Activity 2

MEDICAL SERVICES - The Outreach to Health Community Clinic provides medical care and prescriptions to GRACE clients with no health insurance and marginal means to access care elsewhere. The clinic provided 2,548 patient visits this year. Over 200 volunteer medical professionals provide general practice services; referrals are made for clients needing specialty treatment. Well-Woman Clinics provided education, screening, and testing. A monthly Diabetic Education Program also provides ongoing care for diabetic patients and educational seminars for clients and the public. These volunteers provided 2,936 hours of time to GRACE. We have added limited specialty services like Cardiology and Epilepsy. Additionally the Clinics provided medicine and/or medical supplies to over 453 patients. We are members of the Texas Association of Charitable Clinics.

Activity 3

TRANSITIONAL HOUSING - The program provides long-term assistance and case management for homeless individuals and families willing to work toward becoming self-sufficient. Case management appointments are conducted weekly and volunteer case aids assist with day-to-day functioning. This program includes housing and basic needs based on the clients progress towards self-sufficiency. The program is available to the families for a minimum of six months and a maximum of two years.
