The mission of bluepath service dogs, inc. Is to provide autism service dogs, offering safety, companionship and opportunities for independence. Bluepath's expertly trained autism service dogs work alongside parents and caregivers to prevent wandering. The dogs significantly lessen parents' safety concerns and help children to lead fuller lives. Form 990, Part VI, Section B, Line 11B

Bluepath service dogs (the corporation) engaged an outside cpa to perform all accounting services and bookkeeping functions for the corporation. The cpa prepares financial statements which are reviewed by another independent accounting firm. After the financial statements are reviewed, the outside cpa prepares the form 990. A draft form 990, was provided to the president and each director of the corporation by electronic mail prior to filing with the irs. Each director was asked to review the draft form 990 and provide comments or questions. The president, officers, and management of the corporation were in frequent communication with the outside cpa by electronic mail and telephone to provide information, raise questions and provide comments on the form 990 prior to filing with the irs. After all questions and comments have been addressed, the final form 990 is prepared and submitted to the president of the corporation for his approval and signature. The form 990 is then electronically filed with the irs.

Form 990, Part VI, Section B, Line 12C

Each director, principal officer and member of a committee of the board of directors with delegated powers must annually sign a statement affirming that he or she has received a copy of the conflicts of interest policy, has read and understands the policy, and has agreed to comply with the policy. All officers, directors and management employees must disclose any potential or actual conflict of interest to the board of directors promptly upon learning of such potential or actual conflict. In the event that a director, officer or management employee has or proposes to enter into a transaction that would cause said person to be an interested person, or has a financial interest that is, or may be an actual or possible conflict of interest with the corporation, the interested person shall promptly disclose the existence of the financial interest and all material facts to the board of directors in order to permit the board of directors to consider the proposed transaction or arrangement. If the board of directors has reasonable cause to believe a director, officer or management employee has failed to disclose actual or possible conflicts of interest, it shall inform said person of the basis for such belief and afford him/her an opportunity to explain the alleged failure to disclose. If, after hearing said person's response and after making further investigation as warranted by the circumstances, the board of directors determines the person has failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective action. Any director may recuse himself or herself at any time from involvement in any decision or discussion in which the director believes he or she has or may have a conflict of interest without going through the process for determining whether a conflict of interest exists.

Part I Summary, Line 1

The mission of bluepath service dogs, inc. Is to provide autism service dogs, offering safety, companionship and opportunities for independence. Bluepath's expertly trained autism service dogs work alongside parents and caregivers to prevent wandering. The dogs significantly lesssen parents' safety concerns and help children to lead fuller lives. Reduced stress, improved sleeping routines and more meaningful social interactions can allow families to feel a renewed sense of hope and empowerment. Bluepath's autism service dog program includes puppy raising, formal training and placement components. Volunteer puppy raisers devote their time and expertise to preparing labrador and golden retriever puppies to become well-adjusted service dogs. Beginning at eight weeks of age, puppies are taught basic obedience skills and appropriate home behavior. Raisers expose the eager pups to all the world has to offer, enabling them to develop a reference library of sights, sounds, and experiences needed to become well-trained service dogs. At approximately 18 months of age, bluepath dogs begin formal training during which time a professional instructor builds on the foundation the puppy raiser has instilled, teaching the dog increasingly advanced behaviors. Dogs acquire skills like "anchoring," (learning how to stop a child from wandering or bolting), and "visit," (providing emotional support by the dog resting its head on the child). Integral to the formal training of bluepath service dogs is the "kids and canines learning together program" which enables future service dogs to learn in real-life classroom settings, and to gain the specific skills needed to keep children with autism safe. Dogs work with a professional trainer in classrooms of children with autism, special needs and blended classes over an eight-month period. Nearly 1,000 children benefit from this program annually. At approximately two years of age, bluepath service dogs are ready to be placed in the homes of families with children with autism. Bluepath currently serves families with children with autism between the ages of 3 and 11 years old who live within a two-hour radius of hopewell junction, ny. Lifetime follow-up support is provided. Dogs and lifetime support are free of charge.

The organization has been classified as an entity that is not a private foundation within the meaning of section 509 (a) and qualifies for deductible contributions as provided in section 170 (b) (1) (a) (vi). The organization adopted the provisions of accounting principles generally accepted in the united states of america regarding accounting for uncertain tax positions. Management evaluated the organization's tax positions and concluded that the organization had taken no uncertain positions that would require adjustment to the financial statements in order to comply with the provisions of this guidance.

REDUCED STRESS, IMPROVED SLEEPING ROUTINES AND MORE MEANINGFUL SOCIAL INTERACTIONS CAN ALLOW FAMILIES TO FEEL A RENEWED SENSE OF HOPE AND EMPOWERMENT.
