Major Activities

Activity 2
Governance - In 2011, R4D launched the third phase of its flagship Governance program, the Transparency and Accountability Program. The primary component of TAP3 in 2011 was the selection and awarding of fifteen sub-grants to civil society organizations in sub-Saharan Africa to design and undertake projects that will improve spending and service delivery in the health and education sectors. Five of the sub-grantees were selected in early 2011 to complete 2.5-year projects in the effectiveness of pharmaceutical supplies (Uganda), reproductive health and family planning supplies (Uganda), nine-year basic education (Rwanda), textbook procurement (Ghana), and resources for teaching and learning (Burkina Faso). In May 2011, R4D designed and led a technical support and peer learning workshop for these five organizations to provide training on expenditure and service delivery monitoring, support in designing research objectives and questions, peer learning opportunities, and advocacy training. Since the May 2011 workshop, the five sub-grantees have completed the first phase of their research, identified useful findings for improving service delivery and spending in their countries, and begun advocacy - all with the support of R4D. The additional ten sub-grantees were selected in November 2011 to design and initiate citizen report cards with the financial and technical support of the TAP3 program. These organizations are based in Burkina Faso, Ghana, Kenya, Rwanda, Senegal, Tanzania, and Uganda. In addition to the financial support, technical support, and training of civil society organizations, R4D undertook several activities to develop resources that can provide wider support to organizations interested in undertaking social accountability across the world. These resources include a strengthened website and library of resources on public expenditure tracking, service delivery surveys, absenteeism studies, and citizen report cards and a series of videos highlighting successful evidence-based advocacy efforts of civil society organizations that R4D has worked with. The videos are expected to be completed in mid-2012.
Activity 3
Market Dynamics - Developed Market Shaping Strategy for the Global Fund's $7 billion health products portfolio: The Global Fund's Market Dynamics Committee (MDC) commissioned R4D to develop concrete strategies which optimized value for money' across its $7B health products portfolio. Based on R4D's analysis, Global Fund launched an innovative Market-Shaping Strategy. The initial application was to HIV/AIDs drugs, and will save an initial $520 million - this can fund 3.5 million new patients on ARVs. The strategy also identified strategies to prevent 90,000 children from facing treatment disruption.Enabled implementation of the Global Fund Market Shaping Strategy: Following development of the Market Shaping Strategy (see above), the Global Fund Secretariat commissioned R4D to develop an implementation strategy to enable its voluntary pooled procurement mechanism (VPP) to strategically manage demand. Much of the impact of the Global Fund's market shaping strategy relies on the effective functioning of the VPP, which requires strategic management of demand. The team developed a model of industry best practices for strategic demand, along with the policy and operational steps for the Secretariat to successfully optimize VPP functioning.Ensured sustainable supply of drug for neglected disease: The Market Dynamics Practice was charged by the Gates Foundation with ensuring sustainable supply of drugs for treating neglected tropical disease schistosomiasis. R4D worked with partners across the market place - including major donors, procurement agents, and pharmaceutical manufacturers - to map supply constraints and recommend strategies to break bottlenecks. Identified strategies to save over $600M in the bed net market for malaria prevention: Over 560 million long-lasting insecticide-treated bed nets (LLINs) are required through 2015 alone to achieve and maintain universal coverage for malaria prevention in Africa, at a total global cost of $2.4 billion. With Gates Foundation support, R4D developed concrete strategies to save the global community more than $600 million over 5 years, which can finance $150M bed nets and cover 300 million more individuals. Strategies include financial incentives to drive uptake of the most cost-effective LLINs while ensuring development and use of innovative new technologies. The work was the first-of-its kind, and included consultations with over 140 actors across the private and public sector. The findings of this project will be released in 2012 and R4D will work to ensure the strategy is implemented by working with major financiers and normative agencies (i.e. WHO, USG).

IRS990/Description0Health - Joint Learning Network for Universal Health Coverage: The Joint Learning Network for Universal Health Coverage (JLN) aims to accelerate the progress and improve the success of low and middle-income countries in implementing financing reforms designed to improve health outcomes and financial protection for the poor and move toward universal health coverage. Founded in 2010 as a network of policymakers in Asian and African countries that are pursuing national health insurance reforms, since its inception, the JLN has grown to include ten member countries: Ghana, India, Indonesia, Kenya, Malaysia, Mali, Nigeria, the Philippines, Thailand, and Vietnam. In 2011, R4D played a pivotal role in the development of a broad coalition of JLN collaborating partners and pioneered a new and innovative model for supporting south-south learning among countries. Other major accomplishments in 2011 included launching and coordinating the work of the four technical tracks and the Joint Learning Fund to flexibly fund small, demand-driven cross-country learning activities; developing and publishing 19 online, modular, comparative case studies and 15 promising practice case studies; managing the country engagement process, including network membership; organizing multilateral joint learning workshops for JLN member countries and resource countries; and launching a JLN web portal and pilot member platform to facilitate information sharing and exchange among countries. R4D receives funding from the Rockefeller Foundation and the Bill and Melinda Gates Foundation to support the JLN. The Center for Health Market Innovations (CHMI) promotes policies and practices that improve health care delivered by the private sector for the poor in developing countries. The CHMI website (www.healthmarketinnovations.org) is the most comprehensive global repository of innovative health programs that harness private providers to serve the poor, currently reporting information about over 1,000 programs operating in 104 countries. CHMI works in close partnership with CHMI "hub" organizations operating in Africa, Asia, and Latin America to identify, document, and promote the diffusion of promising models. In 2011, R4D made significant progress in expanding the reach and activities of CHMI, including continuing to source innovative models for its growing database of programs, expanding the collection of reported results from featured programs, completing a series of analytic products, and beginning to lay the groundwork for future activities that identify successful programs and accelerate their scale up and replication. R4D receives funding from the Bill and Melinda Gates Foundation and the Rockefeller Foundation to support CHMI. Center for Global Health R&D Policy (the Center): aims to expand and improve the information used by governments, philanthropists and private investors in making decisions on new ways to drive global health research and development (R&D). Founded in 2009, the Center serves as a policy translator and catalyzes critical discussions to progress R&D in order to better diagnose, prevent and treat neglected diseases. From topic selection, to assessment research, through release of findings, the Center identifies the policymakers and stakeholders relevant to each topic area and seeks to engage them at all stages. In 2011, the Center's work on Prizes for Global Health Technologies evaluated whether prizes could spur medical innovation for neglected diseases, finding that prizes are a mechanism which hold promise and a prize contest to drive the development or certain products, such as diagnostics, is worth piloting. Other topics included evaluating tax credit proposal to lower the costs of global health R&D for US companies, and an investigation of how open source models can move the global health community toward a knowledge commons for neglected disease R&D.
