Management reviews all questions and prepares the documentation for the form 990. The 990 is then submitted electronically to the board of directors for comment. Should no comments be forthcoming, the 990 will be signed and submitted

The conflict of interest policy is part of the personnel policy manual, which is issued annually to all employees and to new employees upon hire during orientation. Employees are required to read and sign off that they have received and read the personnel policy manual. Program directors and above are sent a conflict of interest questionnaire annually and they are required to complete the form and submit it to the hr manager.

In 2017 the human resources manager purchased the guidestar nonprofit compensation report to obtain analysis of non-profit compensation data based entirely on irs data to validate company compensation practices.

Form 990, Part VI, Section C, Line 19

Governing documents are made available upon request, the conflict of interest policy is part of the personnel policy manual, which is issued annually to all employees and to new employees upon hire and during orientation and is made available upon request. Annually, following the board of director's approval the audited financial statements are submitted to funding sources and the bank and are made available upon request. Form 990 is also available through guidestar.org.

THE OVERALL FY2017 SUCCESS RATE FOR THE ADULT WORK RELEASE PROGRAMS WAS 78%.

MST VIEWS AN INDIVIDUAL'S BEHAVIOR AS BEING INFLUENCED BY THE SYSTEMS AROUND THEM, FAMILY, PEERS, SCHOOL, AND NEIGHBORHOOD. MST INTERVENTIONS ADDRESS THE MANY FACTORS THAT ARE KNOWN TO CONTRIBUTE TO DELINQUENCY WHERE THE YOUTH LIVES, WORKS AND PLAYS. CSI HAS BEEN DELIVERING MST SERVICES FOR SEVENTEEN YEARS. WE CURRENTLY DELIVER SERVICES IN GA, MD, NJ, PA AND RI. THE 2017 DISCHARGE DATA SHOWS: YOUTH LIVING AT HOME-86.0%, STILL IN SCHOOL-83.0%; YOUTH WITH NO NEW ARREST-86.1%.

FUNDED BY THE CONNECTICUT JUDICIAL DEPARTMENT COURT SUPPORT SERVICES DIVISION (CSSD) AND THE SANTA BARBARA SHERIFF'S DEPARTMENT. THESE PROGRAMS ACCEPT COURT-REFERRED CLIENTS AS PART OF A PRETRIAL DIVERSION PROGRAM. THEY ALSO PROVIDE SERVICES TO PROBATION AND PAROLE PRE-VIOLATORS. THE PROGRAMS ARE OPEN FOR EXTENDED DAY, EVENING AND WEEKEND HOURS. THE AIC AND DRC STAFF MEMBERS ADMINISTER ASSESSMENTS, USE TARGETED INTERVENTION STRATEGIES, AND ASSIST CLIENTS IN GAINING FULL-TIME EMPLOYMENT AMONG MEETING OTHER INDIVIDUALIZED NEEDS. CSI HAS BEEN PROVIDING AIC SERVICES FOR OVER 20 YEARS AND IS THE LARGEST AIC PROVIDER IN CONNECTICUT. DURING FY2017 THE AIC'S AND DRC'S HAD A CUMULATIVE AVERAGE UTILIZATION OF 2772 AND ACHIEVED AN 87% SUCCESSFUL COMPLETION RATE.
