Mission

The CareSource Foundation is organized and operated exclusively to further the charitable purposes of its supported organization, CareSource, by improving overall community health and addressing health-related issues in the counties in which CareSource operates. The CareSource Foundation improves the lives of the underserved by providing grants to non-profit organizations that can significantly impact individuals and families through a broad array of health and human services and programs. This charitable assistance targets the neediest populations in Ohio, and provides innovative solutions to critical health care needs and creates meaningful collaboration. The CareSource Foundation's mission is to advance the health and well-being of underserved people in the communities CareSource serves by providing innovative solutions and funding to address health care needs.

CareSource is the sole member of The CareSource Foundation. CareSource is recognized as exempt from federal income taxation under Section 501(c)(3).

CareSource, the sole member of The CareSource Foundation, has the authority to appoint The CareSource Foundation's board.

Form 990, Part VI, Section B, Question 11B

The Form 990 was provided to the organization's Finance committee for their review of the return prior to the time of filing Form 990. The Form 990 was made available to each voting member of the governing body of the organization for their review prior to the time of filing Form 990. The Form 990 was reviewed by the CEO, CFO, VP Treasury, Director Treasury, an outside CPA firm, outside legal counsel, and by general counsel prior to filing Form 990 with the IRS.

Form 990, Part VI, Section B, Question 12C

Annually, each trustee, principal officer and member of a committee with Board-delegated powers ("interested person") shall sign a statement which affirms that such person a) has received a copy of the conflicts of interest policy, b) has read and understands the policy, c) has agreed to comply with the policy, and d) understands that the organization is a charitable organization and that in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes. In connection with any actual or possible conflicts of interest, an interested person must disclose the existence of his or her financial or conflict of interest and must be given the opportunity to disclose all material facts to the directors and members of committees with Board delegated powers considering the proposed transaction or arrangement. After disclosure of the financial interest and all material facts, and after any discussion with the interested person, he/she shall leave the Board or committee meeting while the determination of a conflict of interest is discussed and voted upon. The remaining Board or committee members shall decide if a conflict of interest exists. An interested person may make a presentation at the Board or committee meeting, but after such presentation, he/she shall leave the meeting during the discussion of, and the vote on, the transaction or arrangement that results in the conflict of interest. The conflicted trustee has no vote on the matter involved with the conflict of interest. At the beginning of each Board Meeting and Committee Meeting, the Board Members and Committee Members are asked to disclose if they have a Conflict of Interest on any item contained in the agenda. In addition, the Board Members and Committee Members are also asked to inform the board if a Conflict of Interest arises during the course of the Board or Committee Meeting. When such a conflict is disclosed, then the procedure outlined above is implemented. In addition, a Conflict of Interest Schedule is kept with respect to each declared Conflict of Interest during the course of a Board or Committee meeting. On an annual basis, the Nominating and Governance Committee reviews each Conflict of Interest raised by a Board or Committee Member. Also, the Nominating and Governance Committee conducts a bi-annual evaluation of the Board Members' performance during the course of the Board Meetings and Committee Meetings. The review includes but is not limited to the Board Member's engagement, participation, contributions to board business, attendance, and any absence from a meeting due to a conflict of interest. The Conflicts of Interest declared by the Board Members are reviewed on an annual basis by the Nominating and Governance Committee, and memorialized in meeting minutes, to make sure all policies on Conflict of Interest are adhered to by the Boards, the Committees and the Individual Board or Committee Member.

IRS990/Desc0The CareSource Foundation furthers the charitable purposes of CareSource by providing advancing innovative solutions and addressing health care needs of the underserved through grants, outreach, medical expertise, strategic community partnerships, and volunteerism. The CareSource Foundation funds responsive grants to nonprofit organizations that impact children, adults, and families in areas that mirror the social determinants of health. Specifically, the CareSource Foundation targets the following: Economic stability; safe and healthy homes and neighborhoods; job skills and employment; access to healthcare, including physical, mental and behavioral health; early childhood education and child development; healthy behaviors and lifestyle concerns; public policy surrounding critical need health issues; and strategic public/private partnerships designed to increase healthy communities. In 2018, The CareSource Foundation provided grants to over 130 Ohio nonprofit organizations totaling $951,500. Since 2006, the CareSource Foundation has awarded over 1,353 grants with total charitable assistance exceeding $17.3 million to strengthen the network of health and human service nonprofit organizations throughout Ohio. The CareSource Foundation provides funding to nonprofits who are working to eliminate poverty, providing much-needed services to low-and moderate-income families, building healthier communities, and developing innovative approaches to address critical issues of children, adults and families. The CareSource Foundation is a leader in cultivating partnerships that enhance and improve the health care and life services for consumers. All of these grants have been selected through a rigorous review process that evaluates innovation, impact and sustainability. Over the last decade, the Organization has seen great challenges in the areas of well-being and health care. What is most exciting, however, is the impact The CareSource Foundation has had as the result of partnerships with some of the most impressive nonprofits in the country. The CareSource Foundation signature grants are focused on innovative, high-impact approaches to solving critical social issues. The CareSource Foundation awarded a total of $250,000 in signature grants to the following organizations in 2018: (1) Ohio Community Development Corporation Association (Ohio CDCA). A $100,000 grant was awarded in addition to $200,000 previously awarded in 2017. The grant was used to further develop the Empowering Communities' Request for Proposal Grant process, which awards nonprofits for innovative approaches to community development; (2) Kenmore Neighborhood Alliance was awarded $150,000 to be funded over two years. In the Kenmore community of Akron, single mother-headed households comprise 11% of all households. Single mother-headed households also account for 47% of all families with children living in Kenmore, compared to 27% in Ohio and 25% in the U.S. Single mother-led households in Kenmore earn a median income of $17,917, 17% less than those living elsewhere in Ohio and 29% less than those throughout the U.S. The grant will support the Jobs Collaborative project aimed at assisting the 47% low-income, single-mom population in south Akron. The grant will also support improved job skills, certifications and increase their employability while addressing the need for economic inspiration for the broader business district. The CareSource Foundation also funded second half grants in 2018 for the following grants awarded in a prior year: (1) $125,000 to The Columbus Foundation and CelebrateOne to continue expansion of the CareHome Model through Healthy Beginnings at Home, a housing stabilization program providing wraparound support for low-income pregnant women.
