“BluePath Service Dogs (the Corporation) engaged an outside CPA to perform all accounting services and bookkeeping functions for the Corporation. The CPA prepares financial statements which are reviewed by another independent accounting firm. After the financial statements are reviewed, the outside CPA prepares the Form 990. A draft Form 990, was provided to the President and each Director of the Corporation by electronic mail prior to filing with the IRS. Each director was asked to review the draft Form 990 and provide comments or questions. The President, officers, and management of the Corporation were in frequent communication with the outside CPA by electronic mail and telephone to provide information, raise questions and provide comments on the Form 990 prior to filing with the IRS. After all questions and comments have been addressed, the final Form 990 is prepared and submitted to the President of the Corporation for his approval and signature. The Form 990 is then electronically filed with the IRS.”
“Each director, principal officer and member of a committee of the board of directors with delegated powers must annually sign a statement affirming that he or she has received a copy of the conflicts of interest policy, has read and understands the policy, and has has agreed to comply with the policy. All officers, directors and management employees must disclose any potential or actual conflict of interest to the board of directors, promptly upon learning of such potential or actual conflict. | IRS990/Desc | 0 | Bluepath Service Dogs sole program is the provision of service dogs to children with autism. The program includes three sub-programs: Puppy Raising, Kids and Canine Learning Together and Formal Training. In the Puppy Raising Program, volunteer puppy raisers teach puppies basic obedience skills and appropriate house manners. They socialize the puppies to everything the world has to offer, enabling them to develop comfort with a variety of sights, sounds and experiences. With guidance from professional training staff, puppy raisers give the dogs a solid foundation and prepare them for the next phase of their service dog training. Fifteen dogs were active in the puppy raising part of the program in 2017. The Kids and Canines Learning Together Program places BluePath dogs in educational settings where theyre exposed to children with a variety of special needs. The dogs learn to remain calm and focused despite a high level of distraction, while the kids learn important life skills through interaction with the dogs. At about 14 months of age, BluePath dogs begin Formal Training with a professional instructor and learn more advanced behaviors. These include skills like anchoring, when a dog learns how to stop a child from wandering or bolting, and visit, in which a dog rests his head on a child to calm and provide support. At approximately two years of age, BluePath dogs are ready to be placed with families with children with autism. Lifetime follow-up support is provided. |

A draft Form 990, was provided to the President and each Director of the Corporation by electronic mail prior to filing with the IRS. Each director was asked to review the draft Form 990 and provide comments or questions. The President, officers, and management of the Corporation were in frequent communication with the outside CPA by electronic mail and telephone to provide information, raise questions and provide comments on the Form 990 prior to filing with the IRS. After all questions and comments have been addressed, the final Form 990 is prepared and submitted to the President of the Corporation for his approval and signature. The Form 990 is then electronically filed with the IRS. |
