Our Care Team provides ongoing needs assessments to inform coordination of care such as referrals and wellness management, dental care, medical care, social services, and resources for immediate and long-term care.Prevention and HIV Testing are provided using the most recent testing technology through the CDC and we conduct testing at local universities, healthcare facilities, and community organizations as well as community events. In addition, we provide LGBTQ+ education for all ages within the communities we serve. |
