Thompson Cancer Survival Center Foundation has one member, Covenant Health, a Tennessee nonprofit corporation. Covenant Health is the sole member of Thompson Cancer Survival Center, which is the supported organization of the Foundation.

The members of the board of directors, other than the ex officio members, shall be elected by Covenant Health from a slate of nominees nominated by the board of directors of the Foundation and recommended to the Member for election. The sole member, Covenant Health, has the exclusive right to vote on the election, appointment of, or removal of directors.

Endowment funds are intended to be used for the following purposes: - Radiation oncology staff education and training, - Radiation oncology research, - Laser center operating funding, - Cancer outreach services, and - Patient assistance. Part V, line 1c: The funds currently in the endowment funds are all temporary funds that have not been donor or board requested to earn interest. These funds are pooled with other Foundation funds and any amounts earned are considered unrestricted revenue.

Part X, Line 2:

Excerpt from the consolidated audited financial statements of Covenant Health (Covenant), parent company to Thompson Cancer Survival Center, the supported organization of Thompson Cancer Survival Center Foundation: Covenant and certain of its subsidiaries or controlled entities are exempt from income taxes pursuant to Section 501(c)(3) of the Internal Revenue Code. Accordingly, no provision for income taxes on qualifying activities has been made for these entities in the accompanying consolidated financial statements. However, certain entities and operations are subject to income taxes. Covenant had no unrecognized tax benefits at December 31, 2018. As such, no interest or penalties were recognized in the consolidated financial statements related to unrecognized tax benefits. At December 31, 2018, tax returns for 2015 through 2018 are subject to examination by the Internal Revenue Service. Covenant has no uncertain tax positions that would require financial statement recognition or disclosure under GAAP at December 31, 2018.

IRS990/Desc0Multidisciplinary Operating Grant: Thompson Cancer Survival Center Foundation provides grant support for the operations of the Multidisciplinary Treatment Department of the Thompson Cancer Survival Center. Multidisciplinary Treatment facilitates specialty physicians discussing treatment options based on the individuality of one's circumstances. A Multidisciplinary Nurse Navigator schedules a private consultation for each patient with the members of the specialty team. This approach affords the opportunity for the physician to relate one-on-one with the oncology patient and explain treatment choices in layman's terms. Patients in need of this support come from various Covenant Health facilities, as well as outside facilities. Nurse navigators and a department assistant currently organize, oversee, summarize, and provide follow-up for 4 to 5 multidisciplinary cancer conferences per week. A new monthly liver conference was recently created in partnership with the Vanderbilt multidisciplinary team. In 2018, 799 patients were presented at multidisciplinary conferences, with 145 of those patients being seen in our multidisciplinary clinics where a nurse navigator is in attendance with each patient to take notes, educate, offer support to patients and families, and become their liaison between all physicians and services.The nurse navigators assist oncology patients according to their needs and help them overcome multiple barriers to healthcare. Common barriers include financial, educational, transportation, logistical, linguistic, cultural, and emotional factors, as well as the complicated health care system in general. Our navigators guide patients to sources of financial support, help complete paperwork, educate patients on diagnoses and medical procedures, arrange and coordinate appointments with multiple providers, help with transportation, assist with housing, communicate with providers about unique patient needs, ensure medical records arrive at scheduled appointments, direct patients to support groups or counseling services, and attend patient appointments for support. Working with patients having all solid tumor cancer types, they work diligently to meet the needs of our patients and families. There were 7,368 oncology patient education encounters in 2018, and 485 patients were navigated with complicated issues. For those patients being followed by navigators that have curative treatment, over 200 Survivorship Care Plans were completed by Nurse Navigators and discussed with patients in 2018.Nurse navigators also led the Low Dose CT Lung Screening Program for Fort Sanders Regional and Fort Sanders West until a Registered Medical Assistant was recently added to the program. There were 512 total patients scheduled for screenings in 2018. All Low Dose CT patients are followed by the multidisciplinary treatment department.

IRS990/ProgSrvcAccomActy2Grp/Desc0Social Services Operating Grant:The Foundation provides grant support for the operations of the Social Services Department of the Thompson Cancer Survival Center. The Social Services Department receives referrals from Covenant Health facilities when oncology patients are identified that could benefit from the services provided by this department. In 2018, Social Services supported 1,368 new patient referrals and 4,001 follow-up patients. Additionally, there were 807 new patients referred for nutritional counseling and education and 3,040 follow-up patients were also supported. Healing Touch is another service provided by the Social Services Department. Healing Touch offers a space for patients to relax and escape from the stress of cancer treatment. Patients are seen for pain management and anxiety and find the treatment relaxing and helpful. The Social Services Department also offers Genetic Counseling services including breast cancer and hereditary cancer risk assessments. In 2018, the Genetics Clinic provided services for 360 new patients who were in need of counseling, including 143 who pursued genetic testing. Whether patients tested positive or negative for a hereditary cancer syndrome, decided not to pursue testing, or were not appropriate candidates for testing, they and their families were able to make informed decisions regarding their medical management based on the genetic counseling they received.

IRS990/ProgSrvcAccomActy3Grp/Desc0Clinical Trials Operating Grant:The Foundation provides a grant to the Thompson Cancer Survival Center (TCSC) for the operation of its Clinical Trials program. Since the opening of TCSC in 1988, the Clinical Trials Department has enrolled 2,377 patients in clinical trials. The TCSC Clinical Trials Department currently has 14 active treatment patients and 14 patients actively enrolled in registry, data collection and specimen research studies, and 139 patients in follow-up. In 2018, TCSC Clinical Trials Department enrolled 14 patients to oncology clinical trials. To help with screening efforts, Research Nurse Coordinators are assigned to specific physician schedules for screening and to specific locations for coverage, including Thompson Oncology Group's Oak Ridge, Sevierville, West Knoxville, Downtown Knoxville, and Lenoir City locations. Opening new studies increases referrals locally and regionally and gives current patients the ability to be treated with state-of-the-art care in their own community. Involvement in prevention, treatment, and data and specimen collection trials offers an improved means of health strategies for individuals at risk for certain cancers, as well as access to proven standard cancer treatments and new, developing cutting edge therapies for those diagnosed with cancer.

IRS990/ProgSrvcAccomActyOtherGrp/Desc0In addition to the program services detailed above, funding was also provided in 2018 for cancer education, prevention and early detection programming; direct patient assistance; esophageal cancer treatment equipment; and community-based patient and family support programs.Thompson Cancer Survival Center Foundation supports the work of Thompson's cancer outreach department which provides hundreds of free and reduced-cost cancer screenings annually in the East Tennessee area. Additionally, more than 1,000 people across 24 East Tennessee counties received skin cancer prevention and sun safety education, and hundreds of men and women participated in smoking cessation programs. Early detection and prevention efforts are the first step to help patients get diagnosed quickly or, if possible, never have to battle the disease at all. The Foundation's Thompson Cares program assisted more than 730 cancer patients in 2018 by helping with life's necessities such as gas and electric bills, medications, prostheses, housing, and more. Also, approximately 260 newly diagnosed patients were are provided with a comfort care basket filled with products to help them through the many and varying side effects of chemotherapy. The baskets include items such as Biotene products, lip balm, lemon hard candies, hand sanitizer, antibacterial hand soaps and wipes, a soft toothbrush, a turban, footies, books of encouragement and a journal to record their thoughts and experiences. In 2018, the Foundation supported the work of the Laser Center (now a department of Fort Sanders Regional Medical Center) with the distribution of donations restricted for the Laser Center's use to fund a new therapeutic gastroscope and a Digitrapper pH/Impedance monitoring system. Both pieces of equipment are used in the screening and treatment of esophageal cancer and precancerous conditions.
