“Ottawa pathways to better health-community spoke played the role of convener and facilitator to help launch a new innovative model of healthcare in ottawa county, mi. Community spoke convened dozens of healthcare stakeholders over a period of six-months to identify a strategy to address the top health issues experiences in ottawa county (access to health, mental health, and obesity). Out of these meetings came the identification and design for "community health worker" model called pathways to better health. This model pairs individuals experiencing two or more chronic conditions with a lay-health person from the community to help them better navigate their healthcare and social service needs. Community spoke then convened an advisory group and raised money to launch a three-year pilot of this program (which hired 5 community health workers that began receiving referrals in february 2017). This program is expected to improve health outcomes for several thousand ottawa county residents during this three-year period. Great start collaborative -community spoke was contracted to help facilitate a three-year strategic plan on behalf of ottawa county's great start collaborative (early childhood collaborative). This plan identified clear objectives and strategies aimed at accomplishing the primary goals of the great start collaborative: 1.children are born healthy 2.children are healthy, thriving, and developmentally on track from birth to third grade. 3.children are developmentally ready to succeed in school at time of school entry. 4.children are prepared to succeed in fourth grade and beyond by reading proficiently by the end of third grade. Building an age-friendly holland/zeeland-beginning in the fall of 2016, community spoke began leading an effort in holland, mi called building an age-friendly holland/ zeeland. This effort aims to design a community where all people (with a particular focus on older adults) can age comfortably. Through this effort transportation was identified as a major barrier to the age-friendliness of holland/zeeland resulting in additional community planning intended to improve the transportation options for older adults.”

COMMUNITY SPOKE CONVENED DOZENS OF HEALTHCARE STAKEHOLDERS OVER A PERIOD OF SIX-MONTHS TO IDENTIFY A STRATEGY TO ADDRESS THE TOP HEALTH ISSUES EXPERIENCES IN OTTAWA COUNTY (ACCESS TO HEALTH, MENTAL HEALTH, AND OBESITY). OUT OF THESE MEETINGS CAME THE IDENTIFICATION AND DESIGN FOR "COMMUNITY HEALTH WORKER" MODEL CALLED PATHWAYS TO BETTER HEALTH. THIS MODEL PAIRS INDIVIDUALS EXPERIENCING TWO OR MORE CHRONIC CONDITIONS WITH A LAY-HEALTH PERSON FROM THE COMMUNITY TO HELP THEM BETTER NAVIGATE THEIR HEALTHCARE AND SOCIAL SERVICE NEEDS. COMMUNITY SPOKE THEN CONVENED AN ADVISORY GROUP AND RAISED MONEY TO LAUNCH A THREE-YEAR PILOT OF THIS PROGRAM (WHICH HIRED 5 COMMUNITY HEALTH WORKERS THAT BEGAN RECEIVING REFERRALS IN FEBRUARY 2017). THIS PROGRAM IS EXPECTED TO IMPROVE HEALTH OUTCOMES FOR SEVERAL THOUSAND OTTAWA COUNTY RESIDENTS DURING THIS THREE-YEAR PERIOD. GREAT START COLLABORATIVE -COMMUNITY SPOKE WAS CONTRACTED TO HELP FACILITATE A THREE-YEAR STRATEGIC PLAN ON BEHALF OF OTTAWA COUNTY'S GREAT START COLLABORATIVE (EARLY CHILDHOOD COLLABORATIVE). THIS PLAN IDENTIFIED CLEAR OBJECTIVES AND STRATEGIES AIMED AT ACCOMPLISHING THE PRIMARY GOALS OF THE GREAT START COLLABORATIVE: 1.CHILDREN ARE BORN HEALTHY 2.CHILDREN ARE HEALTHY, THRIVING, AND DEVELOPMENTALLY ON TRACK FROM BIRTH TO THIRD GRADE. 3.CHILDREN ARE DEVELOPMENTALLY READY TO SUCCEED IN SCHOOL AT TIME OF SCHOOL ENTRY. 4.CHILDREN ARE PREPARED TO SUCCEED IN FOURTH GRADE AND BEYOND BY READING PROFICIENTLY BY THE END OF THIRD GRADE. BUILDING AN AGE-FRIENDLY HOLLAND/ZEELAND-BEGINNING IN THE FALL OF 2016, COMMUNITY SPOKE BEGAN LEADING AN EFFORT IN HOLLAND, MI CALLED BUILDING AN AGE-FRIENDLY HOLLAND/ ZEELAND. THIS EFFORT AIMS TO DESIGN A COMMUNITY WHERE ALL PEOPLE (WITH A PARTICULAR FOCUS ON OLDER ADULTS) CAN AGE COMFORTABLY. THROUGH THIS EFFORT TRANSPORTATION WAS IDENTIFIED AS A MAJOR BARRIER TO THE AGE-FRIENDLINESS OF HOLLAND/ZEELAND RESULTING IN ADDITIONAL COMMUNITY PLANNING INTENDED TO IMPROVE THE TRANSPORTATION OPTIONS FOR OLDER ADULTS. |
