Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: Housing ProgramsOur Housing resources program provides housing counseling and referrals to over 2,000 desperate households each year. We do this through individual interviews focusing on rental history, credit issues, special needs and potential barriers and income. We provide housing referrals that realistically match the housing option to the household. We temporarily house individuals sent from Sutter Hospital while WellSpace works with them to obtain permanent supported housing. We also provide short term housing to homeless individuals referred by the Cathedral of the Blessed Sacrament and the City of Elk Grove. In 2016, 296 homeless individuals stayed in SSHH short term housing, and 135 successfully moved into permanent housing which included the Common Cents and Coordinated Exit programs. The Community Outreach program develops Homeless Assistance Resource Teams in communities throughout Sacramento County. Significant groups of volunteers have formed in Elk Grove, Carmichael, Rancho Cordova and Citrus Heights. They organize Winter Shelter programs, Veteran Stand Downs, transitional housing and volunteer housing counseling in their local communities. Through the Community Outreach efforts we served 2,139 households in 2016. OTHER PROGRAM SERVICES 5: Veterans Affairs Supportive Housing (VASH)SSHH staff provides housing counseling and referral services for veterans in crisis. SSHH staff partners with the Veterans Affairs Northern California Healthcare System, and since 2010, assessed veterans needs, provided a list of housing options, and then assisted them in completing the applications. We housed 12 veterans in 2016 and a total of 95 since 2010. OTHER PROGRAM SERVICES 6: Renter's HelplineRenters Helpline is available to tenants who are experiencing difficulties with the landlord. SSHH sorts out over the phone the nature of the problem, provides information on renters rights and responsibilities and offers to mediate with the landlord to resolve the difficulty. In some cases when situation appears to be discriminatory there is a referral to our fair housing attorney for further investigation. In 2016 the Renters Helpline responded to over 8,286 calls, helping 66% of families maintain their housing, and assisting 33% of the families in relocating to appropriative permanent housing. Our fair housing partner, Project Sentinel, was asked to review 543 of the cases. Of those 543 referrals, 42 turned out to be serious cases which were closely investigated and in a few cases legal action was taken. Over half of the cases were related to discrimination on the basis of a disability, reflecting the needs of some of our most vulnerable residents and people. OTHER PROGRAM SERVICES 7: Sacramento Employment Training Agency (SETA)SSHH recognizes that housing instability can be a significant barrier to employment. Since 2014, SSHH has provided housing counseling to SETA clients while receiving employment and training services. This helps lower-income families become self-sufficient through training and employment. We enrolled 117 individuals or families in 2016, helping them achieve their long-term success. OTHER PROGRAM SERVICES 8: Coordinated ExitAn interim housing program which provides a short term place to live while people work on their housing barriers and then move on into their own permanent housing. Ready to Rent curriculum is taught, employment opportunities or disability benefits are obtained, and we assist in locating decent housing for them. During 2016, a total of 87 persons were served of whom 53 went on to obtain permanent housing. OTHER PROGRAM SERVICES 9: Common CentsAn interim housing program which provides a short term place to live while people work on their housing barriers and then move on into their own permanent housing. Ready to Rent curriculum is taught, employment opportunities or disability benefits are obtained, and we assist in locating decent housing for them. During 2016, a total

IRS990/Desc0Friendship HousingFriendship Housing is a HUD funded program that houses chronically homeless individuals in Sacramento County in scattered shared housing sites with case management services. To be a client of this program one must have a disability and have been homeless for a year or four times in three years. Each house has a house leader to maintain order and security for the clients and the property. All clients have a case plan and are assigned caseworkers to assist them in obtaining their goals. The most typical goals are health care, increased income and personal relations and better mental health. SHA collaborates with other agencies to assist its clients in meeting their goals.Housing First is our model, in which we move people directly from the street or an emergency shelter into their permanent home. Intensive services are offered to help with mental and physical health issues, employment training, life skills training and alcohol and drug issues. We have over a 98% rate of continuance in permanent housing in this program which began in 2006 and in 2016 housed 94 people. Our average cost of housing chronically homeless individuals was $673 per month, which includes significant services assisting with mental, physical and behavioral health issues.

IRS990/ProgSrvcAccomActy2Grp/Desc0Housing ProgramsOur Housing resources program provides housing counseling and referrals to over 2,000 desperate households each year.
