“Having health coverage means consistent access to comprehensive and continuous medical and preventative services for people to get and stay healthy, a much better alternative to episodic care at emergency departments. Coverage is good for the patient, good for Kaiser Permanente and good for the US overall because it helps people get and stay healthy and avoid costly hospital services. i. Coverage provided through Medicaid, CHIP and other government programs The Affordable Care Act has had a far-reaching impact on the landscape of government-sponsored programs, as these options have become a key source of health coverage for a significant portion of the US population. Kaiser Permanente has responded to this challenge by developing organizational strategies to enable low-income individuals to obtain and/or retain health coverage through Medicaid, CHIP or other government programs, even as their personal or financial circumstances may be changing. At the end of 2020, Kaiser Foundation Hospitals was providing access to care to over 908,000 people through these government programs. ii. Coverage provided through CHC CHC is a unique approach to caring for low-income uninsured persons in the community. The program provides a premium subsidy for a KFHP off exchange plan to low income individuals and families who are not eligible for other public or privately sponsored coverage. Eligible participants receive a regular KFHP membership card and have access to the same services and providers as other KFHP individual and family plan members. CHC members also receive cost sharing support that eliminates out of pocket costs for most covered services provided at Kaiser Permanente facilities. CHC provides individuals/families who would otherwise not have access to coverage, consistent access through the "front door" of the health delivery system, including a medical home and preventive services, better alternatives than episodic care. At the end of 2020, Kaiser Foundation Hospitals was providing access to care to over 5,000 people through the CHC program. B. Care To get and/or stay healthy, people need access to high quality care by providers they trust. This care must include preventative services and required medications so that people can avoid ending up in the emergency room or requiring more extensive services down the line. Kaiser Permanente helps low income populations gain access to this type of care by leveraging the full scope of its integrated delivery system, including not only critical hospital-based services but also outpatient primary, specialty and pharmacy services. i. Care provided through Medicaid, CHIP and other government programs Kaiser Permanente provides a wide range of health care services to individuals enrolled in Medicaid, CHIP and other government programs, regardless of whether they are assigned to Kaiser Permanente or not. In addition to the individuals who received health coverage in 2020 due to Kaiser Permanente's participation in these government programs, Kaiser Foundation Hospitals also provided care to over 179,000 people who are enrolled in these programs but not formally assigned to Kaiser Permanente. ii. Care subsidized by MFA Medical Financial Assistance program (MFA) helps low-income, uninsured, and underserved patients receive access to care. The program provides temporary financial assistance or free care to patients who receive health care services from our providers, regardless of whether they have health coverage or are uninsured. The MFA program is one of the most generous in the health care industry and is available to those patients in greatest need. Eligibility is based on financial need. In general, patients whose household income is at or below 200 percent, and in some regions up to 400 percent, of the federal poverty guidelines are eligible for the MFA program. “Rooted in the belief that nobody should go hungry in America, the nation's largest integrated, nonprofit health system is again going beyond traditional care delivery to tackle one of the most basic human needs - access to affordable, healthy food. Before COVID-19, 1 in 9 households in the United States were at risk for food insecurity, the lack of consistent access to adequate food because of limited resources. Feeding America now estimates this to be 1 in 6 adults and 1 in 4 children, with the potential to double as COVID-19 continues to unfold. Food for Life is a comprehensive approach for transforming the economic, social, and policy environments connected to food so that people across the nation have access to, and can afford, healthy food. Kaiser Permanente is supporting diverse community partners who are increasing purchasing power for food, providing meal and nutrition distribution with an emphasis on equity, and driving essential change in food system policy and research. The following demonstrates the range of community organizations supported through partnerships and funding to address the rising food security demands in response to the COVID-19 pandemic. Kaiser Foundation Hospitals funded $1.7 million to organizations across Northern California to ensure that all eligible individuals in our communities have access to governmental nutrition programs like CalFresh. This funding creates innovations for outreach and enrollment in the time of COVID-19, strategies that advance equity or target underserved communities, and allows for participation in national, state, and local policy activities that increase access to healthy, affordable food for at-risk populations. The impact will be measured both in how CalFresh enrollment is increased as well as desired sustainable policy and practice changes. Kaiser Foundation Hospitals supported Nourish California's "Fair access to food for a healthy life" project, which aims to address inequities in food access across the state of California. The target population is Californians below 200% of the Federal Poverty level. The partnership will also be used to address stark inequities faced by Black, Latinx, and immigrant households. Some of the work will address immediate needs emerging from the current health and economic crises but most of the efforts will focus on putting policy and system changes in place for the long term. Kaiser Foundation Hospitals funded $200,000 to Alameda County Sheriff's Deputy Activities League. The Dig Deep Farms Food Hub Expansion program will increase food security and economic empowerment. This program will create a network of integrated food businesses in the community that collectively improve access to healthy food, create jobs, incubate small businesses, and boost the livelihood of those who cultivate, grow and sustain local farms and local businesses. The grant will provide additional internships to workers reentering the community from incarceration; expand the number of hours available for urban farmers and "farmacy" staff for expanding Food as Rx operations; and grow food entrepreneurship among low income county residents while also distributing meals and produce to food-insecure residents.”
“Inclusive economic growth is critical to both individual and community health. When there is a lack of economic opportunity in communities, the prospects for upward social mobility are diminished, often resulting in poorer health and higher mortality rates for people living in those communities. By contrast, economic growth and opportunity provides individuals with jobs, income, a sense of purpose, and opportunities to improve their economic circumstances overtime. As a large, influential institution in our communities, Kaiser Permanente also recognizes that the way we do business can support economic opportunity in local communities through how we hire, purchase, build our facilities and partner with communities. Some ways that we're helping revitalize and grow our communities by strengthening economic opportunity include: - Providing good jobs to individuals facing barriers to employment through high-impact hiring and workforce pipeline efforts. - Pursuing a social impact investment strategy to support impact investments aimed at addressing key social issues that have a significant impact on health. - Purchasing goods and services from local minority- and women-owned businesses and encouraging good employment practices by our vendor partners. - Building new facilities with an emphasis on positive local community impact, including local construction hiring, local and diverse purchasing, healthy and sustainable design features, neighborhood revitalization, and deep community engagement. - Partnering with community organizations to grow local business capacity, increase access to good jobs, and support stable, quality, affordable housing. “Without a safe place to call home, it's nearly impossible to focus on basic health and medical needs. Kaiser Permanente is leading efforts to end homelessness and preserve affordable housing by making strategic impact investments, shaping policy, and catalyzing innovative partnerships. COVID-19 continues to impact social health needs in our communities, and people experiencing homelessness or housing insecurity are at increased risk. Nationwide, people of color are more affected by homelessness and housing insecurity, made worse by the pandemic. The health crisis and economic fallout from the pandemic are hitting low-income and communities of color hardest and threaten to widen the health equity gap in our country even further. As Kaiser Permanente continues to respond on the front lines of the COVID-19 pandemic, we are accelerating efforts to increase affordable housing, address homelessness, and advance equity in the communities we serve. Kaiser Permanente's multipronged approach to increase affordable housing and address homelessness helps improve the health outcomes of the people who live in the communities we serve. The following demonstrates our key efforts in 2020 to support housing insecurity and homelessness: Kaiser Foundation Hospitals partnered with Enterprise Community Partners to support the California Housing Services & Operating Subsidy Fund for Project Homekey. The partnership will establish a fund to support operating costs and wraparound service supports for new state-acquired housing units, meeting a critical need of permanent housing for people experiencing homelessness across the state and providing vital wraparound services to ensure the state's most vulnerable families and individuals remain safely housed now, during the COVID-19 pandemic, and for the long-term. Kaiser Foundation Hospitals' leadership in this endeavor will highlight the importance of cross-sector partnerships and strategies to solve the housing and homelessness crisis. Leveraging private resources with public funding will allow increased flexibility and implementation of bold solutions for long-term success including creating key data analysis to advance models of care that result in social and physical environments that promote good health and positive outcomes. Kaiser Foundation Hospitals provided $5 million in grant funding to the City of Sacramento to support rehousing shelters. The rehousing shelters aim to reduce unsheltered homelessness among individuals and families living in Sacramento by lowering barriers to entry and providing on-site supportive services, case management, housing coordination and workforce training. The rehousing shelters will also support 24/7 hours of operations to allow guests to remain on-site during both daytime and nighttime, incorporating accommodations for guests with pets, allowing for partners and adult family configurations, allowing for storage of larger amounts of personal belongings, and co-location of robust on-site support services. Kaiser Foundation Hospitals provided support to the City of Oakland to meet a critical need for interim housing and additional wraparound services for up to 90 homeless adults at one time. About fifty percent of the population served will be frail elderly persons. While in the program, participants will be provided a safe, clean, warm, and resource-rich environment. This includes prepared meals, laundry facilities, substance abuse support, and computer access. Participants will be assisted with benefits advocacy, employment support, budgeting, rental assistance, and mental health care. Through Kaiser Foundation Hospitals' ongoing partnerships with community organizations, municipal leaders, and public health champions, we are working to incorporate health, equity and sustainability considerations into public policy and the built environment in ways that influence how neighborhoods take shape and grow. In 2018, we joined forces with the de Beaumont Foundation, a leader in public health philanthropy, to help U.S. cities thrive through the CityHealth initiative. “Kaiser Permanente has a long history of conducting health research related to both prevention and treatment of disease that benefits its members, the communities it serves and the nation. Kaiser Permanente's research efforts are core to the organization's mission to improve population health, and its commitment to continued learning. Kaiser Permanente researchers study critical health issues such as cancer, cardiovascular conditions, diabetes, behavioral and mental health, and health care delivery improvement. Kaiser Permanente's research is broadly focused on three themes: understanding health risks; addressing patients' needs and improving health outcomes; and informing policy and practice to facilitate the use of evidence-based care. Kaiser Permanente is uniquely positioned to conduct research due to its rich, longitudinal, electronic clinical databases that capture virtually complete health care delivery, payment, decision-making and behavioral data across inpatient, outpatient, and emergency department settings for its geographically and demographically diverse members. In 2020, Kaiser Foundation Hospitals engaged in almost 1,600 active studies (including 530 clinical trials), and published 1,920 journal articles. Kaiser Foundation Hospitals' research center, the Division of Research (DOR) conducts research on a broad range of topics, including behavioral health and aging, cancer, cardiovascular and metabolic conditions, health care delivery and policy, and vaccines. Poor glycemic control can cause damage to various organs and lead to disabling and life-threatening complications such as cardiovascular disease, chronic kidney disease, and retinopathy (the most common cause of irreversible blindness in adults). Research findings presented at the 56th annual meeting of the European Association for the Study of Diabetes, suggest that housing insecurity may be a risk factor for poor blood sugar control. Researchers examined the relationship between housing insecurity and diabetes control in over 25,000 Northern California patients with type 2 diabetes. The researchers concluded that people with type 2 diabetes who have trouble paying rent, move frequently, stay with relatives, or spend the majority of household income on housing, may struggle with maintaining a routine, medication adherence, and prioritizing their diabetes management. Kaiser Foundation Hospitals' research center, the Department of Research & Evaluation conducts research on a broad range of topics including cancer, cardiovascular diseases, diabetes, health services research and implementation science, and obesity. Given the unique clinical features of COVID-19, physicians required targeted information to inform clinical decisions regarding patients with symptoms suspicious for COVID-19. A study published in the American Journal of Emergency Medicine reported on the developed tool, called COVAS, which looks at co-morbidities, obesity, vital signs, age, and sex. Researchers conducted a retrospective study of 26,600 emergency department visits by adults for symptoms suspicious for COVID-19 at 15 hospitals in Southern California between March 1, 2020 and April 30, 2020. This was a period of time when there was substantial community spread of COVID-19 but COVID-19 test results were not immediately available. The study concluded that physicians evaluating patients with pneumonia, COVID-19, or symptoms suspicious for COVID-19, can apply the COVAS score to assist with decisions to hospitalize or discharge patients during the COVID-19 pandemic.
