“Having health coverage means consistent access to comprehensive and continuous medical and preventative services for people to get and stay healthy, a much better alternative to episodic care at emergency departments. KP is committed to advancing equity and removing disparities in access to coverage and health outcomes. i. Coverage provided through Medicaid, CHIP and other government programs The Affordable Care Act has had a far-reaching impact on the landscape of government-sponsored programs, as these options have become a key source of health coverage for a significant portion of the US population. Kaiser Permanente has organizational strategies to enable individuals with low incomes to obtain and/or retain health coverage through Medicaid, CHIP or other government programs. At the end of 2023, Kaiser Foundation Hospitals was providing access to care to over 1.1M people through these government programs. ii. Coverage provided through Charitable Health Coverage (CHC) Kaiser Permanente's CHC programs provide health care coverage to low-income individuals and families who don't have access to other public or private health coverage. CHC programs work by enrolling qualifying individuals in a Kaiser Permanente Individual and Family Health Plan. Through CHC, members' monthly premiums are subsidized, and members do not have to pay copays or out-of-pocket costs for most care at Kaiser Permanente facilities. Through CHC, members have a medical home that includes comprehensive coverage, preventive services and consistent access through the "front door" of the health delivery system.”

In addition to the individuals who received health coverage in 2023 due to Kaiser Permanente's participation in these government programs, Kaiser Foundation Hospitals also provided care to over 220,000 people who are enrolled in these programs but not formally assigned to Kaiser Permanente. ii. Care subsidized by Medical Financial Assistance (MFA) KP's Medical Financial Assistance program (MFA) improves health care access for people with limited incomes and resources and is fundamental to Kaiser Permanente's mission. Our MFA program helps low-income, uninsured, and underinsured patients receive access to care. The program provides temporary financial assistance or free care to patients who receive health care services from our providers, regardless of whether they have health coverage or are uninsured. Eligibility for financial assistance is based on financial need. In general, patients whose household income is at or below 300%, and in some regions up to 400%, of the federal poverty limit are eligible for the MFA program. Patients who are experiencing high medical expenses as compared to their income may be eligible under high medical expenses criteria, regardless of household income. The MFA program covers emergency and medically necessary health care services, pharmacy services and products, and medical supplies provided at Kaiser Permanente facilities (such as hospitals, medical centers, and medical office buildings), at Kaiser Permanente outpatient pharmacies, or by Kaiser Permanente providers. Over the course of 2023, Kaiser Foundation Hospitals subsidized care for a total of 200,000 people through the MFA program.”
“Kaiser Permanente also increases access to health care in the community by establishing safety net partnerships. Safety net providers are a mix of public hospitals, community-based organizations such as federally qualified health centers, and other health care organizations. Safety-net health care providers provide care for patients who have low incomes and/or are uninsured regardless of their ability to pay. In Kaiser Permanente service areas, federally qualified health centers serve the uninsured, Medicaid, and other vulnerable populations. Strengthening the safety net advances our mission to improve the conditions for health and equity in the community. Our communities' most vulnerable populations, including a disproportionate number of low-income people of color, rely on the safety net for their health care needs. Our support ensures that communities have access to a strong safety net that can equitably meet patients' needs and improve health outcomes for low-income patients including people experiencing homelessness. In 2023, Kaiser Permanente continued its partnership with the California Department of Health Care Services (DHCS) and the California Primary Care Association (CPCA) to support 32 Community Health Centers (CHCs) in California, representing 8 counties to implement the Population Health Management (PHM) model at each health center. Throughout 2023, Kaiser Permanente provided practice coaching and subject matter expertise to support the 32 CHCs on Building the Foundation competencies which included data quality and reporting, empanelment, business case, evidence-based care teams, and launched an official external website to share resources. Additionally, Kaiser Foundation Hospitals partnered with The University of Chicago to support the evaluation design for the California Safety-Net Population Health Management Initiative. Over the next 3.5 years, research will evaluate the effectiveness of the PHMI intervention, outcome measures, and barriers encountered. Kaiser Foundation Hospitals also partnered with the Population Health Management Learning Center to provide foundational resources and technical assistance approaches to support primary care practices that serve individuals enrolled in Medi-Cal across the state of California. The goal of the Learning Center is to be a catalyst for aligned statewide population health transformation for Medi-Cal and adjacent safety net programs. Additionally, Kaiser Foundation Hospitals' Safety Net Partnership Initiative Clinic Consortia Cohort addresses health care disparities and health equity by supporting health care workforce, value-based care, and technology efforts leading to safety net care delivery improvements. Kaiser Foundation Hospitals awarded 12 community clinic consortia across California over $4.1M to support community-based health centers including clinic training programs, transition to value-based care and alternative payment methodologies, and improvements in their technology infrastructure. “Without a safe place to call home, it's nearly impossible to focus on basic health and medical needs. For years, Kaiser Permanente has led efforts to end homelessness and preserve affordable housing. We do this by investing in solutions, helping shape public policy, and forming innovative partnerships. Inadequate housing contributes to several health problems, including chronic diseases, injuries, and poor mental health. Housing security is a crucial health issue for disadvantaged populations, such as low-income communities and communities of color. Working to solve housing insecurity is part of Kaiser Permanente's focus on improving equitable conditions for people of color in America. We are addressing historical disinvestment in our communities, which is directly tied to racism and racial inequities. This kind of disinvestment and lack of economic opportunity can impact affordable housing. Through innovative partnerships and strategic investments, Kaiser Permanente is working to bridge the unique strengths of the health and housing sectors, to strengthen neighborhoods, drive equitable health outcomes and help communities thrive. Kaiser Foundation Hospitals awarded 17 organizations across California and Oregon over $2M of which $1.36M was paid in 2023 as part of the Medical Respite initiative which aims to strengthen the ecosystem of medical respite providers to better align with the Standards for Medical Respite Programs, advance best practices, and enhance state-of-the-field knowledge in medical respite. Medical respite programs provide post-acute care for people experiencing homelessness who are too ill or frail to recover from an illness or injury on the street or in shelter, but who do not require hospital level care. “Social determinants of health - the conditions in which people live, learn, work, and play - affect a wide range of health risks and outcomes. Access to quality health care is critical, but social factors, such as lack of safe housing and transportation, inadequate education or job opportunities, and poor access to good nutrition and physical activity, inevitably create barriers to health and wellbeing. While many social resources may exist in a community or through government programs, they are often unknown to those most in need and disconnected from one another even when serving the same person, creating confusion and exacerbating stress for communities. We launched the Kaiser Permanente Community Support Hub in 2023 as a support center dedicated to helping people meet their basic needs so they can achieve good health. The Kaiser Permanente Community Support Hub's resource directory is a searchable database of tens of thousands of community-based programs and services that anyone can explore online at kp.org/communityresources at their own convenience. It is accessible anytime for anyone seeking help with basic needs - members and nonmembers alike - to explore and connect with social services and assistance programs in their communities. The hub operates with a proactive service model, using text, email, and phone outreach to facilitate social health screenings and offer support connecting to local community-based resources and public benefits. The hub also operates a toll-free call center. Kaiser Foundation Hospitals partnered with several organizations across California to build and expand networks of community-based organizations in our communities that work with us and other health care organizations to address social health needs of our patients and to coordinate care. These networks include 211 organizations, which offer additional information and referral services that connect people to vital social, health, and disaster services provided by a range of nonprofit and government agencies. “Our health professions education programs include both physician training programs like Graduate Medical Education (GME) and non-physician training programs (i.e., residency programs). Our GME program provides training and education for medical residents and interns in the interest of educating the next generation of physicians. Residents are offered the opportunity to serve a large, culturally diverse member and patient base in a setting with sophisticated technology and information systems, established clinical guidelines and an emphasis on preventive and primary care. In a study of postpartum individuals, researchers evaluated the extent to which the association between neighborhood disadvantage and postpartum depression differs by race and ethnicity. The study found that residing in more disadvantaged neighborhoods was associated with postpartum depression among non-Hispanic individuals, with the risk being greatest among Black individuals. Neighborhood disadvantage was not associated with postpartum depression among Hispanic individuals. The researchers concluded in JAMA Open Network that geographic targeting of mental health interventions may decrease postpartum mental health inequities. In 2023, the Department of Research & Evaluation published several research studies, a summary of one publication includes: The COVID-19 pandemic necessitated rapid adoption of telehealth. However, its safety in subspecialty clinical practice remains uncertain. Patients with cardiovascular disease have increased mortality risk and are especially vulnerable to COVID-19 infections. They are also at increased risk for cardiovascular events if routine clinical assessments are not conducted optimally. Researchers investigated the clinical outcomes of patients with coronary artery disease and/or heart failure evaluated in cardiology clinics via telehealth visits during the COVID-19 pandemic. Their outcomes were compared with those of pre-pandemic patients with cardiovascular disease seen in the first 6 months of 2019. In a study published in the American Journal of Cardiology, the researchers concluded that telehealth can be safely incorporated into ambulatory cardiology practices.
