The Svila Collaborative d.b.a. Centro Svila is a treatment program devoted to the recovery and healing of individuals, families, and communities suffering from emotional and psychological distress. Our vision is a healthy, engaged and equitable community. Our mission is providing access to quality healthcare and eliminating health disparities through prevention, education, and collaboration. Our comprehensive model of care works to decrease health disparities in Bernalillo County by providing: outpatient mental health services, systems navigation, no-cost health insurance enrollment assistance for Medicaid and the New Mexico Health Insurance Exchange, school-based counseling, therapeutic gardening, food security support, drug and alcohol counseling and recovery support, and supervision, training and research opportunities for students and health professionals. “OTHER PROGRAM SERVICES 4: Healthcare Professional Pipeline: The Svila Collaborative is actively working to decrease the mental healthcare provider shortage in New Mexico by providing community-based training and internship opportunities for students working towards degrees in Social Work, Counseling, Psychology and Psychiatry. During FY 2014, The Svila Collaborative: Provided free clinical supervision to 2 masters degree student interns, 1 doctoral student conducting practicum placement, 1 psychiatry resident and 1 undergraduate UNM Service Corps volunteer. All of the supervision was for bilingual students that want to go into health professions. The Svila Collaborative is a loan repayment site for clinicians that want to work in federally designated health professional shortage areas (HPSA). The South Valley is a health professional shortage area. In addition to free clinical supervision, the Svila Collaborative provided funding for clinicians to take licensure examination prep courses and for the cost of their licensing examinations. This support helped two of our Spanish-speaking clinicians to obtain independent licensure. All of the members of our health insurance outreach and enrollment team were also given training to become Community Health Workers. During the past fiscal year, The Svila Collaborative was able to enroll one of our clinicians in the National Health Service Corps which payed off $50,000 of the clinicians student loans. Clinicians from Centro Svila provided over 50 hours of school and community information and outreach and school lectures to students interested in or studying mental health related fields; Participated in 5 professional association presentations; and participated in several collaborates and coalitions aimed at decreasing health disparities in Bernalillo County. During this fiscal year, The Svila Collaborative applied for and received funding for he Via a la Salud/Pipeline to Health program which is aimed at increasing the number of culturally and linguistically appropriate mental health professionals in New Mexico. Funding for this program was used to support our Healthcare Professional Pipeline efforts.Food Security:During FY 2014, The Svila Collaborative extended the scope of our therapeutic garden project by increasing the land used and the number of clients served while partnering with other groups in the South Valley to expand our Food Security program; The Therapeutic Garden project was expanded by contracting with a local bilingual farmer to maximize the use of the garden and to educate patients on how to grow healthy and nutritious foods. This year over 20 bags of groceries, and household supplies were distributed weekly to patients with food insecurity. During this fiscal year The Svila Collaborative applied for and received funding to design and build a Community Table which consists of an outdoor table and shade structure where group activities and food distribution will take place upon completion.Research and Advocacy:The Svila Collaborative participates in research examining the social determinants of health, access to behavioral health services, and clinical outcomes. During FY 2014, The Svila Collaborative: Participated in the Building Movement Research Project that examined collaborations of Non-Profit Organizations in New Mexico; Participated in a Hispanics in Philanthropy training on documenting the Social Determinants of Health, participated in focus groups to understand food security, published articles on health disparities in peer reviewed journals and participated in a county-wide task force to examine behavioral healthcare in Bernalillo County. The Executive Director and several employees testified in front of the County Commission about the need for expanded funding for behavioral health services and existing gaps in the States behavioral health safety net.”

| IRS990/ProgSrvcAccomActy2Grp/Desc | 0 | Outreach and Enrollment: During FY 2014, The Svila Collaborative worked to expand health insurance coverage in New Mexico by training employees to become enrollers for Presumptive Eligibility Medicaid On Site Application Assistance (PEMOSAA) and the New Mexico Health Insurance Exchange. Centro Svila hired a team of 8 bilingual outreach and enrollment specialist who scheduled over 1200 appointments and successfully enrolled 534 people in Medicaid and 360 in private health insurance. The enrollment team also participated in outreach efforts to inform the public about health insurance options and eligibility. The Outreach and Enrollment team performed over 930 hours on outreach and education efforts. The enrollment team attended over 190 events within the Bernalillo area, focusing our outreach and education efforts to target the South Valley communities. We encountered over 15,000 individuals by distributing marketing materials within the community and school events, local businesses and community centers. Our bicultural/bilingual team was able to reach out to Latino/Hispanic and Spanish speaking New Mexicans who are the largest uninsured population in NM. |

During FY 2014, The Svila Collaborative participated in the Pathways to a Healthy Bernalillo County program providing services to some of the most economically, psychologically and physically vulnerable residents of Bernalillo County. In addition, The Svila Collaborative coordinated student volunteers and interns from UNM, NMHU, and NMSU to provide case management services for our patients. The Svila Collaborative expanded our Community Health Worker (CHW)/Promotora efforts to integrate research, education and services to reduce behavioral health disparities with the Hispanic and Latino populations. CHW training was integrated into our heath insurance outreach and enrollment efforts to facilitate newly insured in finding a healthcare home and other social supports. The Svila Collaborative also received funding for the Community Partnership for Health Equity program that focuses on upstream healthcare prevention, community engagement, and integration of healthcare and social services. This program allowed The Svila Collaborative to serve as the fiscal agent and liaison for the Girasol and ACCESS programs. The Girasol program provides one on one educational sessions for Spanish-speaking patients diagnosed with diabetes and pre-diabetes to help patients better understand and manage their blood sugar through diet, medication management and exercise. The Girasol program works in collaboration with primary care physicians and mental health providers at Centro Svila to provide better support for patients managing chronic disease and their families. The ACCESS program supports after school activities for Spanish-speaking elementary, middle school and high school students in the South Valley. The program combines community engagement, direct services and health promotion through CHWs. The ACCESS program formed its own soccer league and provided free after school education, athletic, cultural, art and science opportunities for South Valley children and their families. |

| IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt | 0 | OTHER PROGRAM SERVICES 4: Healthcare Professional Pipeline: The Svila Collaborative is actively working to decrease the mental healthcare provider shortage in New Mexico by providing community-based training and internship opportunities for students working towards degrees in Social Work, Counseling, Psychology and Psychiatry. During FY 2014, The Svila Collaborative: Provided free clinical supervision to 2 masters degree student interns, 1 doctoral student conducting practicum placement, 1 psychiatry resident and 1 undergraduate UNM Service Corps volunteer. All of the supervision was for bilingual students that want to go into health professions. The Svila Collaborative is a loan repayment site for clinicians that want to work in federally designated health professional shortage areas (HPSA). The South Valley is a health professional shortage area. In addition to free clinical supervision, the Svila Collaborative provided funding for clinicians to take licensure examination prep courses and for the cost of their licensing examinations. This support helped two of our Spanish-speaking clinicians to obtain independent licensure. All of the members of our health insurance outreach and enrollment team were also given training to become Community Health Workers. During the past fiscal year, The Svila Collaborative was able to enroll one of our clinicians in the National Health Service Corps which payed off $50,000 of the clinicians student loans. Clinicians from Centro Svila provided over 50 hours of school and community information and outreach and school lectures to students interested in or studying mental health related fields; Participated in 5 professional association presentations; and participated in several collaborates and coalitions aimed at decreasing health disparities in Bernalillo County. During this fiscal year, The Svila Collaborative applied for and received funding for he Via a la Salud/Pipeline to Health program which is aimed at increasing the number of culturally and linguistically appropriate mental health professionals in New Mexico. Funding for this program was used to support our Healthcare Professional Pipeline efforts.Food Security:During FY 2014, The Svila Collaborative extended the scope of our therapeutic garden project by increasing the land used and the number of clients served while partnering with other groups in the South Valley to expand our Food Security program; The Therapeutic Garden project was expanded by contracting with a local bilingual farmer to maximize the use of the garden and to educate patients on how to grow healthy and nutritious foods. This year over 20 bags of groceries, and household supplies were distributed weekly to patients with food insecurity. During this fiscal year The Svila Collaborative applied for and received funding to design and build a Community Table which consists of an outdoor table and shade structure where group activities and food distribution will take place upon completion.Research and Advocacy:The Svila Collaborative participates in research examining the social determinants of health, access to behavioral health services, and clinical outcomes. During FY 2014, The Svila Collaborative: Participated in the Building Movement Research Project that examined collaborations of Non-Profit Organizations in New Mexico; Participated in a Hispanics in Philanthropy training on documenting the Social Determinants of Health, participated in focus groups to understand food security, published articles on health disparities in peer reviewed journals and participated in a county-wide task force to examine behavioral healthcare in Bernalillo County. The Executive Director and several employees testified in front of the County Commission about the need for expanded funding for behavioral health services and existing gaps in the States behavioral health safety net. |
