Activity 2

Continuum of Care - The continuum of care board of directors, by resolution, appointed TCHC as the collaborative applicant and lead agency to carry out continuum of care operations, as required by HUD. The continuum of care program grant authored and submitted by TCHC on behalf of the community received a high score, resulting in receipt of an additional $700,485 award for more permanent supportive housing. TCHC began the process of complying with changes in continuum of care governance as detailed in HUD interim rule of The Hearth Act. TCHC was selected to present its best practice of the JPS Homeless Dental Health Program at the National Health Care for the Homeless Symposium. The TCHC Executive Director was appointed to the National Governing Board of the Health Care for the Homeless National Council. TCHC developed a web-based housing inventory database that provides instant searches of assisted and affordable housing to expedite the process of identifying housing interventions to end homelessness.

Activity 3

Direct client services fund - TCHC administers the United Way and city of Fort Worth funded direct client services fund, which is a flexible fund of last resort to provide rapid assistance for items that are not covered by other grants or programs that support employment and training, housing stability or acquisition, transportation, and health care services. In 2012, 1,760 persons benefited from the program, including 47 homeless persons receiving comprehensive dental services and dentures.
