Board governance policies on a conflict of interest include board members, family members, and entities in which the board member or their family member has a material financial interest or control. The policies include requirements for disclosing any potential conflict, prohibits participation in any board actions related to a potential conflict, and prohibits accepting gifts which might influence their actions. Board members are to provide written disclosure to the chair and vice chair of the board of any employment, activity, investment, or other interest which might conflict with the interests of lifetrack. Board members with a potential conflict shall not participate in board discussion or action related to the potential conflict. Board members are asked at the beginning of the board meeting to declare conflicts with agenda items (which would be recorded in the meeting minutes), and they also complete a questionnaire annually to identify potential conflicts.

Financial Statement Notes

PART V, LINE 4:

Endowment gifts to lifetrack are permanently invested and overseen by our board of directors. Interest income from endowment is available to support the work of the organization, and distributed with respect to the donors' intentions. The lifetrack endowments are restricted by donors in three categories - deaf and hard of hearing services, families together program and general use to benefit children, adults and families served by lifetrack resources.

PART X, LINE 2:

Lifetrack resources, inc. Is organized as a minnesota nonprofit corporation and has been recognized by the irs as exempt from federal income taxes under section 501(a) of the internal revenue code (irc) as an organization described in section 501(c)(3), qualifies for the charitable contribution deduction under section 170(b)(1)(a)(viii), and has been determined not to be a private foundation under section 509(a)(2). The organization is annually required to file a return of organization exempt from income tax (form 990) with the irs. In addition, the organization is subject to income tax on net income that is derived from business activities that are unrelated to its exempt purpose. The organization has determined it is not subject to unrelated business income tax and has not filed an exempt organization business income tax return (form 990-t) with the irs. The organization believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The organization would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

THE STATE ANALYZED PROGRAM DATA TO IDENTIFY IF INDIVIDUALS WERE WORKING THREE YEARS AFTER RECEIVING LIFETRACK'S EMPLOYMENT SERVICES. THE RESULTS SHOW THAT 72.73% OF THE CLIENTS ARE WORKING 30 HOURS OR MORE EACH WEEK, WELL-SURPASSING THE GOAL OF 60%.

WE ADDRESS THE WELL-BEING OF DISADVANTAGED CHILDREN AND THEIR FAMILIES, AND SUPPORT INDIVIDUALS IN THEIR EFFORTS OF SELF-SUFFICIENCY MANY OF WHOM ARE FACING OBSTACLES OF PHYSICAL OR MENTAL DISABILITIES, CRIMINAL JUSTICE SYSTEM INVOLVEMENT AND/OR POVERTY.LIFETRACK'S FAMILIES TOGETHER PROGRAM, WHICH SERVED 342 CLIENTS IN 2019, IS DESIGNED TO SUPPORT THE FAMILIES IN ATTAINING HEALTH AND STABILITY THROUGH OUR TWO-SIDED APPROACH: OUR HOME VISITING PROGRAM AND THERAPEUTIC PRESCHOOL. THE HOME VISITING PROGRAM IS AN AFFILIATE OF THE NATIONALLY EVIDENCE-BASED MODEL, PARENTS AS TEACHERS (PAT), WHICH PROVIDES PARENTS WITH THE EDUCATION AND SKILLS NEEDED TO CREATE SAFE AND NURTURING HOME ENVIRONMENTS THAT SUPPORT HEALTHY DEVELOPMENT AND PROVIDES ACCESS TO RESOURCES FOR PARENTS TO ADDRESS THEIR PHYSICAL AND MENTAL HEALTH.THE THERAPEUTIC PRESCHOOL IS NAEYC (NATIONAL ASSOCIATION FOR THE EDUCATION OF YOUNG CHILDREN) ACCREDITED AND FOUR-STAR PARENT AWARE RATED. IT PROVIDES CHILDREN WITH THE INTERVENTION NEEDED TO OVERCOME THE EFFECTS OF ONGOING TRAUMA SO THAT THEY CAN SUCCEED IN KINDERGARTEN AND BEYOND. IN THE PRESCHOOL, EACH CHILD'S TEAM CONSISTS OF TEACHERS, A SPEECH AND LANGUAGE, OCCUPATIONAL, AND/OR MENTAL HEALTH THERAPIST, AND A HOME VISITOR/PARENT EDUCATOR. THE TEAM, WORKING WITH THE CHILD'S PARENT OR CAREGIVER, DEVELOPS AN INDIVIDUALIZED PLAN FOR ADDRESSING COGNITIVE, DEVELOPMENTAL, OR SOCIAL-EMOTIONAL DELAYS AT TIME OF ENROLLMENT, AND HOLDS BI-WEEKLY MEETINGS TO DISCUSS THE CHILD'S PROGRESS.
