Mission

Major Activities

Activity 2
Rehabilitation hospital: the rehabilitation hospital served 1,333 patients in fy11. Patient satisfaction continued to be very strong, with the hospital scoring in the 99th percentile of all other inpatient rehab hospitals for the likelihood to recommend our facility to others. The rehabilitation hospital expanded its medical staff with another employed rehabilitation physician and a geriatric nurse practitioner. While the hospital routinely admits patients 7 days/week, the capacity to increase weekend admissions became a focus this year and led to some changes in staffing for key areas such as medical staff, pharmacy and intake. The hospital was also fully accredited during its first annual site survey by the dnv's healthcare national integrated accreditation for healthcare organization (dnv/niaho).
Activity 3
Hospice: to provide a seamless continuum of care to our patients, we continued the integration of our hospice, palliative care, private duty and home health services this year. A significant step to integration is the migration to one electronic health information record, and this year hospice moved to the new cerner software. This electronic system supports and coordinates care as patients move between levels of care based on their need. Home hospice had an average daily census of 240 patients and our inpatient hospice facility had an average daily census of 19. This growth required the creation of a third clinical team for our home care staff to include and additional medical director. Our smaller teams allowed closer coordination and attending to patient needs.palliative care: our palliative care services expanded in several delivery sites. We continued to provide hospital-based care and nursing home care, while extending to homes and clinic services. We moved our program to use the cerner electronic medical record, another link in an effort to integrate our services and patient information. Carepartners also developed and implemented the first acgme approved hospice and palliative care fellowship program in western north carolina, beginning with two fellows.

THE PROGRAM WAS RECOGNIZED BY OCS HOMECARE AND DECISIONHEALTH AS ONE OF THE TOP 25% OF HOME HEALTH AGENCIES BASED ON AN ANALYSIS OF QUALITY OUTCOMES, QUALITY IMPROVEMENT AND FINANCIAL PERFORMANCE. THE HOME HEALTH PROGRAM CONTINUES TO ENHANCE THE CHRONIC PATIENT PROGRAM. ELEVEN OF OUR VISITING STAFF RECEIVED "HEALTH COACHING" TRAINING. THE ATTENTION TO OUR CHRONIC POPULATION DECREASED OUR RE-HOSPITALIZATION RATE TO 22% AS COMPARED TO THE STATE AND NATIONAL AVERAGE OF 29%. TO PROVIDE ANOTHER OPTION FOR EVALUATION OF SWALLOWING WITHOUT RADIATION, WE PURCHASED THE FIBEROPTIC ENDOSCOPIC EVALUATION OF SWALLOWING (FEES) EQUIPMENT. TWO NEW PRIVATE TREATMENT ROOMS WERE BUILT AT OUR EAST ASHEVILLE CLINIC TO ALLOW FOR MORE CONFIDENTIAL EVALUATIONS AND PATIENT/THERAPISTS DISCUSSIONS. ONE OF OUR THERAPISTS ACHIEVED CERTIFICATION IN INTRAMUSCULAR THERAPY OR DRY NEEDLING, WHICH IS A THERAPEUTIC TREATMENT FOR MYOFASCIAL PAIN. WE ALSO RELOCATED THE SEATING CLINIC TO PROVIDE A MORE SEAMLESS APPROACH TO PATIENTS AND CONSOLIDATE RESOURCES. OUR PATIENT OUTCOMES ARE IN THE TOP PERCENTILE FOR THE COUNTRY FOR FUNCTIONAL STATUS CHANGE. HOW WE USE OUR EXCESS REVENUES:WHILE CAREPARTNERS HAS A MINIMAL SURPLUS OF FUNDS MOST YEARS, EVERY DOLLAR IS REINVESTED IN THE ORGANIZATION TO CONTINUALLY IMPROVE THE QUALITY OF OUR SERVICES AND MEET THE COMMUNITY'S NEEDS.
