The completed 990 is reviewed and approved by the Board Audit & Finance Committee and then sent to the full Board of Directors for their review and approval prior to submission to the IRS.

Form 990, Part VI, Section B, line 12C

Board members and officers shall disclose any potential conflict, and shall not participate in the discussion of board or corporate action related to such dealing and shall not be counted for purposes of determining a quorum when such action is taken. Violations of the conflict of interest policy shall be reported to the Board Chair, or if they concern actions of the Board Chair, to the Vice Chair. The officer receiving the report shall bring this information to the attention of the full board in whatever manner is most appropriate. Board members and officers will certify in writing annually their compliance with the conflict of interest policy.

Form 990, Part VI, Section B, line 15A

Compensation for the Chief Executive Officer shall be based on qualifications and performance, consistent with market comparisons, of the CEO position relative to functionally comparable leadership positions in other organizations in the Twin Cities metropolitan area. Benefits for the CEO, in addition to base salary, shall be substantially similar to those provided to other regular staff of the Corporation. Any incentive pay plan shall be documented and approved by the Board in advance of the evaluation period. Initial and subsequent decisions on CEO compensation shall be documented, including terms of compensation arrangements, authority for the compensation decision, approval date, names of board members on the committee which recommended the decision, data used in arriving at the decision, including review of actual CEO compensation records for the preceding year, and disclosures of conflict of interest, if any. Compensation for Vice President positions is determined by the CEO using market evaluation data, provided by the HR department, on an annual basis. The market evaluation is completed using salary surveys that represent the state of Minnesota, the Twin Cities metro area, nonprofit data, FTE matches, and a national survey.

Financial Statement Notes

Part V, Line 4:

Endowment gifts to Lifetrack Resources are permanently invested and overseen by our Board of Directors. Interest income from endowments is available to support the work of the Organization, and is distributed with respect to the donors' intentions. The Lifetrack Resources Endowment presently has three components: 1. General Endowment - to benefit children, adults and families served by Lifetrack Resources. Designated gift funds on behalf of general endowment include the John and Sheila Mohr Fund and the Jerome G. Byrnes Fund. 2. Early Childhood Endowment - to benefit at-risk children and their families. One designated gift fund is the Thelma Brynolfson Fund for therapeutic preschool activities. 3. Child and Adult Therapies Endowment - to benefit individuals with disabilities. One donor designated fund is the William H. Davis/Hearing Society of Minnesota for Services to Deaf and Hard of Hearing Children and Adults.

Part X, Line 2:

Lifetrack Resources, Inc. is organized as a Minnesota nonprofit corporation and has been recognized by the Internal Revenue Service (IRS) as exempt from federal income taxes under Section 501(a) of the Internal Revenue Code as an organization described in Section 501(c)(3), qualifies for the charitable contribution deduction under Section 170(b)(1)(A)(viii), and has been determined not to be a private foundation under Section 509(a)(2). The Organization is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the Organization is subject to income tax on net income that is derived from business activities that are unrelated to its exempt purpose. The Organization has determined it is not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990-T) with the IRS. The Organization believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Organization would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

IRS990/Desc0Employment and Economic Opportunity Achievements:Lifetrack provides employment services to groups traditionally experiencing the greatest disparities in income, employment, and advancement including at-risk youth (age 18-21), individuals with disabilities, welfare-to-work participants, immigrants and refugees. Our values reflect the core philosophy that everyone can work with the right supports. * In 2015, Lifetrack served 1,760 individuals seeking employment throughout the Employment & Economic Opportunity focus area. * 709 clients were placed in jobs and 72% of clients retained their job for more than 12 months* 1,246 clients completed financial training or coaching * 484 clients completed a training program resulting in an industry recognized credential* 77% of clients who attended onsite employer recruitment events were pursued for hire. These events are an important tool for overcoming hiring bias. They have proven to be an effective tool for working with local employers to hire quality candidates, particularly women or members of minority groups. * Our immigrant and refugee program received continued funding from the State of Minnesota to provide employment services to refugees and immigrants, in partnership with Vietnamese Social Services in Saint Paul, Welcome Center in Austin, and Intercultural Mutual Assistance Association in Rochester. Lifetrack staff members speak 19 different languages.* Our Partnering for Jobs collaborative with Human Services Inc. and the state Vocational Rehabilitation Services is an evidence-based practices-supported employment program offered to improve the system coordination between mental health professionals and vocational professionals to more effectively help people with serious mental illness (SMI) achieve employment goals. It is based on the premise that work is part of recovery.In 2015, Lifetrack received two awards for outstanding employment outcomes: *2015 Recognition Award "Adult Service Provider of the Year" for our Career Pathways work in Construction and Commercial Truck Driving,presented by the Minneapolis Workforce Council; and *2015 Recognition Award "Pioneer Agency Serving Individuals with Mental Health Barriers" presented by the State of Minnesota Vocational Rehabilitation Services

IRS990/ProgSrvcAccomActy2Grp/Desc02015 Child & Healthy Family Achievements:Within Child & Family Healthy Development, our services fall into two categories: Early Childhood & Family Services and Child & Adult Therapeutic Services.Early Childhood & Family Services: Lifetrack is committed to helping every child we serve improve in one or more key developmental indicators: social, cognitive, motor, speech/language and mental health and to seeing that their parents and caregivers increase their knowledge of resources to meet their child's basic, health and developmental needs. *Lifetrack secured its first contract with DHS to provide the "Babies Can't Wait" webinar.*For the first time, Lifetrack took the lead as host agency for the 2015 Work Life & Flexibility Expo. It was very successful with high satisfaction ratings.*MCTHV partnering with MDH to complete a statewide training sector scan of training needs. Goal is to create plan for providing a comprehensive training system to ensure best-practices are used with all home visiting programs across the state.*Completed the statewide needs assessment for families with children who are deaf or hard of hearing. *Minnesota Hands and Voices hosted another successful family picnic with a Super Heroes theme-404 attendees with 100% satisfaction (1 of 12 events this quarter)*All therapy charter school contracts were renewed in August and restructured as auto-renewing each academic year. A comprehensive sell sheet was created and sent to local charter schools.*Families Together has been awarded the Women's United grant to support the start of a System's Navigator at Lifetrack. This two year grant will help us expand our whole family approach to include a deeper health component to our work.*NAEYC accreditation was renewed for Families Together preschool.*Families Together preschool was able to hire two Reading Corps members, enabling all four classes to have additional early literacy support as well as general support for children's ongoing needs.*Therapy Social Skills groups successfully completed 3 group sessions in 2015 for youth 9-12yrs and 5-8 yrs through funding from MOFAS and Medica. The group leaders worked with the curriculum's author to adapt and pilot the program for 5-8 year old children.
