and creates meaningful collaboration. The CareSource Foundation's mission is to advance the health and well-being of underserved people in the communities CareSource serves by providing innovative solutions and funding to address health care needs.

“CareSource is the member of The CareSource Foundation. CareSource is recognized as exempt from federal income taxation under Internal Revenue Code ("IRC") Section 501(c)(3).”

“Annually, each director, principal officer, and member of a committee with board-delegated powers ("interested person") shall confirm that they have received a copy of the CareSource Conflicts of Interest policy and have read, understood, and agree to comply with the policy. Interested Persons have an obligation to immediately report any Conflicts of Interest (including any relationships, positions, or circumstances that could contribute to a Conflict of Interest). All relevant information reported through the Conflict of Interest Policy will be sent to the Chairman of the Board for review. If the Interested Person with the Conflict of Interest is the Chairman of the Board, then the required disclosure must be provided to the Chief Legal Officer of CareSource. If it is not entirely clear whether a Conflict of Interest exists, then the person with the potential conflict must disclose the circumstances to CareSource's Chief Legal Officer. The Chief Legal Officer will consult with the Corporation's Corporate Compliance Officer or the Chairman of the Board to determine whether there exists a Conflict of Interest that is subject to this policy. Before Board action or other action by the organization on a Transaction that involves a Conflict of Interest, an Interested Person who knows he or she has a Conflict of Interest must have disclosed to the Board all facts material to the Conflict of Interest. The Chairman of the Board may postpone Board or other corporate action on a Transaction until the Interested Person provides written information relating to the Conflict of Interest. An Interested Person who knows he or she has a Conflict of Interest must not participate in the Board's discussion of the Transaction except to disclose material facts and respond to questions. The Interested Person must not attempt to influence the Board's action on the Transaction, either at or outside the meeting. Prior to voting, the Board must be given an opportunity to discuss the Transaction without the person who has the Conflict of Interest being present. A Transaction involving a Conflict of Interest may be approved by the Board if the material facts as to the Transaction and the Conflict of Interest are fully disclosed or known to the Board and the Board in good faith determines after reasonable investigation that (a) the Board is aware of all material facts concerning the Transaction and the Interested person's interest in the Transaction; (b) the organization is entering into the Transaction for its own benefit; (c) the Transaction is fair and reasonable as to the organization; and (d) the organization could not have obtained a more advantageous arrangement with reasonable effort under the circumstances. The Person with the Conflict of Interest must not vote on the Transaction and must not be present in the room when the vote is taken.”

Over the last decade, the Organization has seen great challenges in the areas of well-being and health care. What is most exciting, however, is the impact The CareSource Foundation has had as the result of partnerships with some of the most impressive nonprofits in the country. The CareSource Foundation signature grants are focused on innovative, high-impact approaches to solving critical social issues. The CareSource Foundation awarded a total of more than $1.3M in signature grants to the following organizations in 2022: 1) Nurse Family Partnership National Office - $529,034 (Granted over Three Years) (Denver, CO) Cultural Consciousness Pathway: Develop, pilot, implement and evaluate a cultural consciousness pathway for nurses in our national network of local partners. 2) Ronald McDonald House Charities of Dayton - $500,000 (Granted over Three Years) (Dayton, OH) RMHC Keeping More Families Close Capital Campaign: Constructing a new 50,000 sq. ft. building consisting of two finished floors and a shelled third floor which will accommodate future growth. 3) Phoebe Foundation Inc. - $256,226 (Granted over Three Years) (Albany, GA) Nurse Family Partnership Expansion: Establish a Nurse Family Partnership Affiliate in Dougherty County in partnership with the Phoebe-Putney Health System. 4) Indiana University - $250,000 (Granted over Three Years) (Bloomington, IN) Healthy Beginnings at Home Replication: 24 months of rental assistance, housing navigation, and housing case management to pregnant women in Marion County, Indiana. 5) OneFifteen Recovery - $220,000 (Granted over Two Years) (Dayton, OH) Tech-Enabled Care for Rural Ohio: Build and establish a Rural TeleHub Program that will connect rural patients to SUD clinicians virtually and deliver virtual addiction medicinal care. 6) Overdose Lifeline, Inc. - $178,425 (Granted over Three Years) (Indianapolis, IN) Overdose Emergency Response Program for Indiana Schools and School Nurses: Provides partners to schools for naloxone and naloxboxes, training staff, and youth opioid/substance prevention education. 7) Girl Scouts of Western Ohio - $150,000 (Blue Ash, OH) Empower Her: The Campaign to Transform our Camps and our Girls: Modernizing and diversifying the capital and outdoor curriculum for girls attending any of the four local camps. 8) Ohio State University Foundation - $125,000 (Granted over Two Years) (Dayton, OH) Moms2B and Dads2B: Weekly, neighborhood-based group program helps to connect women to medical care, and to learn skills that improve pregnancy and birth outcomes. 9) Ball State University Foundation - $100,000 (Granted over Two years) (Muncie, IN) Healthy Lives for Indiana Youth: A peer-led health promotion program to reduce obesity and promote overall health among adolesc”
