Activity 2

TRANSITIONAL HOUSING - The program provides long-term assistance and case management for homeless individuals and families willing to work toward becoming self-sufficient. Case management appointments are conducted weekly and volunteer case aids assist with day-to-day functioning. This program includes housing and basic needs based on the client's progress towards self-sufficiency. The program is available to the families for a minimum of six months and a maximum of two years.

Activity 3

MEDICAL SERVICES - The Medical Clinic provides care and prescriptions to GRACE clients with no health insurance and marginal means to access care elsewhere. There were over 200 Volunteer medical professionals providing over 2,500 hours of general practice services at the Clinic; referrals are made for clients needing specialty treatment. Well- Woman Clinics and Diabetic Education programs provided education, screening, and testing; along with ongoing care for patients. We have expanded to include educational seminars for clients and the public. As GRACE has grown and adjusted to the needs of the community services such as Cardiology and Epilepsy and dental have been added. The estimated value of donated professional services is $255,960 and we are a member of the Texas Association of Charitable Clinics.
