Mission

A.d.d.'s purpose is to provide the optimum level of quality rehabilitation services to all individuals livining within our residential program. We provide a healthy, safe and homelike environment, in which individuals may achieve the greatest level of independence possible. A.d.d. Strives to surpass all requirements of omh, opwdd, federal, state and local authorities. Guided by core values of respect, choice trust, empathy and dignity, we believe that every individual has value and potential to learn and achieve goals. We do everything to inspire self-esteem, and encourage individuals to help each other and work together whenever possible. We aim to achieve fulfillment for every individual and employee by providing innovative and progressive programming and training. We are consistent in providing competent and compassionate staff who are role models for the people they serve.

Aid to the developmentally disabled, inc. (add) is a multi-service organization providing residential programs, supportive apartments, clinical support and case management services, for the developmentally disabled, mentally ill and the dually disgnosed adults on long island's east end. Add's purpose is to provide the optimum level of quality rehabilitative services to all individuals living within our residential program. We provide a healthy, safe and homelike environment, in which individuals may achieve the greatest level of independence possible. Add strives to surpass all requirements of omh, opwdd, federal, state and local authorities. Guided by core values and respect, choice, trust, empathy and dignity, we believe that every individual has value and the potential to learn and achieve goals. Add is dedicated to creating an environment where individuals are given the opportunity to be productive members of their community.

The form 990 is presented to the finance/audit committee of the board and to the executive director and controller before it is filed with the irs. It is circulated to the board prior to filing. Once accepted at this meeting it is presented to the whole board by the finance/audit committee chairman. Each board member receives a copy. This is performed before the form 990 is filed with the irs.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is required to be reviewed by the board of directors and employees annually. The conflict of interest statement is then signed by the board, executive director, and staff. Each board member must complete a conflict of interest questionaire to disclose any conflict of interest or potential conflict of interest. If the board of directors determines that an actual or possible conflict of interest was not disclosed, a corrective action is taken. Any conflicted person is required not to be present during the decision making process.

The personnel committee along with the board of directors regularly reviews all compensation matters. The board of directors reviews comparable salary data of similar sized agencies from 990s before final approval of salary changes for the executive director. The process was last conducted in 2015.

THERE ARE 82 CLIENTS SERVED. 49 CLIENTS ARE IN COMMUNITY RESIDENCES, 12 ARE IN APARTMENT TREATMENT (SUPPORTIVE) AND 21 ARE IN SUPPORTED APARTMENT. THE COMMUNITY RESIDENCES REQUIRE 24 HOURS A DAY STAFFING. SUPPORT, TRAINING AND ASSISTANCE IN ACTIVITIES OF DAILY LIVING ARE PROVIDED BY TRAINED STAFF. IN THE APARTMENT TREATMENT PROGRAM INDIVIDUALS ARE INDEPENDENT IN MANY AREAS OF DAILY LIVING BUT MAY CONTINUE TO REQUIRE MINIMAL SUPPORT AND SUPERVISION. THE SUPPORTED HOUSING SITES ARE NOT LICENSED WITH OMH. THERE ARE 3 INDIVIDUALS THAT LIVE TOGETHER AND ARE SELF-SUFFICIENT FOR THE MOST PART BUT MAY RECEIVE GUIDANCE AND ASSISTANCE TO ACCESS NECESSARY SERVICES.
