Form 990, Part III, line 3

In April 2016, Charlotte Maxwell Clinic closed our San Francisco location due to financial considerations. Increasingly high rents have forced a number of non-profits to relocate or close their doors. CMC now maintains only one location in Oakland. One staff member was laid off and others reduced their hours. Approximately 70% of our SF-based clients as well as over 50% of our volunteer practitioners were unable to make the transition to Oakland. As a result, we served fewer clients in the following months of 2016. CMC continues to conduct outreach to bring in new clients and new volunteer practitioners.

Financial Statement Notes

Part V, Line 4:

During the year ended December 31, 2013, the full amount of the permanently restricted funds was released by the donor to be applied to the services provided to indigent widows with cancer which was the original restriction of the grant. As a result there were no permanently restricted net assets at December 31, 2016.

IRS990/Desc0CLIENT SERVICESIn 2016, 300 certified, trained practitioners provided treatments to CMC clients. Evidence-based complementary integrative medicine (CIM) is the cornerstone of our mission. CIM addresses the most common complications that result in interruption or termination of cancer treatments. Acupuncture and herbs relieve cancer-related pain, nausea and debilitating neuropathy. Massage/ bodywork addresses pain by increasing circulation, reducing swelling and preventing the formation of excess scar tissue. Topical herbs and ointments provide relief from radiation burns. Tinctures provide symptom relief at end of life and at any time swallowing is difficult. Exercise counseling helps extends range of movement post-radiation. Nutrition counseling helps manage dietary needs during chemotherapy.We provided treatment to 600 women in 2016 with approximately 13,000 hours of treatment. Clients are scheduled taking into account cancer stage, current treatment regimen, and symptomology. This careful prioritization occurs through extensive case management.Each cancer patient is provided with a survivorship care plan that includes the role of nutrition, exercise, social support, and stress reduction in extending quality of life and reducing the risk of cancer recurrence. This kind of plan is mandated for all cancer patients at the end of their treatments, but is rarely delivered in the hectic environment of busy oncology practices.The breadth of client services includes the weekly Clinic Food Pantry. By working with local organic farms, Acme Bread, and Daily Bread, CMC can provide free organic produce and bread every clinic day.Our cultural and linguistic competency has dramatically increased the rate at which underserved, monolingual women are referred to us for integrative cancer care and supportive services, including advocacy and navigation to sustain access to the recommended continuum of cancer care.In July 2016, CMC organized Facilitating Wholeness, a free daylong event for African American women with metastatic breast cancer. The first of what promises to be an annual event, Facilitating Wholeness educated the local community on the benefits of integrative medicine in cancer care and offered breakout sessions for participants to experience acupuncture, jin shin jitsu, guided imagery, and self-care for lymphedema. The event included sessions on cancer and nutrition as well as movement therapy.

IRS990/ProgSrvcAccomActy2Grp/Desc0SOCIAL SERVICESCMC works holistically with low-income women, understanding and supporting the significant social services needs of clients. Demands of housing, finances, food, transportation, and familial support often push cancer treatments from the forefront. For this reason, CMC has a vigorous, proactive on -site program of social services to provide practical, emotional, and financial support to address the crises most likely to interfere with unpaid rent, utilities, and food shortages, the most common reason low-income women are forced to delay or abandon potentially life-saving treatments. Grocery cards supplement their often scant food supply and provide access to basic foods required for their families. In 2016, CMC partnered with Ceres, an organization providing organic, healthy prepared meals to people with advanced illness. CMC and Ceres provided meals to a small pilot cohort of CMC clients for a period of 8 weeks. This pilot program was a success and the program will be fully implemented at CMC for other client cohorts in 2017.Because most of our clients are contending with advanced cancers, keeping them on course with chemotherapy or other cancer treatments is essential to their survival chances. We seek out and foster strong relationships with our partner organizations to ensure that referrals happen rapidly and without undue paperwork and application appointments. In 2016, CMC partnered with Alameda Health Systems patient navigator program, Women's Cancer Resource Center, and Project Open Hand to deepen collaboration, enhancing speed of service provision for clients.

Their consistent efforts at recruitment, recognition, and retention is fundamental to our ability hold 300 committed volunteers within our community, all of whom are highly skilled, compassionate providers.In 2016, we held four trainings which added 55 new volunteers to our team. The weekend long event includes training on the biology of cancer, clinic operations, client needs, and cultural humility. Note that we do not allow any practitioners to provide treatment in the clinic until their volunteer training is complete.A volunteer from the Health Insurance Counseling and Advocacy program donates his expertise to help our clients understand and apply for health insurance and public benefits. He also works with them to create wills, power of attorney, and other legal documents they may need.In 2016, CMC conducted one lymphedema workshop, open to the public, that included a combination of information and exercise breaks that were focused on self-help tools for lymphedema. The class offered both Spanish and Cantonese interpretation.CMC conducted one reflexology class, open to the public, that educated the community on the benefits of reflexology for cancer and other disease care as well teaching self-care tools.

Increasingly high rents have forced a number of non-profits to relocate or close their doors. CMC now maintains only one location in Oakland. One staff member was laid off and others reduced their hours. Approximately 70% of our SF-based clients as well as over 50% of our volunteer practitioners were unable to make the transition to Oakland. As a result, we served fewer clients in the following months of 2016. CMC continues to conduct outreach to bring in new clients and new volunteer practitioners.
