Governance Explanations

Form 990, Part VI, Line 4: Description of Significant Changes to Organizational Documents

In compliance with the HEARTH Act passed in May 2009, and the HEARTH Act Interim Rule in effect beginning August 30, 2012 and detailed in 24 CFR 578, a new separate Continuum of Care Board of Directors was created by charter and the Tarrant County Homeless Coalition Bylaws were amended in September 2013. The TCHC Board of Directors no longer serves simultaneously as the Continuum of Care Board for the Fort Worth/Arlington/Tarrant County CoC TX601 as recognized by HUD. These duties are now transferred to the TX601 Continuum of Care Board of Directors.Key changes to the TCHC bylaws involved removing from eligibility to serve on the TCHC board any agency or representative of an agency that is a recipient, sub recipient or applicant of any McKinney Vento funds. Additionally, established committees including the Community Projects Review Committee and the HMIS Governance Committee were removed and transferred to the Continuum of Care Board of Directors. TCHC now serves as the Collaborative Applicant charged with the operations of the Continuum of Care and is authorized by the Continuum of Care Board to apply for CoC Planning Funds from HUD and represents the Continuum of Care in applying for the Continuum of Care Homeless Assistance program of approximately $12.1 million.

OTHER PROGRAM SERVICES 4: Direct Client Services Fund - TCHC administers the United Way and city of Fort Worth funded direct client services fund, which is a flexible fund of last resort to provide rapid assistance for items that are not covered by other grants or programs. Assistance provides support for client employment and training, housing stability or acquisition, transportation, and health care services. TCHC has developed coordinated centralized contracts for use of the fund for dental services, beds for newly housed homeless, and securing rapid resolution to Texas Department of Public Safety surcharges through their amnesty program to resolve often years-long barriers to getting an identification in order to find work or housing.

IRS990/Desc0HUD HMIS - The Continuum of Care Board of Directors, by resolution, appointed TCHC as the administrator of the Homeless Management Information System (HMIS). The HMIS is a web-based client and services database for 139 homeless housing and services programs involving over $24 million in federal, state and locally funded programs provided by 38 nonprofit and governmental agencies. The HMIS is used for data collection, grants management, data analysis, program monitoring and performance reporting to funders and the community. The HMIS enhances coordination of care by and between housing, supportive services, mental health, behavioral health, health care, and first responders. TCHC maintains and monitors the quality of data in the HMIS, trains users, and issues HMIS identification scan cards to homeless individuals to accurately enumerate the number of homeless persons and record utilization of services. TCHC manages the TB screening process through the HMIS in collaboration with the Tarrant County Public Health Department, significantly reducing the incidence of TB among the high risk homeless population. The HMIS use has expanded use by the JPS Health Network in order to improve and streamline the delivery of health care services and provide supporting documentation for the DSRIP Medicaid Waiver Program Care Connections for the Homeless.

IRS990/ProgSrvcAccomActy2Grp/Desc0Direct Client Services Fund - TCHC administers the United Way and city of Fort Worth funded direct client services fund, which is a flexible fund of last resort to provide rapid assistance for items that are not covered by other grants or programs. Assistance provides support for client employment and training, housing stability or acquisition, transportation, and health care services. TCHC has developed coordinated centralized contracts for use of the fund for dental services, beds for newly housed homeless, and securing rapid resolution to Texas Department of Public Safety surcharges through their amnesty program to resolve often years-long barriers to getting an identification in order to find work or housing.

IRS990/ProgSrvcAccomActy3Grp/Desc0Continuum of Care Planning - The continuum of care board of directors, by resolution, appointed TCHC as the collaborative applicant and lead agency to carry out continuum of care operations, as required by HUD. The continuum of care program grant authored and submitted by TCHC on behalf of the community is subject to national competition. The grant is an exhaustive document that gives details on how the community operates, monitors, evaluates, improves and develops its emergency and stabilization systems and programs for persons how are at risk of or experiencing homelessness. Continuum of care operations also includes the new Coordinated Assessment System which provides a Homeless Helpline from 8 am 10 pm daily to provide information and referral and divert persons with a housing crisis from entering in to the homeless system. The system also provides a mechanism for identifying, documenting and prioritizing persons seeking assistance to assure that programs designed to serve the chronically homeless, veterans, families with children, or presenting with other disabilities are targeted and serving those with the highest level of need. The system also maintains an inventory of housing and intervention resources to assure that programs are at their highest occupancy and maintain the most efficient use of available public resources.
