“Ottawa county human services coordinating council community spoke regularly convened a group of 40+ leaders in the health and human services space in ottawa county to discuss community needs, avoid duplication in services, and look for ways to collaborate. Ottawa county community health improvement plan community spoke was responsible for facilitating the second iteration of ottawa county's community health improvement plan (chip), building off the 2015 plan. The chip is required by hospital systems and local health departments and is designed to improve the health and well-being of residents through collaborative efforts. Community spoke plays the critical role of neutral convener and facilitator of the process. This plan included naming goals, objectives, and strategies for implementation for each of the community's three priority areas of mental health, healthy behaviors, and access to care. Ottawa food community spoke facilitated an updated strategic plan for a local food collaborative (ottawa food) focused on eliminating hunger in the community, encouraging healthier eating, and increasing the sourcing of local food. Community spoke also worked with them to help build sustainability for their programming.”

OTTAWA COUNTY COMMUNITY HEALTH IMPROVEMENT PLAN COMMUNITY SPOKE WAS RESPONSIBLE FOR FACILITATING THE SECOND ITERATION OF OTTAWA COUNTY'S COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP), BUILDING OFF THE 2015 PLAN. THE CHIP IS REQUIRED BY HOSPITAL SYSTEMS AND LOCAL HEALTH DEPARTMENTS AND IS DESIGNED TO IMPROVE THE HEALTH AND WELL-BEING OF RESIDENTS THROUGH COLLABORATIVE EFFORTS. COMMUNITY SPOKE PLAYS THE CRITICAL ROLE OF NEUTRAL CONVENER AND FACILITATOR OF THE PROCESS. THIS PLAN INCLUDED NAMING GOALS, OBJECTIVES, AND STRATEGIES FOR IMPLEMENTATION FOR EACH OF THE COMMUNITY'S THREE PRIORITY AREAS OF MENTAL HEALTH, HEALTHY BEHAVIORS, AND ACCESS TO CARE. OTTAWA FOOD COMMUNITY SPOKE FACILITATED AN UPDATED STRATEGIC PLAN FOR A LOCAL FOOD COLLABORATIVE (OTTAWA FOOD) FOCUSED ON ELIMINATING HUNGER IN THE COMMUNITY, ENCOURAGING HEALTHIER EATING, AND INCREASING THE SOURCING OF LOCAL FOOD. COMMUNITY SPOKE ALSO WORKED WITH THEM TO HELP BUILD SUSTAINABILITY FOR THEIR PROGRAMMING. |
