The form 990 is reviewed by the cfo, executive director and the audit committee prior to filing.

Form 990, Part VI, Section B, Line 12C

Each officer and director annually completes a conflict of interest/related party disclosure form identifying any relationships, positions or circumstances in which he or she believes could contribute to a conflict of interest situation. Any changes in status are communicated immediately to all responsible persons. All board members are instructed to notify either the ceo or the cfo if any conflicts of interest exist. Annually, the audit committee shall review all transactions that represent potential or actual conflicts of interest, shall determine whether such business dealings are fair and of equal value to other such services and shall report such to the board. A trustee who is involved in a potential conflict of interest is not allowed to vote on any matters related with the conflict of interest.

Form 990, Part VI, Section B, Line 15A

The full board of directors or a committee authorized by the board approves any salary adjustment for either the chief executive officer or the chief financial officer on a annual basis. The chief executive officer recommends the compensation for the cfo. The review and approval occurs at the time of initial hiring, when the employment term is renewed or extended, and when compensation is modified. The board ensures the compensation, including benefits, is just and reasonable. This review utilizes salary surveys or surveys of similar organizations, using phone contacts or review of form 990 information through guidestar.org. Approval of any adjustments in compensation is documented in the minutes of board of directors meetings. Wage survey data used are attached to the minutes. Approval of compensation changes occurs before the change in compensation.

In addition to any standing or special committees that may be appointed, the chairperson shall appoint an executive committee. The executive committee shall be composed of not less than five (5) or more than seven (7) members of the board of directors of the corporation. The officers of the corporation shall be members of the executive committee. The executive committee, with the approval of the board, will set duties, responsibilities, level of authority and reporting structure for the chief executive officer. The executive committee will have the following responsibilities: .provide oversight and direction for each of the standing and ad-hoc committees. .annually review the appropriateness of the corporation's mission statement. .annually review the effectiveness and appropriateness of the agency by-laws and articles of incorporation. .establish the annual board calendar. .annually evaluate the effectiveness of the functioning of the board of directors and its committees. .provide orientation for new directors.

PART V, LINE 4:

In 1999, fvo solutions (fvos) received a $10,000 gift restricted in perpetuity for the expansion of the organization's programs. Fvos has been investing the money in cd's and has not used any of the earned interest throughout the years. Cumulative unspent earnings earned interest on the contribution as of 6/30/2015 totaled $4,212.

PART X, LINE 2:

Generally accepted accounting principles provide accounting and disclosure guidance about positions taken by an organization in its tax returns that might be uncertain. Management has considered its tax positions and believes all of the positions taken by fvos are more likely than not to be sustained upon examination. The returns are subject to examination by federal and state taxing authorities generally three and four years, respectively, after they are filed.
